Major Responsibilities:
Performs venipuncture and capillary phlebotomy procedures on patients using appropriate specimen collection equipment and techniques.
Review requisition and orders from physicians to ensure appropriate tests are drawn and tests meet guidelines for Medical Necessity. Prepares Advance Beneficiary Notice (ABN) for tests that do not meet Medical Necessity guidelines and obtains appropriate signatures from patient and self.
Performs EKG, pulse oximetry and point-of-care testing procedures as requested and associated quality control and maintenance of instrumentation.
Processes specimens including centrifugation, pour-off, aliquoting, labeling, and packaging.
Provides essential patient data and insurance information for registration of specimens. May create registration for outreach specimens in Hospital Admission/Discharge/Transfer (ADT) system.
Searches master patient index (MPI) for appropriate patient demographic information using Metricom or other Laboratory Information System as provided.
Logs specimens onto log sheets and assigns accession numbers to specimens and requisitions received.
Labels blood and other specimens for testing according to hospital and laboratory requirements.
Labels and packages specimens for transport to laboratory.
Performs assigned duties to ensure an adequate inventory of supplies for assigned area. Monitors inventory levels and stores equipment and supplies in a neat and orderly manner. Submits orders to replenish inventory.
Maintains accurate records of processes performed, using manual and computer-based systems.
Monitors and records refrigeration temperatures for refrigerators used to store supplies and/or specimens.

High School graduate with successful completion from a recognized program for training in phlebotomy.
Basic life support certification required within 3 months of employment, and maintenance of annual competencies in BLS.

CLERICAL DUTIES:
-Filing appropriately in a timely manner.
-Answering phone requests from floors, Windemere, or Physician offices.

-PHLEBOTOMY DUTIES:
-Recognizing appropriate collection tubes for requested testing.
-Good Venipuncture technique.
-Asking for help when venipuncture is missed.
-Good capillary collection technique, i.e., fingerstick, heelstick.
-Good pediatric venipuncture technique.
-Recognition of appropriate amounts to be drawn for requested testing, including microsampling.
-Looks up “send out” test requirements and processes the specimens appropriately.
-Communicating to testing personnel “URGENT PRIORITIES”, i.e., STAT, ASAP, or call results.
-Proper administration of Glucola, Xylose for tolerance/challenge testing.
-Re-stocking phlebotomy room and participating in inventory control.

-Demonstrates ability to serve patients in various age groups, pediatrics to geriatrics.
-TECHNICAL DUTIES:
-Performs Simplate Bleeding Time tests.
-Performs all duties to support the testing personnel as allowed by CLIA 88.
-Trains and guides new staff members.
-Performs capillary INR testing.
-REGISTRATION DUTIES:
-Ensures patient confidentiality prior to interviewing patient.
-Obtains all relevant information and enters into Epic.
-Ensures that diagnoses appear on laboratory requisition.
-Compiles and quantitates total lab outpatient volume at the end of each day.
-Performs insurance verification for all appropriate patients.
-LEAD DUTIES:
-Plans activities with the Laboratory Manager in direct support of patient care requirements.
-Provides support to other phlebotomy technicians in the department.
-Troubleshoots and assists the Laboratory Manager in resolving equipment problems and failures.

Education Requirements:
Minimum: High School diploma
Preferred: Successful completion of an accredited phlebotomy program.
Preferred: CNA, LPN, RN and/or appropriate healthcare certification that requires formal schooling.

Summary
The Phlebotomist is responsible for receiving patients, gathering patient registration information, ordering tests, generating specimen labels, and escorting patients to phlebotomy services. The Phlebotomist performs phlebotomy on all patient populations and is responsible for performing a variety of semi-technical and clerical tasks involving specimen receiving, sorting, data entry, record retrieval, filing, typing envelopes, and responding to telephone inquiries for results, testing, general information, and routing of other calls.

This position reports to the Phlebotomy Supervisor and operates within established organizational and departmental policies and procedures.

Essential Functions
Obtain blood specimens by capillary or venipuncture technique using appropriate technique from patients of any age or population.
Follow proper procedures for special collections.
-Demonstrates commitment to patient and departmental needs and goals through flexibility in scheduling and teamwork.
-Consistently demonstrates teamwork with coworkers and support staff by participating in department problem-solving, defining group concerns, proposing and implementing solutions and acting in a positive and proactive manner.
-Competent in ordering process for outpatients.

