Responsibilities: Under general supervision and medical direction performs respiratory care related procedures. Exercises considerable judgment in accordance with physician's treatment plan and within the scope of departmental policies and procedures. Scope of practice includes patient/family assessment and management, resource management, care facilitation, and discharge planning as required. Assesses resident's condition, administers prescribed therapy and aerosolized medications, observes and records resident's progress and response to therapy modalities including adverse reactions to drugs and therapy. Provides in-service training to residents, family members, and facility staff. Resident population is primarily adult and geriatric. Initiates, monitors and maintains life support systems for residents in the skilled nursing facility.

Manages artificial ventilator status as directed by an approved protocol.
Maintains and troubleshoots mechanical ventilators.
Participates in cardiopulmonary resuscitation procedures of bag ventilation and closed chest massage.Initiates, monitors and documents performance of various respiratory care modalities.Performs assessment of residents' cardiopulmonary status utilizing physical assessment and history taking skills, laboratory findings, radiological studies of the chest, pulmonary function testing and professional judgment to formulate a respiratory care plan upon the order of a physician.Performs various tasks prior to treating a resident as follows:
Check physician's orders for completeness.
Orders to include: diagnosis, ICD -10 code, type and duration of therapy, type and dosage of medication, FI02 and frequency.
Check resident's chart for history, physical and for pertinent information to determine precautions to be observed at bedside.
Observe and chart: pulse, respiration, breath sounds (number and quality), and resident's general appearance, including breathing exercises and teaching and training to residents.
Notify supervisor of all new starts and discontinued residents.Administers proper technical respiratory care procedures. Dispenses proper dosage of medication, acquires and adjusts devices to insure optimal results from therapy. This includes mask, mouthpiece, or Trach equipment for resident's comfort and ease of operation, regulates flow and pressure of equipment. Observes resident's respiratory rate and pattern, pulse, breath sounds, reaction to medication, any difficulty with therapy during and after treatment. Chart and communicate to the resident's nurse any pertinent information. Observe thorough and accurate charting technique.Inspects and tests equipment, disassembles, cleans, reassembles equipment as required. Maintains adequate supply of necessary equipment. Performs administrative duties and record keeping when required. Disposes of equipment properly.Assists in emergency cardio-pulmonary resuscitation as an integral part of the emergency team. Therapist is required to be CPR certified.8. Initiates, administers, evaluates response to, and documents all therapy modalities according to departmental procedure and ordered by a physician including: intermittent positive pressure breathing, hand held nebulizer, chest physical therapy modalities, including percussion, vibration, breathing and cough retraining exercises, deep tracheal suctioning, incentive spirometry, sputum induction.Recognizes resident's adverse reaction to therapy modalities and takes correct action.Documents all resident care services rendered as part of a medical record.Accepts and transcribes verbal orders related to respiratory care procedures.Sets up, maintains and records log entries on all types of oxygen and respiratory supplies and equipment as required.Instructs residents and family members in the rationale and methodology of every respiratory procedure administered to each resident to facilitate resident cooperation, understanding and efficiency of treatment. Responsibilities
What You'll Do:
The Nurse Practitioner or Physician Assistant will direct, coordinate, and provide medical/clinical services to specified individuals within the facility, including, but not limited to intervention, psychiatric evaluation and assessment, differential diagnosis, treatment planning, medication management, engagement, family meetings, supportive psychotherapy, individual education, risk assessment, and discharge planning.
* Serves as the attending clinician to specified individuals. Completes initial psychiatric assessments, crisis interventions, family interventions, focused crisis diagnostic assessments, assessments of capacity for consent to treatment, risk assessments, individual education, progress toward treatment goals, discharge planning including medical, psychological, social, behavioral and developmental aspects of the individual's care, treatment, and services.
* Documents individual's progress in the clinical record including admission orders, progress notes, and discharge orders. Oversees delivery of medication education and consent, according to established policy.
* Orders laboratory tests according to standards of clinical practice. Reviews the health assessment completed by nursing staff, obtains a past and current medical history, and conducts a focused physical examination as indicated, according to established guidelines.
* Initiates contact with Primary Care Physicians and Emergency Room clinicians according to established guidelines.
* Assesses individual's needs and guides the clinical team while managing treatment plan updates, in addition to initiating contact with Outpatient Behavioral Health Provider according to established guidelines. Reviews and utilizes all available documents from the crisis clinical team.
* Provides necessary information to clinical team regarding inpatient utilization. Utilizes appropriate medication based on best practices.
* Collaborates with other personnel to ensure that each treatment modality is appropriate for the individual in addition to working with outside psychiatric providers in crisis and inpatient services. Works with the community to review involuntary petitions, assess involuntary patients, and coordinate with the legal system to follow established guidelines for Involuntary Evaluation and Treatment.
* Participates in medical staff meetings including regional and quarterly meetings; participates in all required meetings; and participates in any meeting as requested by Medical Leadership.
* Performs all other duties as assigned.
Qualifications
What You'll Bring:
* NP: Unrestricted Registered Nursing license in the State. Completion of an accredited Nurse Practitioner program with a Master's level certification in Psychiatric Mental Health (PMHNP).
* PA: Completion of an accredited Physician Assistant program.

About the Medical Scribe/Clinical Information Specialist (CIS) Position
A medical scribe is an active and valuable member of a healthcare team who complete real-time charting for physicians by shadowing them throughout their shifts and operate under the direct oversight of the physician, with the physician reviewing and approving actions to be taken at key junctures during patient care. In doing so, medical scribes minimize the doctor's clerical workload and maximize their ability to devote attention to highest quality patient care.
As a medical scribe you will receive hands-on experience and an in-depth understanding of the medical environment, terminology, and fundamentals of medical providers. This role provides an opportunity to make a positive contribution in the lives of both patients and healthcare providers while also creating a career path with advancement opportunities in healthcare, both with CIS PA and in the medical field at large. No clinical experience is required to apply and receive PAID training to become a Medical Scribe.

Job Responsibilities (General):
As a CIS you will be responsible for accurate documentation, data collection, and retrieving patient information the physician will need to make the best evidence based clinical decision. This will include:
Following a physician through the clinical shift, charting patient encounters in real time using Electronic Health Record software, and other templates.
Providing assistance with the clerical activities of doctors and mid-level providers.
Being under the direct oversight of the physician.and mining data for all needed comparisons and decision making.
Gathering laboratory results, radiology reports, medical records, and other data for review by the physician.
Independently obtaining past medical history, review of systems, social, and family history.
Gathering data for all medical encounters .
Preparing discharge paperwork and/or prescriptions.
Serving as a customer service liaison with clinical end-users.
Preparing plans for follow-up care.
Requirements:
Punctuality
Organizational and time management skills
Ability to follow and observe a physician for extended periods of time
Ability to navigate a stressful and fast-paced environment
Ability to read, write, and comprehend through listening
English fluency - (secondary languages may be required at some locations)
Ability to operate a computer and/or laptop through proficient typing, clicking and viewing a monitor for extended periods
Fluid typing skills
Detail oriented with proven ability to work effectively under conditions requiring accuracy
High motivation, experience-driven personality
Though CIMS will fully train you to become a CIS, *optional*, preferred education and volunteer/work experiences include:
Pre-health track (pre-med, pre-PA, pre-nursing) or interest in human relations
Biology coursework at the undergraduate level or above
Anatomy and Physiology and Medical Terminology coursework
Work, volunteer or shadowing experience in the healthcare field; Experience in a direct patient care environment preferred, i.e. Emergency Medical Technician, physician shadowing, clinical technician in a hospital, or a medical assistant in a doctor's office, etc.
