    Job Title: RN Home Health Clinical Manager Department: Home Health Administration Reports To: Home Health Director The Home Health Clinical Manager (RN) provides leadership and oversight of clinical operations to ensure high-quality, compliant patient care in the home health setting. This role supervises clinical staff, supports regulatory compliance, and collaborates with interdisciplinary teams to drive positive patient outcomes and agency performance. Duties & Responsibilities: Provide clinical supervision and leadership to RN's, LPN's, home health aides and therapy team members. Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies. Review, approve, and oversee OASIS assessments, plans of care, and clinical documentation. Monitor clinical quality indicators, collaborating with the Home Health Director in identifying performance improvement initiatives. Support staff through training, mentoring, competency validation, and performance management. Coordinate care with physicians and interdisciplinary team members. Address patient care issues, incident reports, and risk management concerns. Assist with staffing, scheduling, and census management. Participate in interviewing, onboarding, and staff evaluations. Serve as a clinical resource and liaison between field staff and executive leadership. Primary Function/General Purpose of Position The Nurse Manager is responsible for the integration, facilitation, and coordination of administrative and clinical practice for the assigned nursing unit(s) with twenty-four hour accountability. Provides management and clinical support to other nursing units and house-wide as required. Serves as a professional role model, mentor, and clinical resource/educator for nurses and other members of the health care team.
