Job Description
1. Coordinates team based care to provide health services to individuals, families and/or their communities through effective partnerships with patients, their care givers and their physician. (ARACO, PCHH care coordination)
2. Managing transitional care patients post discharge for needs.
3. Educating patients about their disease process & medications.
4. Prepares reports, graphs, and statistical data for reporting thru committee structures.
5. Collaborates with physicians, nurses, and other health care personnel to ensure effective plan of care.
6. Participate in Quality Improvement activities related to readmissions.
7. Facilitate and attend meetings between patient, families, care team, payers, and community resources, as needed.
8. Administrative duties include: Track and trend readmissions, care plans, quality measures, ER and inpatient admissions and report at Quality Committees and participate in Quality Improvement Activities related to Population Health.
QUALIFICATIONS:
EDUCATION: Graduate of accredited ADN or BSN program.
LICENSE: Possess current license by Missouri State Board of Nursing.
CREDENTIALS: Registered Nurse.
EXPERIENCE: Three (3) to Five (5) year experience in clinical or community health settings preferred
Previous Care Coordination and/or Case Management experience preferred.
SPECIAL:
PATIENT CONTACT: Must have the knowledge and skills necessary to provide care appropriate to the age of the patients served.
TYPING: Moderate
MACHINE OPERATION: Computer for entry or retrieval of data.
TRANSPORTATION: Must be able to travel around 40 miles from the hospital.
PHYSICAL:
STANDING: Minimal periods, but must be able to sit for six to eight hours.
WALKING: Must be able to walk quickly to all areas of the hospital and occasionally to physician offices
LIFTING: Minimal - light loads (no more than 15 lbs)
VISUAL: Must have good acuity to review medical records, occurrence forms, computer screens, manuals, and other tools used in job performance
HEARING: Audio reception essential for effective understanding and communication with hospital staff and outside agencies
SPEAKING: Must have the ability to orally speak for effective communication with hospital staff and outside agencies
