Unit Position Summary
Job Description
In a clinical-like setting (in-person or online), portrays cases similar to what learners would encounter in actual clinical settings. They may act as a Health Care Provider in a situation or be teaching a special task. Health Care Providers are expected to consistently and accurately teach and portray the tasks and cases. Health Care Providers may also provide feedback and/or assess learners using specific measurement tools. Participation in group or individual training is expected in order to prepare for case portrayal. Work hours are variable and on an on-call basis.

Responsibilities*
Under physician supervision, coordinates the care of adult patients admitted to the cardiac surgery service. Specifically, the role requires the clinician to:
* Manage pre- and post-cardiac surgery patients in a telemetry/step-down unit setting.
* Perform, order, and interpret advanced diagnostic and therapeutic tasks associated with patients undergoing cardiac surgery or procedures including but not limited to: coronary artery bypass surgery, valve replacement or repair, aortic aneurysm repair, myectomy, transcatheter aortic valve replacement (TAVR), heart transplantation, total artificial heart, and left ventricular assist device (LVAD) placement and revision.
* Complete and document detailed histories and physical exams; present pertinent data to the faculty.
* Order and interpret diagnostic lab tests, radiographic procedures, and clinical studies.
* Educate patients and their families on disease management, information and details regarding surgical procedures, post-operative instructions, and preventive care.
* Perform limited diagnostic, therapeutic, and minor surgical procedures such as removing temporary pacing wires, removing chest tubes, minor chest wound debridement, incision and drainage, placing wound vacs, and placing NG tubes.
* Discuss surgical procedures, obtain consent, and facilitate both necessary order entry but also effective interpretation of pre-operative testing.
* Maintain continuity of care throughout the Cardiovascular Center; provide team members of oncoming shift with thorough and accurate hand-offs.
* Confer with attending physicians and communicate with other care providers/consultants to coordinate clinical care, escalating care as needed.
* Maintain accurate documentation regarding patient status, evaluations, and treatment.
* Streamline the patient transfer process between levels of care, including writing discharge orders and communicating specific patient needs to team members.
* Participate in peer evaluations and quality-assurance activities, support research projects and any other initiative deemed necessary and in the best interest of patient care.
* Perform other related duties as required.
* Maintain an in-depth knowledge of medical principles, practices, standards, and techniques in accordance with Michigan Medicine policies and procedures, Medical Staff Bylaws, and State and Federal laws/regulations.
* Participate in activities that promote professional growth and self-development.

* Experience and expertise in interpreting imaging and other diagnostic studies, including chest x-rays, CT scans and electrocardiograms.
* Experience and comfort independently managing patients with Left Ventricular Assist Device (LVAD).
* Recent experience in performing Discharge Navigator, writing discharge summaries, and completing patient discharges.
* Experience writing succinct but thorough history and physical exams, progress notes, discharge summaries.
* Demonstrated experience in caring for patients across the continuum of care.
* Experience functioning as an independent provider in addition to contributing to a team environment, work efficiently, and experience independently managing a high volume of high acuity patients.
* Advanced understanding of cardiac surgery procedures, including an ability to explain procedures in detail to patients and their families.
* Demonstrated ability to order and interpret appropriate diagnostic studies; synthesize information and make informed clinical decisions independently; develop appropriate care plans, ability to be patient-focused and respond to patient and family preferences and needs.
* Demonstrated provider experience with heart transplant patients.
* Demonstrated knowledge of adult cardiac physiology and complex cardiac pathology with experience teaching cardiac physiology and pathology in a high level learning environment.
* At least one year of recent experience caring for acutely ill patients in a hospital setting.
* Demonstrated excellent verbal and written communication skills, organization, and problem-solving skills.
* Demonstrated high level of critical thinking.

The home health aide (HHA) provides home health services to patients in the home meeting all agency, state and federal regulations.
As a member of a multi-disciplinary home health staff, the HHA function as a team member for assigned individuals and families and ensures the provision of safe delivery of personal care following all department policies and state and federal regulations.

