ESSENTIAL FUNCTIONS: * Provides brief, evidence-based, solution focused behavioral interventions with active development of psychotherapeutic/psychoeducational behavioral change planning for patients of Mountain Park Health Center. * Provides quality clinical consultation to patients and providers regarding mental, behavioral, emotional, and physical health related issues. * Provides screening and functional assessment of behavioral health issues for Mountain Park Health Center patients. * Collaborates with medical providers, support staff, and operational managers to identify patients with medical, cognitive, behavioral, and/or psychological conditions that could benefit from behavioral health interventions, such as motivational interviewing, and provide these services in a timely manner within the clinic setting. * Develops and implements specific clinical pathways for select populations. * Facilitates groups to address biopsychosocial issues impacting medical issues. * Assists with coordination of care and access to behavioral health agencies in the community to meet the patient's behavioral health care needs with consistent, expeditious feedback to referring providers. * Provides crisis triage and intervention. * Helps with de-escalating situations with patients who are upset or frustrated. * Maintains regular and predictable attendance. * Performs other duties assigned. QUALIFICATIONS: MINIMUM QUALIFICATIONS: * Current licensure in the state of Arizona as a Clinical Social Worker. * Must be eligible for credentialing and contracting with all insurers. * Current CPR Card. PREFERRED QUALIFICATIONS: * Training in Motivational Interviewing (MI). * Training in Acceptance and Commitment Therapy (ACT).

The Pharmacy Technician supports and collaborates with the healthcare team, primarily the pharmacy team, to ensure patients receive high-quality care related to dispensing medications at Mountain Park Health Center. ESSENTIAL FUNCTIONS: * Processes prescriptions and enters patient information into Mountain Park Health Center's computer software systems. * Answers patient questions, troubleshoots, and resolves patient concerns and complaints. Refers patients with questions regarding prescriptions, drug information, or health matters to a pharmacist. * Assists the pharmacist in dispensing, filling prescriptions, inventory control, stocking shelves, clerical work, and data entry. * Communicates patient care related information with members of the pharmacy staff, external contacts, and other healthcare professionals. * Fills and documents medications in the automated dispensing machine. * Administers vaccines to patients * Ensures efficient workflow of pharmacy operations. * Supports compliance with the 340B drug pricing policy and abide by all rules and regulations set forth by the State Board of Pharmacy and Mountain Park Health Center's policies and procedures. * Ensures and supports the cost-effective use of materials, supplies, and equipment by limiting waste of all pharmaceutical or organizational supplies and resources. * Handles all methods of payments using the cash register and follows cash handling procedures. * Prepares cash register deposits. * Travels to multiple locations during shift. * Maintains regular and predictable attendance. * Performs other duties as required. MINIMUM QUALIFICATIONS * High school diploma or general education degree (GED). * Licensed as a Pharmacy Technician in Arizona. * Immunization Administration Certificate or the ability to obtain within 90-days of hire. * Must be bilingual and able to interpret for Spanish-speaking patients.

As a Licensed Professional Counselor, you will provide traditional counseling services to Mountain Park patients aged 6 years through adulthood. Your work will help diverse populations access high-quality care and ensure culturally competent services. Our integrated health model ensures comprehensive services for our patients, fostering collaboration between medical and behavioral health professionals. ESSENTIAL FUNCTIONS: * Provides evidence-based outpatient psychotherapy, including but not limited to: Acceptance and Commitment Therapy, Cognitive Behavioral Therapy, Motivational Interviewing, and Solution-Focused Therapy. * Conducts individual, family, and couples therapy sessions. * Performs intake assessments, develops treatment plans, and updates them with measurable, age-appropriate objectives. * Provides assessment and DSM-5TR diagnostics for patients referred by medical staff. * Collaborates and co-manages behavioral health care alongside medical providers and behavioral health consultants. * Delivers care in-person and via telehealth as needed. * Maintains accurate clinical records, ensuring treatment and discharge plans align with compliance standards. * Offers crisis triage and intervention as needed. * Maintains regular and predictable attendance in accordance with organizational standards. * Travels to multiple locations during shift and within a reasonable amount of time. * Performs other duties as assigned to support patient care. The Women, Infants, and Children grant program (WIC) Nutritionist, screens, certifies, provides comprehensive nutrition assessments, and medium-risk nutrition counseling services to participants of the Special Supplemental Nutrition Program for WIC, and provides participant centered services according to federal guidelines, rules, and regulations. Essential Functions: * Provide participant centered nutrition and breastfeeding support services to WIC participants and offer State Agency approved resources and incentives. * Maintain documentation of nutrition services and administrative as described in the WIC Policy and Procedure Manual. * Assign and tailor a food package appropriate to the participant's nutritional risk(s), category, and cultural preferences and issue food benefits as set forth in the WIC Policy and Procedure Manual. * Provide information, check for understanding, and document training to participants on program rules, regulations, WIC approved foods, use of eWIC cards, and food benefit use and redemption. * Proficient user of State of AZ Computer Data System (HANDS). * Document the Nutrition Discussion Contact with appropriate note after each service. * Maintains regular and predictable attendance. * Performs other duties as required.
