Complete supper meal preparation as directed from Executive Chef or Cook.
Assist Resident Associates in preparing for serving of supper meal, including setting tables, making coffee, pouring ice water and milk, setting salad on table.
Function in Position #3 in kitchen (includes coordinating dishing up of meal plates for servers and dishwashing.)
Ensure special diets and alternate menu selection requests are followed.
Follow sanitation, infection control, and safety procedures.
Prepare and/or deliver meal tray to resident apartments.
Handle food leftovers per defined procedure.
Complete dishwashing of all meal prep pots and pans and supper dishes.
Complete kitchen cleaning tasks as assigned.

In this dynamic role, you will collaborate closely with dedicated colleagues to deliver exceptional care and empower our members to thrive in their daily lives. As a Case Manager, you will provide comprehensive social service care management to frail elders and adults with intellectual, developmental, or physical disabilities who qualify for Wisconsin's FC program. Bring your compassion and expertise to help members access vital resources, promote independence, and enhance their quality of life within their communities.
Key responsibilities:
Assess members to identify their strengths, interests, and preferences, focusing on health and safety needs to develop a comprehensive Member Care Plan (MCP).
Coordinate services that address members' health and safety needs, ensuring support is provided in the least restrictive environment in accordance with the MCP.
Collaborate continuously with a Field Care Nurse (RN) to review and update care plans and address members' evolving needs.
Conduct face-to-face social assessments with members upon enrollment and at minimum, every six months, typically at the member's residence.
Schedule, conduct, and document quarterly in-person visits and maintain monthly contact with members by phone.
Identify, arrange, and monitor support services for members, including those related to social integration, community resources, employment, housing, and other non-medical needs.
Engage in clear and empathetic communication with members to better understand their needs, support informed decision-making, and ensure cost-effective service delivery.
Prioritize safety by continuously evaluating risk factors and providing education to members to promote overall health and wellness.
Maintain accurate and timely documentation of activities, including case notes, service authorizations, and updates to the Member Care Plan.
Foster direct collaboration with service providers, natural supports, and other community partners to enhance member outcomes.
Travel is necessary to conduct member visits and fulfill role responsibilities.

· Assists in maintaining a safe, neat and clean environment; reports environmental deficiencies to the charge nurse such as lighting or equipment problems.
· Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items.
· Observes residents for changes in condition or behavior and promptly reports these changes to appropriate licensed nursing personnel.
· Provides care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas.
· Answers residents' call bells promptly and courteously.
· Performs various tasks assigned by the charge nurse, including checking vital signs, weighing residents, applying creams/ointments and collecting specimens.
· Assists with orienting residents and their families to the nursing home on admission and to the unit when transfers occur.

Provides direct bedside care, and restorative care such as ambulation and range of motion.
Administers medications and treatments as prescribed by the physician, assessing responses, documenting in medical records, and reporting adverse reactions or change of condition to Nurse Leader, Physician, and family/guardian.
Counts all controlled substances on unit with on-coming Staff Nurse at shift change, and destroys outdated and discontinued medications in a timely manner.
Verbally communicates residents' acute conditions and instructions for follow through at shift change.
Completes 24 Hour Report of residents' acute conditions.
Reports all infections to Infection Control Coordinator/Nurse Leader and sets up precautions as needed.
Assists physician, as delegated by the RN, during acute rounds, chart reviews and medical record reviews.
Transcribes physicians' orders.
Completes Minimum Data Set (MDS), updating residents' Care Plan as necessary.
Responds to code situations, assessing residents' critical needs and taking proper steps to resolve the situation.
Acts as lead person, assigning and reviewing work of Nursing Assistant, and resolving work problems.
Monitors residents during meals in dining room.
Assembles and uses equipment such as catheters, tracheotomy tubes, oxygen suppliers, etc.
Records food and fluid intake and output, and collects samples from residents for testing.
Updates nursing care plans as delegated by RN.
Assists with and participates in social activities of resident's choice which provide meaningful ways to spend time such as recreation, exercise, relationship building and fun.
Supports maintaining a clean safe environment.
Respects the resident's room as home.
Participates in team decisions and completes tasks as a team.
Maintains regular and predictable attendance, works overtime/extra hours as required.
Checks e-mail daily.
