The Medical Assistant (MA) I position requires a basic level of knowledge regarding the functions necessary to becoming a MA. Essential Functions * Performs and completes clinical and non-clinical assigned duties, tasks, responsibilities, and assignments in the back office. * Adheres to and follows procedures for immunization and medication administration, including but limited to labeling, storing, and utilization. * Performs and adheres to all Clinical Laboratory of Improvement Amendments (CLIA) testing instructions; collects specimens, processes and documents results in the electronic health records (EHR) * Fulfills provider orders within scope of practice and provides patients' discharge instructions. * Assists with coordinating the continuity of patient care with internal health departments. * Maintains proper levels of supplies in exam rooms and prepares exam room for patient care and procedures. * Identifies and reports any safety hazard, or potential hazard, by directly notifying supervisory staff immediately of concerns. * Interviews patient to obtain reason for visit, history, and chief complaints; performs vital signs and other patient care needs, documents the same to complete the patient intake process. * Utilizes and abides by proper charting and documenting procedures for all patient related services, exams, interactions, and assignments performed. * Maintains regular and predictable attendance. * Performs other duties as required. The role of the Registered Nurse (RN) is to work collaboratively with providers and the care team to provide safe, efficient, and effective care to empaneled patients. Under the clinical direction of the Chief Medical Officer (CMO) or designee, the RN (1) facilitates health and disease management, (2) uses evidence-based care to improve quality and patient outcomes, (3) assists in transitions of care, and (4) is responsible for assessing, planning, implementing, and evaluating an organized program for staff education directed toward the achievement of professional excellence. Serves as a point of contact for nursing and clinical support service staff education. Contributes to the creation of clinical resources and instructional leadership for outreach programs directed to health care professionals. Essential Functions: * Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and determines next steps. * Sends those with high-risk chief complaints such as to Emergency Department (ED) immediately. Shall schedule those patients with non-emergent symptoms as soon as possible and may provide appropriate home care advice until patient is seen by a provider. * Maintains follow up of all patients directed to the ED per established protocols. * Acts, when designated, in "Ask a Nurse" capacity, handling routine information requests from patients. * Acts as a clinical leader in the department and is a clinical resource for support staff. * Responds to emergent patient needs and conducts triage as needed. * Participates in assessment and evaluation to sign off support staffs' competency skills checklists. * Functions as a liaison with community health professionals to promote professional/education programs and services. * Designs, conducts, monitors, and evaluates educational programs for new and current nursing and clinical support staff. Recommends direction of program focus. * Assesses educational needs of the learner and develops plans for learning experiences appropriate for meeting recognized needs. * Maintains regular and predictable attendance. * Performs other duties as assigned. Mountain Park Health Center is looking for Registered Dietitians (RD) to join our well established and accredited team of providers. As a large Federally Qualified Health Centers (FQHC) in Arizona, we are dedicated, to sustain and improve health by providing excellent, patient-centered primary care. The Registered Dietitian (RD) performs medium and high-risk nutrition counseling services for participants on the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) at Mountain Park Health Center; this includes but is not limited to formula authorization, and as necessary, Certification of clients. Essential Functions: * Performs WIC services and procedures such as categorical and income screening and health and nutrition assessments in accordance with local, state, and federal regulations and procedures. * Plans, creates, and provides individual and group nutrition and breastfeeding education to medium and high-risk participants, providers, and professional staff utilizing interactive teaching methods and diverse multi-media materials and aids. * Consults with physicians and other healthcare professionals regarding clinical nutrition issues or concerns, including customizing individual meal plans for program participants with medical needs. * Assigns and tailors an appropriate food package for eligible participants based on age, category, health risks, and family needs and issues WIC food benefits. * Reviews and authorizes special formulas and documents approved in HANDS. * Develops and revises nutrition education materials as assigned. * Assists with the self-assessment efforts by conducting assigned observations and chart reviews. * Serves as preceptor to incoming dietetic interns and provides mentoring. * Maintains regular and predictable attendance. * Performs other duties as required.
