The primary job responsibility of the Nutrition Education Specialist is to greet and initiate exceptional patient service to all stakeholders, fulfill job expectations and task assignments efficiently and effectively. The Applications Specialist's position is to aid in the development, testing, and maintenance of clinical applications, interfaces, and the electronic health record (EHR). This role plays a crucial role in the effective adoption of the system by helping to implement, manage, and support the software applications that Mountain Park Health Center uses. This role assists in enhancing application administration, usage, and on demand training. This position will be incorporated into the clinic setting to ensure real-time analysis of all clinical applications. Essential Functions: * Actively handles EHR, interfaces, and other clinical applications support issues across multiple clinic sites using various communication methods, such as in person interactions, phone calls, emails, or Microsoft Teams. * Assists with the analysis, design, build, installation, evaluation, optimization, and updates end user training of new and established Mountain Park Health Center computer software systems. * The role involves prioritizing, investigating, and troubleshooting issues related to system applications, interfaces, and data processing. Solves the problem and/or helps determine correct measures to be taken toward the solution. * Supports and assists in designing and implementing workflow processes. * Keeps appropriate records and documents for all clinical applications building and testing. * Works closely with both internal team members and external stakeholders, including vendors. * Acts as a change agent to encourage the adoption of new processes and technologies that enhance best clinical practices. * Involves integrating knowledge of clinical/billing information systems and clinical/billing practice workflow processes to collaboratively plan and carry out implementation activities. * Evaluates the cross-departmental impact of individual user or department-level system/workflow change requests. The Customer Service Representatives (CSR) must additionally be able to perform patient scheduling duties, process payments/co-payments, and perform other cashiering functions. The Credentialing Analyst is responsible for analysis, evaluation, and validating provider information against business/credentialing requirements and maintains information in Credentialing databases and other data sources. The Credentialing Analyst's goal is to see all providers credentialed for all plans and work closely with the Revenue Cycle Director to meet Revenue Cycle metrics. ESSENTIAL FUNCTIONS * Assists with developing metrics that will eliminate and/or reduce timely filling issues due to late credentialing of Mountain Park Health Center providers. * Maintains an accurate database containing credentialing status by Provider and Plan. * Coordinates with Plans maintaining accurate provider rosters by Plan. * Completes provider credentialing and re-credentialing applications and submit credentialing packets to plans before providers start work. * Obtains Arizona Health Care Cost Containment System (AHCCCS) and Medicare numbers for providers before providers start providing patient care. * Holds plans accountable to credentialing new providers within rules and timeframes established for Federally Qualified Health Centers (FQHC's). * Partners with billing to resolve credentialing and billing issues. * Maintains knowledge of Medicaid, Medicare, private health plans, and agency requirements for credentialing Providers. * Sets up and maintains provider information for Council for Affordable Quality Healthcare (CAQH) databases and system. * Ensures practice addresses and other key information are current with health plans, agencies, and other entities. * Maintains Plan information by provider in credentialing database and uses system of alerts to proactively monitor credentialing deadlines. * Creates system to track and respond to communication from Plans and credentialing requests in a timely fashion. * Coordinates with Privileging and Human Resources (HR) to ensure initial credentialing forms are returned timely and accurately by new Providers. * Performs other duties as required. Mountain Park Health Center is looking for Licensed Clinical Social Workers (LCSW) to join our well established and accredited team of providers. As one of the preferred Federally Qualified Health Centers (FQHC) in Arizona, our mission is to sustain and improve health by providing excellent, patient-centered primary care. As a Licensed Clinical Social Worker (LCSW), you will be working in one of our primary care clinics as an integrated team member and will deliver brief consultation-based services including behavioral interventions for psychological, mental, and physical conditions in patients of all ages. LCSW's will also provide traditional counseling/therapy to our pediatric and adolescent patients. ESSENTIAL FUNCTIONS: * Provides brief, evidence-based, solution focused behavioral interventions with active development of psychotherapeutic/psychoeducational behavioral change planning for patients of Mountain Park Health Center. * Provides quality clinical consultation to patients and providers regarding mental, behavioral, emotional, and physical health related issues. * Provides screening and functional assessment of behavioral health issues for Mountain Park Health Center patients. * Collaborates with medical providers, support staff, and operational managers to identify patients with medical, cognitive, behavioral, and/or psychological conditions that could benefit from behavioral health interventions, such as motivational interviewing, and provide these services in a timely manner within the clinic setting. * Develops and implements specific clinical pathways for select populations. * Facilitates groups to address biopsychosocial issues impacting medical issues. * Assists with coordination of care and access to behavioral health agencies in the community to meet the patient's behavioral health care needs with consistent, expeditious feedback to referring providers. * Provides crisis triage and intervention. * Helps with de-escalating situations with patients who are upset or frustrated. * Maintains regular and predictable attendance. * Performs other duties assigned. POSITION QUALIFICATIONS MINIMUM QUALIFICATIONS: * Current licensure in the state of Arizona as a Clinical Social Worker. * Must be eligible for credentialing and contracting with all insurers. The Women, Infants, and Children grant program (WIC) Nutritionist, screens, certifies, provides comprehensive nutrition assessments, and medium-risk nutrition counseling services to participants of the Special Supplemental Nutrition Program for WIC, and provides participant centered services according to federal guidelines, rules, and regulations. Essential Functions: * Provide participant centered nutrition and breastfeeding support services to WIC participants and offer State Agency approved resources and incentives. * Maintain documentation of nutrition services and administrative as described in the WIC Policy and Procedure Manual. * Assign and tailor a food package appropriate to the participant's nutritional risk(s), category, and cultural preferences and issue food benefits as set forth in the WIC Policy and Procedure Manual. * Provide information, check for understanding, and document training to participants on program rules, regulations, WIC approved foods, use of eWIC cards, and food benefit use and redemption. * Proficient user of State of AZ Computer Data System (HANDS). * Document the Nutrition Discussion Contact with appropriate note after each service. * Maintains regular and predictable attendance. * Performs other duties as required.
