This position supports individuals as they navigate mental health challenges, build resilience, and develop the tools needed for lasting wellness. At CenterPointe, this person will work alongside a multidisciplinary team to provide compassionate, evidence-based care in a safe and healing environment. Hourly rate is in the $30.00 and up range!

ESSENTIAL FUNCTIONS:
Apply psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral health disorders.
Conduct psychosocial assessment and develop an initial discharge plan.
Conduct individual, group or family sessions as appropriate for the treatment plan of the patient.
Provide initial treatment planning for patient based on assessment and coordinate any additional services needed and revise as necessary.
Maintain documentation of therapy services provided to each patient, according to facility and professional standards and revise as needed.
Maintain a caseload as assigned.
Participate in treatment team meetings and collaborate with other staff to facilitate services for the patient as appropriate to their treatment plan.
May provide these services in an inpatient or outpatient setting.
Provide case management, discharge planning or after-care planning according to treatment plan.
May assist in educating and training staff on behavior management or therapeutic crisis intervention.
May coordinate various clinical and psychosocial or educational activities with patient and their families.
Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

ESSENTIAL JOB FUNCTIONS
* Greets all new admissions and their family members.
* Completes admission assessments timely and accurately.
* Assists staff with assessing the level of care required and helps residents and families accept the new level of care.
* Performs crisis intervention with problems such as alcoholism, death, depression, anxiety and loneliness.
* Gives direct input to resident plan of care by participating in care plan meetings and formulating social service goals for each care plan.
* Works with residents in a counseling situation to help meet their psychosocial needs.
* Participates actively in discharge planning and serves as a referral source for meeting basic needs of resident returning to a different level of care/facility.
* Assists residents and their families with discharge planning and offers assistance in the form of various support services.
* Keeps abreast of current legislation, trends, resources and facilities related to the field and fulfills all requirements set forth by Federal/State regulations and laws.
* Performs accurate coding of MDS.
* Adheres to all safety regulations and requirements.
* Handles stress well and gets along with others within the workplace and with customers outside the department.
