SEE SCHEDULE O I. GENERAL PROGRAM SERVICE INFORMATION MOUNTAIN VIEW NURSING HOME, INC. ("MVNH") IS A TAX-EXEMPT, PENNSYLVANIA NOT-FOR-PROFIT CORPORATION ORGANIZED UNDER SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE WHICH OWNS AND OPERATES A 180-BED LONG-TERM CARE, SKILLED NURSING, AND REHABILITATION FACILITY. MVNH IS LOCATED IN THE CITY OF SCRANTON APPROXIMATELY FOUR AND A HALF MILES FROM COMMUNITY MEDICAL CENTER DBA GEISINGER-COMMUNITY MEDICAL CENTER (GCMC). MVNH PROVIDES NURSING AND MEDICAL SERVICES TO RESIDENTS AT VARYING LEVELS OF CARE INCLUDING REHABILITATIVE THERAPIES, ASSISTANCE WITH DAILY LIVING ACTIVITIES, AND 24- HOUR SKILLED NURSING CARE. THE REHABILITATIVE THERAPIES THAT MVNH PROVIDES INCLUDE PHYSICAL, RESPIRATORY, OCCUPATIONAL, AND SPEECH. THE THERAPIES ARE DESIGNED TO ASSIST RESIDENTS IN THEIR PURSUIT OF IMPROVED HEALTH AND WELLNESS. FIFTY-EIGHT PERCENT OF MVNH DISCHARGES WERE TO HOME OR OTHER LESS ACUTE SETTINGS. IN ADDITION TO NURSING ASSISTANCE AND SKILLED NURSING, MVNH ALSO HAS A 24- BED UNIT SPECIALIZING IN THE CARE OF INDIVIDUALS WITH ALZHEIMER'S DISEASE. THIS UNIT'S STAFF IS SPECIALLY TRAINED TO DEAL WITH RESIDENTS WITH DEMENTIA, ESPECIALLY THROUGH BEHAVIORAL MANAGEMENT AND COMMUNICATION TECHNIQUES. TO ENHANCE THEIR QUALITY OF LIFE, ALL RESIDENTS HAVE ACTIVITIES INCORPORATED INTO THEIR DAILY ROUTINE. THERE ARE MANY ACTIVITIES THAT ARE AVAILABLE, SUCH AS EXERCISE PROGRAMS AND PET THERAPY. EACH ACTIVITY IS SELECTED FOR THE RESIDENTS BASED ON INDIVIDUAL NEEDS AND ABILITIES. IN FY2018, MVNH PROVIDED THEIR SERVICES TO 334 INDIVIDUALS. II. UNCOMPENSATED CARE MOUNTAIN VIEW NURSING HOME, INC. RECOGNIZES ITS MISSION IS TO SERVE ALL MEMBERS OF THE COMMUNITY WITH RESPECT TO THE PROVISION OF HEALTHCARE SERVICES AND EDUCATION. IN THIS REGARD, MVHN PROVIDES FREE OR SUBSIDIZED CARE BELOW COST AND SUPPORTS VARIOUS HEALTH-RELATED ACTIVITIES AND PROGRAMS IN SUPPORT OF THE COMMUNITY. A. MEDICARE/MEDICAID IN RECOGNIZING ITS MISSION TO THE COMMUNITY, MVNH PROVIDES SERVICES TO THE ELDERLY (MEDICARE) AND THE INDIGENT (MEDICAID). MVNH PROVIDES CARE, BELOW COST, TO PERSONS COVERED BY THESE GOVERNMENTAL PROGRAMS TO THE EXTENT THE GOVERNMENT REIMBURSEMENT IS BELOW THE COST OF PROVIDING HEALTH CARE. THE UNREIMBURSED VALUE OF MEDICARE AND/OR MEDICAID IS EQUAL TO THE COST OF PROVIDING SERVICES LESS THE AMOUNT RECEIVED AS REIMBURSEMENT UNDER THE PROGRAM. THE COST OF PROVIDING HEALTHCARE SERVICES TO MEDICARE AND MEDICAID PATIENTS EXCEEDED REIMBURSEMENT BY APPROXIMATELY 3,776,725 DURING THE FISCAL YEAR ENDED JUNE 30, 2018. B. OTHER UNCOMPENSATED PATIENT SERVICES IN ADDITION, MVNH PROVIDES SERVICES FOR WHICH FULL PAYMENT IS NOT RECEIVED. THE UNCOMPENSATED COST OF PROVIDING SUCH PATIENT SERVICES DURING THE FISCAL YEAR ENDED JUNE 30, 2018, WAS 1,260,514. III. COMMUNITY BENEFIT SUMMARY MEDICAID SHORTFALL 3,776,725 UNCOMPENSATED CARE 1,260,514 TOTAL 5,037,239 IV. OPERATIONAL CHANGES EFFECTIVE FEBRUARY 1, 2018, MOUNTAIN VIEW NURSING HOME, INC CEASED OPERATIONS AND SOLD ITS BUILDING TO MOT MENASHE REALTY, LLC AND ITS EQUIPMENT TO MOUNTAIN VIEW CARE AND REHABILITATION CENTER. BOTH TRANSACTIONS WERE CONDUCTED AT FAIR MARKET VALUE AND IN ACCORDANCE WITH OUR GOVERNING DOCUMENTS. THE FACILITY IS NOW OPERATED BY MOUNTAIN VIEW CARE AND REHABILITATION CENTER, AN ORGANIZATION NOT AFFILIATED WITH GEISINGER.Mountain View Nursing Home is a well-established organization.
