Job Title: Licensed Physical Therapist
Department: Physical Therapy
Job Type: Full-Time
Job Summary: As a Licensed Physical Therapist, you will organize, plan, and implement physical therapy programs, working with patients to aid recovery from physical disabilities. You will also provide supervision and guidance to therapy assistants and aides.
Schedule: 4 10 hour days, or 7:45 to 5 with 1/2 day off, or 8-5, or 7-3:30. (with potential for weekend hours based on patient needs)
Duties and Responsibilities:
Provide physical therapy services to diverse patient groups, including pediatrics and skilled care patients.
Evaluate each patient to develop and implement personalized treatment plans.
Supervise therapy programs, ensuring quality patient care and addressing any concerns or obstacles.
Collaborate with physicians to review and adjust treatment plans as necessary.
Maintain accurate patient records, including evaluations, progress notes, and discharge summaries.
Communicate treatment protocols to the care team to support consistent patient care.
Attend professional development opportunities to stay current with best practices in physical therapy.
Perform other duties as required by hospital administration.
Qualifications:
Graduate of an accredited Physical Therapy program.
Must hold a current license as a Physical Therapist in the State of Nebraska.
Physical Demands and Work Conditions:
Ability to sit, stand, and walk for extended periods with intermittent movement.
Occasional lifting and supporting of patients may be required.
Primarily indoor work, with occasional travel to other facilities.

Job Description:
Provides patient care services along with direct treatment and follow-up.
Evaluates patients new to service where there are physician orders, utilizing motor evaluation, sensory-perceptual-cognitive evaluation, and/or performance evaluation as indicated by the patient's condition.
Develops a plan of care appropriate to the patient's diagnosis, utilizing assessment findings and data.
Establish measureable goals in compliance with patient's physical assessment.
Documents patient care data accurately, thoroughly, and in a timely matter, reflecting evidence of improvement/progress/decline in patient's condition.
Coordinate regularly with COTA's and provide monthly supervision of the COTA's patient care.
Submits the daily/weekly patient visit schedule to the Scheduler.
Communicates as necessary with intercompany and intracompany contacts.
Discusses company and third party reimbursement policies and guidelines with the patient/family and discuss treatment plans with the Patient Care Coordinator, physician, and patient/family.
Coordinates with Patient Care Coordinator in making referrals to appropriate disciplines.
Provides in-service education, health care professional education, and public education in addition to supervising peers and supporting personnel and students in training as needed by the company.
Conducts research and assists with orientation of new employees as needed.
Job Requirements:
Minimum bachelor's degree in occupational therapy from an accredited school of occupational therapy, successful completion of the National Certification Exam and current licensure from the state where therapy will be provided.
Prefer a minimum of one year's therapeutic experience in a non-home health care environment.
Strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to evaluate and treat patients for whom a physician's order exists.
CPR certified and strong communication and organization skills.
Minimum of twelve hours of continuing education annually.

The Sr. Physical Therapist is responsible for providing guidance and support to rehab staff in the assigned area. A Brief Overview
The Sr. Physical Therapist is responsible for providing guidance and support to rehab staff in the assigned area. Plans and provides physical therapy treatment for patients in accordance with professional and MLH standards of treatment. Contributes to specific programs and projects supporting the department.Models appropriate behavior as exemplified in MLH Mission, Vision and Values.
What you will do
Provides day-to-day direction to associates in assigned area. Assists in scheduling to ensure adequate departmental staffing, participates in interviewing and training potential new Associates, provides input into Associate evaluations and corrective actions and collaborates with technical staff in organizing daily workflow.
Serves as a resource for associates, answering questions, researching issues, and resolving problems.
Performs initial patient assessments.
Assesses history, medical diagnosis, and current condition of assigned patients.
Ensures that policies, procedures, and standards of Physical Therapy are followed at all times.
Performs and implements patient treatments based on assessment and written plan. Performs patient treatment activities to reduce physical, emotional and social limitations including teaching self care skills and the use of adaptive equipment, including mobility, manual dexterity and daily activities; assisting patients to maintain maximum function by teaching proper body mechanics and adaptations to daily activities and assessing patient's potential to return to work and determining appropriate referrals.
Provides ongoing patient assessment through observation of patient and records. Documents and changes in patient's condition and any resulting changes in patient treatment (both physical and human response).
Evaluates treatment plan and patient's response to treatment. Re-assesses short-term goals as needed.
Identifies patient specific Physical Therapy rehabilitation learning needs and provides appropriate education for patients and their families. Assesses effectiveness of patient and family education. Documents learner response and outcomes of teaching.
Demonstrates initiative in identifying opportunities for self-development and enhancement of professional competency.
Performs other job functions as assigned or requested.

Physical Demands
The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
Must have good balance and coordination.
The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.

The Skilled Rehab and Home Health team at Good Samaritan Grand Island and Hastings seeks to hire a Physical Therapist to join our team. The position is full time, 35-40 hours per week, M-F (weekdays/weekends/holidays can be flexible), covering the Skilled Nursing Facilities and surround area Home Health clients. Currently, we treat the following conditions: orthopedic, weakness, balance/falls/gait, cervical/back pain, general deconditioning, and other various conditions. Job Summary
Plans, organizes and delivers physical therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions clients/patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. Demonstrate and incorporate knowledge of current research into daily treatment of clients/patients/residents.
May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization.
Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Will be interacting with individuals outside of the department including but not limited to: providers, case managers, third party payers, public organizations, companies/contractual partners, etc. May be asked to supervise assistants, aides, and students as they provide client/patient/resident care, treatment, or other services. Will be expected to provide ongoing education and training to peers in the area(s) of expertise as determined by the department leadership. Performs other related duties as requested by manager.
Based on facility needs and assigned locations, may focus on those receiving intervention in the post-acute, long term care and home health care environments.
Qualifications
Appropriate education level required in accordance with state licensure.
Must be licensed in the state(s) of practice as a Physical Therapist.

Position Summary
The Residential Therapist (PRN) is responsible for overseeing and coordinating the delivery of inpatient, continuing care and family services. Must have a Master's Degree and be licensed or licensed eligible in the State of Virginia in the field of psychology or a related field.
