Responsibilities The Hospice Aide Under the direction and ongoing supervision of a registered nurse, provides personal direct patient care services as established and delegated in the Plan of Care daily. * Reports after patient encounter with the Executive Director, or designee, regarding the patient's condition or environment, following the Care Management process on as needed basis. * Provides assistance with the following ADLs during each patient visit: mobility, transfers, walking, grooming, bathing, dressing, or undressing, eating or toileting. * Performs incidental household services essential to the patient's health care at home, conducting household services necessary to prevent or postpone institutionalization. * Participates in QAPI (QUALITY ASSESSMENT & PERFORMANCE IMPROVEMENT) plan or process identifying problems and reporting administrator daily or as needed. * Must have a reliable vehicle/transportation. Education and Experience License Requirements * Successful completion of a competency evaluation prior to patient care. * Current CPR certification requirements. * Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. NAITs must be actively enrolled in a Minnesota MDH Nursing Assistant Registry-approved nursing assistant training course at the time of hire. Upon completion of the course, the NAIT will take and pass the competency exam within 4 months of hire at which time the NAIT will apply internally for the open CNA position at the facility of their choosing. The student is allowed three chances to pass both the written and skills portion of the exam. A new training program must be completed if the student fails to pass by the third attempt. As an NAIT, you will be responsible for most duties of a certified nursing aide (CNA) as officially deemed competent through in part, completion of at least 16 hours of training in core areas. Qualifications: Must have: * Active enrollment in a Minnesota DHS Nursing Assistant Registry-approved Nursing Assistant program. * Completion of 16 hours of supervised practical training. The CNA provides residents with the highest quality of direct routine care in all activities of daily living, under the supervision of the LPN or RN and in accordance with the MDH Nursing Assistant Program standards, the Resident's Bill of Rights and the facility Quality Assurance and Performance Improvement (QAPI) program. According to the Plan of Care for the resident, CNAs apply skills in basic nursing, personal care and basic restorative services while understanding the mental health and social service needs of the resident and being mindful of the needs in cognitively impaired residents. Qualifications: Must have: * Completion of a Minnesota DHS-approved Nursing Assistant program. * Current certification and listing on the Minnesota Nursing Assistant Registry Required Skills and Abilities. Preferred: * Experience in healthcare, skilled nursing facility or assisted living community.
