DUTIES AND RESPONSIBILITIES: Ability to perform a head-to-toe assessment on all patients and reassessments Able to effectively perform the duties of a staff medical surgical registered nurse and as a clinical team lead/charge registered nurse. Coordinate day to day operations with CTLs as directed by manager. Assist manager with daily workflow, staffing, orientation, training, and overall unit department development. Assist manager with monitoring and accountability of performance measures, reporting requirements, and other process improvements as assigned. Support and create policies and procedures as directed by department manager. Monitor and review quality of care and documentation to ensure performance metrics are being met Provide clinical and financial data to manager and assist with evaluations as assigned. Qualifications QUALIFICATIONS: Education/Certification: Graduate of an accredited RN program. Current RN licensure in the state of Idaho. Current BLS. ACLS, PALS, and Basic Dysrhythmia within 6 months of hire. Preferred chemotherapy certification within 1 year of hire. Experience: Preferred 2 years acute care nursing experience and able to demonstrate progressive leadership experience and/or training. Equipment/Technology: Able to demonstrate basic computer knowledge and skills. Preferred experience with Cerner EMR. Language/Communication: Ability to communicate effectively and clearly with different patients, families, visitors, staff, members of the interdisciplinary team and others. Mathematical Skills: Knowledge of basic nursing medication calculations and the ability to educate nursing staff regarding medication calculations. They may teach patients and their families about the disease and its treatment. Because of their specialized knowledge of the care of cancer patients, oncology nurses often serve as resources to other staff members. DUTIES AND RESPONSIBILITIES: Infusion Patients: Review doctor's orders for medications to be administered and labs to be drawn. Verify if preauthorization is needed. Complete nursing assessment of patient. Start IV or access central line and obtain laboratory tests per written policies. Run CBC and record, review any concerns from CBC or assessment with physician. Collect premedication from the Lynx machine and prepare. Administer pre-medications as ordered. Double check calculated dosages with another nurse. Safely administer medications to patients ordered and according to written pharmaceutical instructions and written policies. Administer supportive medications as ordered. Monitor patient for signs and symptoms of allergic reaction or side effects to medication being administered. Meet patient's needs for comfort and safety. Teach patient regarding treatments being administered and answer questions. Review home medications and antiemetic with patients, call prescriptions into pharmacy as necessary. Before infusions and during while medications are being infused, assess line for patency and signs or symptoms of infiltration. Document care given, assessments, patient's response and teaching. New Infusion Patients: Comfort patient as necessary and familiarize patient and family with facilities. Review physician's orders. Go over teaching packet regarding management of side effects of chemotherapy. Give patient medication information according to medications ordered. Review above information with patient and family and answer questions. Obtain a signed consent for administration of chemotherapy. Administer chemotherapy as noted above. Assist doctors in rooms as needed for breast exams, procedures, lab draws and injections. Current RN licensure in the state of Idaho. Current BLS required, ACLS, PALS, and TNCC preferred Successful completion of ACLS and PALS course within 6 months of hire and renewal every 2 years prior to expiration. Through risk stratification tools available by Population Health, the Care Coordinator identifies patients with a need for early intervention related to discharge education, transitional care or disease management and coordinates intervention with PCPs and other care providers. DUTIES AND RESPONSIBILITIES: 1. Work under direct supervision of nurse supervisor to provide excellent patient care to all clients. 2. Participate in the transition of the clinic into the ACO/PCMH. 3. Engages with population health team on Quality Based programs in the clinics. 4. Offers solutions to road blocks while implementing programs. 5. Run needed reporting to track MIPS and ACO measures 6. Run CCM and Annual Wellness Visit outreach reporting for all clinics to work through and help with outreach. 7. Monitor high risk patients within the clinic and follow up appropriately with clinic staff or RN care coordinator within Population Health. 8. Helps to ensure all deadlines are met during transition stages. 9. Offers education and tutoring to nurses involved in the program. May also provide education to all providers if needed. 10.
