Responsibilities
Mental Health Worker Inpatient: Under the direction and supervision of the RN, LVN or Charge RN assist the licensed nursing staff in providing and documenting patient care including assistance with activities of daily living. Supports the unit functions and maintains a safe and clean environment for patients, visitors and co-workers; functions as an integral part of the interdisciplinary care team and communicates pertinent patient information to the supervising RN. Performs some clerical duties as assigned and provides basic nursing care for the adult and geriatric patients. Performs responsibilities within the guidelines of the Nursing Structure Standards, Hospital/Nursing Policy and state regulatory and accrediting requirements.
Mental Health Worker Outpatient: Under the direction and supervision of the Director of Nursing, RN and the Program Director carries out activities of daily living program and individual treatment plans for the psychiatric patients.

Current and valid license as a Certified Nursing Assistant issued by the State if a CNA, preferred. Current BLS upon hire and maintain current. One (1) year prior mental health experience preferred. Familiarity with medical and psychological terminology preferred.

Reviewed patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Insurance and Financial Assistance:
Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Understands FMCNA financial policies and procedures as they related to patient services. Collaborate with Financial Coordinators to educate patients about programs that may be available to assist with uncovered services, how to resolve billing concerns, and understand financial responsibilities.
Staff Related:
Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
Develops and provides training to staff pertaining to psychosocial topics as needed. Trains staff in Ultra Care Programs as needed.

Employee will work collaboratively with community mental health providers and community support services to develop, provide and support a continuum of care for patients.
Case Management and Treatment Coordination: The employee will work collaboratively with the primary care team to provide referral, case management and follow up services for patients with behavioral health needs. This will include engaging patients, initiating referrals, addressing barriers to care and follow-up. Employee should be familiar with community resources to address patient psychosocial and behavioral health needs.
Documentation: The employee will provide accurate and timely documentation in Epic, Agility or other authorized electronic health records to support the identification and treatment of patient's psychosocial and/or behavioral health needs. Will provide assistance in capturing program evaluation and fidelity measures as directed.
Quality Improvement: The employee will work collaboratively to assist in monitoring the behavioral health program and identifying areas for improvement. Employee will participate in meetings and quality improvement activities, continuing education, community education and serve as a member of committees, or related activites, as directed by supervisor.

Responsibilities include, but not limited to:
Collaborate with the treatment team, interdisciplinary team, and Directors as needed.
Provide individual and group sessions.
Help support patients with their issues, concerns, and expectations to reach their goals.

Complete documentation within appropriate guidelines using our electronic medical/health system.
Take responsibility for professional development and meeting licensure requirements to maintain credentials through attendance at relevant workshops and seminars.
Qualifications:
Experience working with a geriatric population is a plus.
History of treating Anxiety, Depression and Adjustment Disorders.
Previous clinical experience in Skilled Nursing, Hospital, Hospice, or Home Care setting is a plus.
Excellent time management and organizational skills.
