Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
Instructs and supervises the patient/family in self-care techniques when appropriate.
Maintains accurate and relevant clinical notes regarding the patient's condition.
Experience Requirements
One year of clinical experience.
License Requirements
Current RN licensure in the state of practice and one year of clinical experience.
Current CPR Certification.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
Additional State Specific Requirements
LA: a minimum two years full time experience working as an RN. However, two years of full time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.

POSITION RESPONSIBILITIES
• Provides the Synergy Bank standard of excellence in customer service.
• Maintains honesty and the highest level of ethical standards.
• Receives cash and/or checks for deposits to accounts, verifies amounts, examines checks for proper endorsement, and enters deposits into computer records.
• Cashes checks and processes withdrawals; pays out money after verification of signatures and customers balances.

BALANCING PROTOCOL & RESPONSIBILITIES:
The responsibility of balancing is a priority of the Teller I position. Every day, they are responsible for physically hand counting all the loose cash, strap cash, and coins to ensure the teller drawer, ATM, and cash vault all balance. Cheat sheets indicating the coin amounts in the vault are strictly prohibited in lieu of physically counting it daily. Trades must be done through Teller machines at the time the funds are given and received. There is no exception to this procedure.
If at any time during the day, the drawer is counted to balance, the cash count is to be printed, and placed in the work bin. At the end of the workday, a clear screen must be used to balance the drawer and physically count all monies. If a transaction is completed for a customer either during or after this count, again, a clear screen must be used, and all monies shall be physically recounted.
It is important to understand that if money is miscounted, and the drawer balances, then that is force balancing the drawer. This is grounds for disciplinary action. If a drawer is counted and found to be out of balance, a second employee must count that drawer as well. The count must agree with the initial count, and if not, a supervisor must count to verify accuracy. Outage entries must be run before the day can be closed. All Tellers must ensure that their transactions have been received by CSI before exiting their teller.

Network with other health professionals, such as private physicians/nurses, in the development and implementation of interagency information/referral systems to identify patients with nutritional problems and needs.
Utilize evidence-based practice in research and development of current concepts in nutrition and nutritional education.
Utilize appropriate time management skill in conducing appointments to ensure everyone is seen in a timely manner.
Maintain schedules and prepare reports and correspondences related to duties performed as requested and/or noted in WIC policy and procedures.
Coordinate a wide range of health education service, including establishing effective referral and publicity systems, monitoring quality and documentation, providing scheduling and logistical support and facilitating use of community services to promote cost-effective health education services.
Document appropriately in medical record and LaWIN following Teche Action Clinics and State of Louisiana WIC program guidelines.
QUALIFICATIONS
Minimum Qualifications:
Baccalaureate degree in nutrition, dietetics, human ecology, home economics, family and consumer sciences, health promotion, pre-medicine, or nursing.
Substitutions:
An associate degree in nutrition, dietetics, human ecology, home economics, family and consumer sciences, health promotion, pre-medicine, or nursing plus two years of experience as a WIC Clinic Nutrition Educator in the State of Louisiana will substitute for the baccalaureate degree. Twelve semester hours in food and nutrition or dietetics plus three years of experiences working as a WIC Clinic Nutrition Educator in the State of Louisiana will substitute for the baccalaureate degrees.
Skills:
Communication Ability: Demonstrates ability to effectively communicate and establish rapport with participants.
Math Ability: Basic math skills.
Reasoning Ability: Demonstrates ability to utilize skills and knowledge of nutrition to appropriately evaluate lab values and nutritional requirements in prescribing food packages and monitoring interventions of nutritional instructions.
Computer Skills: Computer basics to include proficiency in Microsoft Word, Excel, Outlook, PowerPoint, and the Internet.
Professional Skills: Demonstrate organized approach to work with detail and an ability to multitask. Demonstrate knowledge of the principles of human nutrition and clinical nutrition principles and practices. Adhere to the policy and procedures as regulated by TAC and State of Louisiana WIC program.

Successful completion of State of Louisiana WIC initial and continuing education on Couresmill Learning Modules System.
Completion of OPH and TAC mandatory initial and continuing educations along with other in-services.
Attends work with minimal absences.
