Job Description Join Our Team - and make a difference in the lives of others! Job Summary: The Service Coordinator fulfills all duties of the Designated Coordinator in a Rule 245D program licensed by the MN Department of Human Services. This position provides case management services, direct care and instruction for adults with developmental and other disabilities in a day training and habilitation program. Duties include: Working directly with persons in the program throughout the day and providing leadership/work direction to other program staff to ensure that individualized plans are properly implemented and that people served in the program receive quality services. Performing all aspects of internal case management, developing individualized program plans, acting as a member of the support team for persons served, acting as the liaison with each participant's family and residential provider. Planning and implementing individualized and small group activities within the center and the community. Working closely with the Program Supervisor and Program Director to ensure quality services and adherence to all program and regulatory standards. Hours, Benefits: 7:30 AM to 4:00 PM, Mon- Fri; full time. No evenings or weekends. Competitive wages. You must also have a valid driver's license, a clean driving record and be insurable under the standards set by our insurance company. Candidates must pass a drug screen and criminal background screening as determined by DHS and MSS. Must be able to frequently stand, bend, stoop and reach and push manual wheelchairs used by adults. MSS is looking for a candidate who has experience with preparing and processing payroll and processing Accounts Payable functions. Experience working with Paylocity to process payroll is a plus. We are looking for a candidate who will take ownership of the process and is continuously looking for opportunities to add value and make improvements. Accurate record keeping skills and demonstrated ability to work with spreadsheets along with the ability to complete reconciliations is key to success in this position. Review and maintain files to make sure they are up-to-date and contain all required materials (i.e. Progress Review, Outcome Statement Plan, Support Plan Addendum, Self-Managed Assessment, Individual Abuse Prevention Plan and signed policies and procedures.). All staff are expected to create and file their own documents pertaining to individuals supported. These documents are to be saved within the required online database. Submit weekly billing, mileage, expenses and timesheet to the manager in a timely manner. Maintain proper communication with the Employment Services Manager and the assigned Team Lead; ask for assistance in developing or implementing program planning as needed. Participate in staff performance reviews with the Employment Services Manager Attend and participate in regular staff meetings Adhere to company policies and procedures at all times. The case manager assesses the individual's changing needs, develops a plan in collaboration with other service providers, links the individual to appropriate community resources, and monitors the effectiveness of planned services. Qualifications A minimum of 2+ years of supervision or demonstrated leadership role in a human service field related to adult disability services. Must also have: (a) A four-year degree in related area of study and one year of work experience with persons with disabilities; or (b) A two-year college degree in related area of study and two years of work experience with persons with disabilities. Candidates must have strong written and verbal communication skills, demonstrated leadership skills and the ability to work both independently and cooperatively with coworkers and supervisors. Must have strong customer relations skills and relate well with people with disabilities, coworkers, supervisors, funders, family members and other rehabilitation professionals. Job Summary: The Service Coordinator fulfills all duties of the Designated Coordinator in a Rule 245D program licensed by the MN Department of Human Services.