* Provides hands-on personal care as planned and supervised by the RN and/or case manager.
* Performs simple procedures as an extension of therapy or nursing services in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
* Assists in ambulation or exercises in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
* Assists in administering medications that are ordinarily self-administered in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
* Performs and documents observations and interventions in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
* Works with the Customer Service Representatives and Nursing Supervisors/Clinical Managers to schedule work time and plan assignments.
* Participates as an interdisciplinary team member by reporting changes in the patient's condition to a registered nurse or other appropriate skilled professional and completing pertinent records in compliance with the organization's policies and procedures.
* Communicates and interacts with patient/caregivers and families in a positive and effective manner.
* Encourages and assists patients and caregivers to achieve their maximum degree of independence.
* Participates in department development by sharing observations about individual and aggregate health care needs, department reputation, program planning needs, etc. to administrative staff of the department.
* Manages assigned patient caseload, meeting department productivity expectations.
* Provides care based on the Plan of Care developed by the RN, department mission, protocols and Patient's Bill of Rights.
* Completes all documentation according to department guidelines to ensure inclusion in the medical record within regulated time frames.
* Participates in the Quality Assessment and Performance Improvement (QAPI) program.
* Participates in organization sponsored in-service training.
* Patient and Family Centered Care:
* Actively engages the patient, patient representative (if any), and family by exhibiting respect and dignity, sharing information in a timely and unbiased manner, and welcoming and encouraging participation to achieve shared decision making.

Required Qualifications*
* High school diploma or equivalent
* Prior experience as a health aide, nursing assistant or medical assistant
* Ability to read, write and carry out directions
* Ability to demonstrate compassionate attitude toward care of the sick
* Ability to travel from point A to point B throughout the work day as assigned
* Successful completion of the pre-employment screening test
Physical Requirements:
The home health aide will be required to carry out assignments in patients' homes, health care institutions and HCS offices. The employee will perform patient care duties at the clients' homes during the work period. These duties include bathing activities, assisting and/or performing prescribed exercises, assisting with ambulating and transferring clients from one place to another, ADL assist, etc. The duties require the ability to lift up to 50 lbs on a regular basis and the ability to maintain stable balance while performing job duties, especially during tasks associated with patient ambulation and transfers. The duties also require the ability to reach, bend, push, pull, climb stairs, drive, and tolerate prolonged or considerable walking distances and/or standing for a great length of time. Must be able to walk on uneven surfaces in all weather conditions considering varying work site locations. Visual acuity and hearing is required to perform clinical tasks and skills. Employee may drive personal or University- owned vehicle to various work site locations on a regular basis including in-town and highway driving in good and inclement weather.
Desired Qualifications*
* Certified nursing assistant (Completion of a nurse aide program consisting of 75 hours of training and competency program)
* Prior home care experience with completion of home health agency nurse aide competency program
* One year of education from an accredited nursing program
* Excellent customer service skills
* Proficiency in email, communication and web based learning

Neuro Critical Care at Harborview Medical Center takes care of a patient population that includes Subarachnoid Hemorrhage, Intracranial Hemorrhage, Acute Ischemic Stroke, Seizure and Status Epilepticus, Isolated Traumatic Brain Injuries, Neuromuscular disorders, Brain Tumors, CNS infections, hypoxemic-ischemic encephalopathy, Spinal Cord Injury, and post-operative spine patients
PRIMARY JOB RESPONSIBILITIES
Perform patient care to include admitting new patients, participating in daily patient rounds, daily care evaluation and management of ICU patients, writing daily progress notes and formulating/implementing plans
Collaborate with acute care services (ortho spine, neurosurgery, and neurology services) to daily sign out patients leaving the unit
Use ICU Checklist on morning rounds to complete the daily checklist for each patient to ensure proper care and communication with nursing staff
Facilitate family meetings and participate in goal of care discussions as appropriate
Perform procedures including Central Venous Catheters, Arterial Lines, Bronchoscopy, Lumbar Punctures, Point of Care Ultrasound
Quality Improvement and Research including QI meetings and ongoing QI projects
REQUIREMENTS
If ARNP: Master's or Doctorate degree in nursing from an accredited school of nursing AND Washington State Advanced Registered Nurse Practitioner License with prescriptive privileges AND national certification as an Acute Care Nurse Practitioner
If PA-C: Licensure as a Physician's Assistant in the State of Washington
