Take on this impactful role. Use your expertise in wound and ostomy care as you deliver pre- and post-operative treatment and education to patients and their families. Regularly evaluate patient outcomes, and participate in initiatives focused on improving quality and performance, such as minimizing readmissions of our patients.
This is a Full Time 12.5 hour shift position working Monday - Friday 6a-7:15p and 7a-8:15pm; flexibility on start and end times is required to meet department needs. Also includes occasional Split weekend coverage.
Preferred Criteria
Applicable experience in an academic medical center (includes experience partnering with interns/residents/attendings)
Masters preferred
Required Criteria
Bachelor's degree in Nursing
NYS RN License
At least 4 years of acute care nursing experience
Certification in Wound and Ostomy Care
Graduation from an accredited wound and ostomy nursing education program
BCLS certification
Strong communication, interpersonal and analytical skills to drive engagement

These specialized units are staffed by a highly skilled teams of intensivists from multiple subspecialties, including anesthesiology, pediatric cardiology, pediatric heart surgery, neonatology (newborn care), cardiac critical care, and pulmonology (lung care).
The Patient Care Director assumes front line responsibility and accountability for managing the delivery of patient care. Responsible for the total management of a designated unit, which includes setting goals and objectives in accordance with the NYPH strategic initiatives, and for planning, organizing, directing, and monitoring unit activities on a twenty-four (24) hour basis. Fiscally responsible for the unit, facilitating efficient and cost effective daily operations. Coordinates and evaluates the effectiveness of staff, unit performance, and system operations. Implements unit's strategic plan relevant to NYPH initiatives, while continuing to develop professional and support staff in establishing an environment where staff and patients are able to reach their full potential.
Preferred Criteria
* Five years of leadership experience in a Pediatric ICU within a large academic medical center
* Demonstrated leadership in leading unit development
* Experience with union environments
Required Criteria
* At least three years of Pediatric ICU nursing experience

Case Manager - RN - Inpatient Units- Brooklyn Methodist - Days
Located in Park Slope, Brooklyn, this acute-care teaching hospital has been providing outstanding medical care to the surrounding communities for over 135 years.
The Case Manager is responsible for all aspects of discharge planning and utilization management/review for an assigned group of patients to determine the appropriateness of the admission and continued stay, assist in the development of the plan of care; ensure that the plan is implemented timely and identify the expected length of stay (ELOS). The Case Manager works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team. The Case Manager actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improving efficiency and quality, and ensuring resource utilization is appropriate.
Case manager/utilization manager: Responsible for an average of 60-70 medical/surgical daily admissions that requires risk identification and clinical reviews. A utilization nurse completes no less than 20 initial clinical reviews daily.
Responsibilities also includes:
• Completing 100% clinical reviews for managed care, Medicare and Medicaid admissions
• Completing concurrent and retrospective reviews
• Communicating with physicians and the inpatient case management teams on patients who do not meet initial inpatient criteria or continued inpatient stay
• Coordinating expedited appeals for inpatient denials with attending physicians and insurance providers
• Ensuring organization resources are being utilized appropriately
• Completing patient assessments within 24 hours
• Coordinating safe and appropriate post-acute care plans
• Obtains authorizations from managed care companies for post-discharge services.
There is an onsite home care agency and a transfer and authorization center that can facilitate the referral process.
This is a full time Monday - Friday position 8:30am -5:00pm
Position requires weekend rotation and holidays.

Research
1. Participates in the planning of new protocol implementation on the patient care unit(s) or practice(s)
2. Administers research interventions; collects patient data according to protocol specifications
3. Evaluates the patient response to therapy; responds to variances in protocol implementation; reports variances to the research team
4. Reports adverse events and serious adverse events in accordance with the IRB standard of practice, sponsor guidelines, and federal regulations.
5. Integrates evidence-based practice into nursing practice; and evaluates patient outcomes.
Patient Care
1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards, in relation to the requirements of the research protocol.
2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
3. Integrates relevant assessment and intervention skills in the delivery of nursing care.
4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care, in conjunction with the requirements of the research protocol.
5. Prioritizes all aspects of patient care including assessing, teaching, coaching, ordering research protocol specified tests and delegating to others as appropriate.
6. Educates patients and caregivers while anticipating needs and readiness to learn, about their research specified plan of care, promotion of health, and prevention of disease.

8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively.
