CTI Care Manager
Responsibilities of the Critical Time Intervention (CTI) Care Manager:
Provide care management services to individuals across both Rensselaer County and Schenectady County through the Critical Time Intervention (CTI) model, which is a time limited, evidence-based care management approach focused on continuity of care for individuals with severe mental illness during times of transition from an in-patient setting back into the community.
Provide person-centered care planning, linkages to community resources, engage in skill building, offer crisis case management, and work on support development during critical times in participants' lives.
Be a part of a unique project that partners Unity of House of Troy in Rensselaer County and Mohawk Opportunities in Schenectady County together to provide a multi-disciplinary team approach to supporting participants.
Create linkages to community resources, assist participants in navigating complex systems, provide a range of treatment, rehabilitation and support services while promoting overall wellness and self-management.
Promote a culture of inclusion and belonging.
Provide transportation to clients using the agency's vehicles when necessary. An employee must be approved to drive for business purposes - this includes his/her/they personal vehicle along with agency vehicles - by maintaining and valid NYS driver's license and must be an approved driver. In addition, he/she/they must drive in a safe manner.
Requirements
Skills/Qualifications for the CTI Care Manager include:
A minimum of an associate's degree and one year experience working with individuals living with mental illness, substance use disorders or other disabilities preferred.
A demonstrated ability to function as a member of an interdisciplinary team as it relates to the mental health system of care is required.
Knowledge of case management defined as experience in assessment of individual's needs, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services.
A valid and insurable NYS driver's license. Must have own vehicle to use for business purposes. Agency vehicles are available for individual transportation.
Benefits
Salary and Schedule of the CTI Care Manager:
$45,760/annual equivalent. 40 hours per week. Monday through Friday 8:30am-4:30pm. On-call on a rotating basis and on-call stipend.

The Forensic Assertive Community Team (FACT) Psychiatric Nurse is a clinical staff member of the FACT
Community based multidisciplinary treatment team. The Nurse has the lead responsibility for assessing physical health needs; making appropriate referrals to community physicians; collaborating with medical providers and pharmacies; teaching wellness skills; and providing management and administration of medication under the direction of the team Nurse Practitioner. Additionally, the Nurse is responsible for providing a full range of services that are culturally relevant and recovery based. These services may occur in the home, community, or office. Service provision is designed to promote recovery by improving psychiatric symptoms, preventing relapses, teaching skills, and providing direct assistance and securing necessary community resources.
1. Develop a working knowledge of FACT as an evidenced based model and practice according to
the guidelines set forth by OMH. Participate in daily morning meetings with the purpose of reviewing client progress/needs, creating daily visit schedule and developing team relationships.
2. Conduct person-centered comprehensive health assessments, including personal and family medical and psychiatric histories, current medical needs, substance use history, and daily living skills with the purpose of linkages to and collaboration with providers in order to improve health outcomes.
3. Develop and implement strength-based treatment plans that are responsive to client needs and facilitate achievement of goals on an as needed basis.
4. Oversee all aspects of medication management in compliance with agency procedures and
protocols. This includes administration of injectable medications, managing Clozaril protocols, ordering of medication, overseeing processes for medication distribution, utilizing a medication step system towards client self-management, and managing all aspects of the medication room.
5. Provide support, treatment, case management and community advocacy and linkage services, per
the treatment plan. Complete required documentation of interventions as well as client's responses and outcomes.
6. Readily respond to all crisis situations and urgent needs using sound clinical judgement in order to ensure the safety and well-being of the individual. Provide on-call crisis intervention services on a rotating schedule amongst team members. Always act to ensure client's safety, readily assess needs, provide support, and intervene immediately as needed.
7. Mohawk's programs and offices are multi-level with stairs only. Individual services in the community include visits to individuals' apartments which can be on the second and third floors. Employees must be able to travel to all levels of the facility in a safe and reasonable time frame.
8. Provide transportation to individuals when necessary. In addition, an employee must be approved to drive agency and personal vehicles for business purposes by maintaining a valid NYS driver's license and must be approved by the agency's vehicle insurance carrier. An employee must always drive in a safe manner and use all safety devices.
Requirements
Skills/Qualifications for the Forensic ACT Team Psychiatric Nurse include:
1. Licensed Registered Professional Nurse by New York State.
2. Bachelor's degree in nursing preferred.
3. Three years' experience in a psychiatric setting, with at least one year's experience in an outpatient setting is preferred.
4. Must have a strong commitment to the rights and ability of each person with a mental illness to make own decisions and choice, live in the community, work, and have access to continuous support and services to enhance wellness.
5. Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.
6. Must uphold individuals' rights and personal preferences in treatment and treat everyone with respect and dignity. Continuously acting as a guest in individuals' homes and family homes, is courteous, polite and an active listener.
7. Valid and insurable driver's licenses. Must use your own vehicle for business. Agency vehicles are available and need to be used to transport individuals.

Daytime Coverage will entail in office presence to respond to crisis calls, conduct both crisis intervention and crisis planning services, and to conduct both walk-in and scheduled intake sessions.
Evening/Weekend coverage will be remote and will be on-call coverage. It will entail responding to any crisis calls that may come in, with immediate availability to respond in person to crisis within one hour.
Provide immediate crisis intervention and stabilization, counseling and support to individuals in the community experiencing acute distress and behavioral health crisis.
Provide screening assessment including of suicide risk and protective factors, safety assessment and safety planning.
Deliver crisis services via phone crisis line, telehealth and in community based settings.
Assess need for law enforcement or EMT services in crisis situations.
Crisis Planning services utilizing proactive response to avert crisis and promote individual's well-being.
Participate in a care team devoted to the care and well-being of individuals at high risk for behavioral health crisis.
Participate in suicide prevention committee devoted to oversight of individuals with suicide risk and promoting prevention best practices.
Follow up on referrals and linkage to services.
Develop relationships and partnerships with healthcare providers, hospitals and other community based organizations to enhance the continuum of care, and timely access to services

Residence Counselor - Part Time
Responsibilities of the Residence Counselor are to provide services to adults who have a diagnosis of mental illness and living in a group home. Specific duties of the Residence Counselor included:
Provide supportive counseling;
Providing rehabilitation skills such as teaching of daily living skills including medication management and coping skills, crisis intervention and prevention;
Recreation activities for socialization and stress reduction; and
Maintaining a healthy and clean environment for our clients ensuring clients are doing their chores correctly and by assisting with cleaning activities.
Requirements
Skills/Qualifications for the Residence Counselor include:
High school diploma required, college classes in human services preferred.
Demonstrated ability in helping others.
Valid and insurable driver's license.
Experience working with people who have a diagnosis of mental illness or other human service experience.
Both the ability to work independently and as a team member.
Excellent communication skills both verbal and written.
The ability to be organized and have good time management skills.