-Respond to inquiries from physicians, patient floors, specimen collection sites and other outside agencies for verbal test results, lab orders, general information and printed copies of reports.
-Track down missing information and communicate with doctors' offices to clarify conflicts in registration information.
-Input patient's laboratory orders into LIS, prioritizing correctly according to urgency and ensuring accuracy of orders and physicians needing copies.
-Understand and act on the rules of medical necessity.
-Review standing orders and request renewals from physicians for pending expirations.
-Distribute kits and written instructions on the proper specimen collection for all types of specimens including stool, sputum, urine, and 24-hour urine containers.
-Receive and process specimens.
-Sort, correct, enter, receive, bar code, and label specimens in the Lab Information System (LIS) from the Emergency Department, Outpatient Laboratory Department, specimen collection sites, and outpatient clients (nursing homes, VA, visiting nurses, etc.) while exercising appropriate universal precautions.
-Process requisitions from laboratory information system according to specific procedures.
-Coordinate distribution of confidential test result reports to physician offices, patient floors, specimen collection sites, and outside agencies.
-Print and distribute copies of laboratory reports to patients with proper identification fulfilling “Patient's Right to Know” law.
-Maintain and rotate inventory/stock items.
-Order stock weekly or as needed.
-Perform necessary and required maintenance and daily quality control.
-As required, participate in weekend, holiday and on-call assignments.
-Maintain complete confidentiality.
-Comply with OSHA regulations.
-Maintain established hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection control and environmental standards.

Job Summary
Under general supervision of the Supervisor of Clinical Support Services, the individual collects specimens for laboratory analysis including blood, urine and nasal swabs. Explains safety protocols and procedures to patients, is customer focused and provides certain clerical functions.

-Tie tourniquet and prepare site for venipuncture
-Collect specimen according to order of draw

-Review prior to collection a standard checklist which includes checking for potential adverse reactions such as history of fainting, and latex allergy.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

3. Demonstrates the skills and competence necessary to perform the venipuncture/stick.
Selects the best area available for blood drawing.
Checks the wound site before leaving the patient.

POSITION SUMMARY:
The Phlebotomist is responsible for blood collections; obtaining urine and other non-blood specimens; processing specimens; maintaining lab log book; maintaining lab supplies; and distributing lab related documents.

• Participates in the overall optimal functioning of the health center.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
PROFESSIONAL BEHAVIOR:

TASKS:
• Accomplishes accurate and timely completion of tasks and procedures.
• Obtains specimens from patients. Instructs patients on procedure for specialty testing.
• Properly labels specimens and completes necessary forms.
• Positive ID of all patient by asking patient to state their full name and DOB
• Bag specimens safely and appropriately for transfer to reference lab.
• Maintains log book for tests ordered and results received.
• Assures that lab results are received in the patient's chart within three (5) business days, (except for special testing which may take longer)
• Follows up on any missing reports or results.
• Uses and maintains laboratory equipment appropriately.
• Acts as liaison for internal and external lab issues.
• Responds to phone calls related to the lab except for anyone requests for results (refer to nursing department).
• Documents accurately in-patient record as necessary.
• Maintain neatness and cleanliness in lab area.
• Interprets as needed.
• Preceptor for students.
• Ability to adjust to changes

• Quality Assurance Monitoring - participates in the betterment of the health center through studies and reviews as necessary.

WORKING CONDITIONS:
Works in a busy community health center in a downtown business district. Long hours are sometimes required. Covering other facility locations is required.
PHYSICAL REQUIREMENTS:
Physical demands requiring lifting small pieces of equipment, boxes, and/or patients not to exceed 40 pounds. Mostly standing and walking required. Visual acuity sufficient for frequently reading. Hearing acuity sufficient for holding conversation with or without audio devices.
Qualifications
QUALIFICATIONS:
• Graduate of a phlebotomy training program.
• State or National certification in phlebotomy required.
• Bilingual preferred.
MINIMAL KNOWLEDGE BASE:
• Has good understanding of phlebotomy skills.
• Understands common laboratory tests/ procedures
• OSHA guidelines.
• Infection control standards.
• Maintains current information related to laboratory issues.
