Medical Records Clerks play an important supporting role to the staff and patients of Midland Community Healthcare Services. They are expected to provide excellent customer service while performing tasks such as compiling, processing, and maintaining medical records of the clinic patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. They also process, maintain, compile, and report for health requirements and standards.
JOB DUTIES
Protect the security of medical records to ensure that confidentiality is maintained in accordance with legal and regulatory guidelines.
Review records for completeness, accuracy, and compliance with regulations.
Retrieve patient medical records for appointments, medical personnel, physician review, and audit review.
Maintain charts for ease of use and understanding (clean/organize) before re-filing alphabetically.
Ensure that all patient information is filed in the medical record in order to maintain complete patient records.
Process and accept payment for patient record request for authorized persons, departments, facilities, or agencies.
Participate in cross-training opportunities in order to help other front office and medical records areas.
Pull charts for Rx and e-scripts requests and provide to clinical staff.
Pull charts for abnormal lab results and provide to clinical staff.
Maintain EMR message system and ensure that clinical staff has appropriate medical records needed.
Distribute all mail, interoffice correspondence, and faxes to correct personnel within the clinic.
Monitor printers for lab results, pull their corresponding medical records, and provide to clinical staff.
Process requests for medical records needed from other facilities, ensure medical records are received.
Retain records until the appropriate dates, then purge old records, deceased patient records, and destroy records past the retention date according to legal and regulatory guidelines.
Update patient records as required.
Send patient correspondence as required through clinic procedure.
Maintain clinic office supplies through the proper procedure.
Performs other duties as assigned by the Clinic Manager.

JOB SUMMARY
Care Managers play an important supporting role to the patients and physicians of Midland Community Healthcare Services. They are expected to lead the patient through their healthcare needs by providing excellent customer service while assessing, implementing, and evaluating their individual cases. They are responsible for maintaining accurate and complete medical records of appropriate patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system.
JOB DUTIES
Observe, interview, and asses referred patients to identify care needs.
Monitor care/case managed patients in collaboration with other health care professionals.
Be knowledgeable of current trends in nursing and case/care management, network with colleagues; participate in professional organizations or conferences to keep abreast with changes.
Augment providers plan of treatment for patients by encouraging the patients to set achievable goals for themselves. Thus making the plan of treatment successful.
Follow the guidelines of the Board of Nursing when evaluating standards of nursing practice in specialty areas such as pediatrics, geriatrics, obstetrics and gynecology.
Assists patients in goal setting based on information gathered by observing, and interviewing patients, or by monitoring or analyzing their records.
Make recommendations physicians, other health care providers, insurance companies, patients, or health care organizations to improve patient care.
Identify training needs or conduct staff training sessions as needed in regards to care management.
Assist in leadership opportunities related to clinical staff under the direction of Clinic Manager.
Assist management in implementing initiatives vital to the operation of MCHS and the success of its mission.
Educate patients with regard to treatment procedures, medications, diets, and providers instructions related to care/case management
Follow up on patient calls, lab results, referrals, prescription request and questions related to Care/Case management.
Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate care plans related to Case/Care management.
Schedule patient referrals and educate patient as to why a referral is needed as well as the referral process. This is collaboration with Provider and Referral Coordinator.
Review records for completeness, accuracy and compliance with company, state, and federal regulations.
Assist in quality improvement, auditing, safety, and compliance measures of the organization.
Follow Primary Care Medical Home standards in relation to patient care and interaction.
Performs other duties as assigned by the Clinic Manager.

JOB DUTIES
* Interview patients in order to obtain medical histories, vital statistics, weight, height, blood pressure, temperature, and obtain any information such as test results in order to place them in the medical record.
* Review records for completeness, accuracy, and compliance with regulation.
* Prepare treatment rooms for patient examinations, keeping the rooms neat and clean.
* Clean and sterilize instruments and dispose of contaminated supplies in accordance with OSHA and clinic policy.
* Participate in cross-training opportunities in order to help other departments as needed.
* As directed by physicians educate patients with regard to treatment procedures, medications, diets, and physician's instructions.
* Collect blood via finger stick, collect urine, fecal, cultures, and other laboratory specimens, log the specimens, and prepare them for laboratory testing through the proper vendor procedure.
* Assist physician as needed in examining and treating patients during procedures, ensuring that proper tools and supplies are available.
* Maintain patient charts in the electronic medical record system, while ensuring accuracy of the patient record.
* Update patient records as required.
* Send patient correspondence as required through clinic procedure.
* Maintain clinical supplies through the proper procurement procedure.
* Assist in maintaining proper stock and inventory of medical supplies.

They are expected to provide excellent customer service while performing tasks such as compiling, processing, and maintaining medical records of the clinic patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. They also process, maintain, compile, and report for health requirements and standards.
JOB DUTIES
* Protect the security of medical records to ensure that confidentiality is maintained in accordance with legal and regulatory guidelines.
* Review records for completeness, accuracy, and compliance with regulations.
* Retrieve patient medical records for appointments, medical personnel, physician review, and audit review.
* Maintain charts for ease of use and understanding (clean/organize) before re-filing alphabetically.
* Ensure that all patient information is filed in the medical record in order to maintain complete patient records.
* Process and accept payment for patient record request for authorized persons, departments, facilities, or agencies.
* Participate in cross-training opportunities in order to help other front office and medical records areas.
* Pull charts for Rx and e-scripts requests and provide to clinical staff.
* Pull charts for abnormal lab results and provide to clinical staff.
* Maintain EMR message system and ensure that clinical staff has appropriate medical records needed.
* Distribute all mail, interoffice correspondence, and faxes to correct personnel within the clinic.
* Monitor printers for lab results, pull their corresponding medical records, and provide to clinical staff.
* Process requests for medical records needed from other facilities, ensure medical records are received.
* Retain records until the appropriate dates, then purge old records, deceased patient records, and destroy records past the retention date according to legal and regulatory guidelines.
* Update patient records as required.
