Community Care Hospice is currently seeking a dedicated and skilled LicensedPracticalNurse (LPN) to provide high-quality hospice care in a home setting. This position plays a key role in supplementing the nursing care provided by the RN and ensures the delivery of patient-centered, end-of-life care with dignity, empathy, and professionalism.
Employment Type: Full Time | M-F 8am-5pm

Key Responsibilities:
Deliver direct patient care under the guidance of a Registered Nurse and in alignment with the patient's plan of care.
Monitor, report, and document changes in patient condition; collaborate with the interdisciplinary team.
Educate patients, families, and caregivers on care techniques and safety measures.
