RN/LPN - Flex 1, 2, or 3 Per Diem Program
Must be an Experienced RN or LPN to apply
Available Shifts: 3pm-11pm weekdays and 7am-3pm weekends
Per Diem positions where employees commit to work:
Flex 1 work 1 shift every other weekend.
Flex 2 work 2 shifts to include every other weekend.
Flex 3 work 3 shifts to include every other weekend.
The position is float - shifts will be assigned based.

Rotating weekends
Additional shifts may be worked. These shifts will be assigned according to the needs of the organization. Second and third shift differential will apply to these additional shifts. In the event the facility requests additional coverage, any time over forty hours worked will be paid at time and one-half.
Required experience:
**Only experienced RN/LPN with a minimum of 1 year of experience will be considered for Per Diem positions.
**Experience preferred in Skilled Nursing Facility/Long-Term Care environment.
CT Licensure: RN or LPN
Current BLS/CPR: Only excepts American Red Cross or American Heart Association

Chart on his/her shift in accordance with facility policy, state guidelines and residents status.
Supervise serving of prescribed diets and fluid intake; report persistent unresolved problems to the physician and/or DON or appropriate discipline.
Timely and accurately prepare incident reports.
Timely and accurately complete admission assessments and appropriate discharge documentation.
Supervise residents who self-administer medication while following community and regulatory guidelines
Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living.

Qualifications/Skills/Educational Requirements
Requires a LPN RN license.
Ability to lift at least 51 lbs on a regular basis and ability to help residents who require physical assistance.
Fluently read, write, speak, and understand the English language.
May require experience with caring for residents with memory impairment if assigned to memory care areas of a community.

GENERAL DESCRIPTION
The households at Mozaic Senior Life will each be led by a team consisting of a Clinical Nurse Leader (RN/LPN), Household Coordinator (non-clinical leader), and Lead Homemaker. These leadership teams will work to create home, community, family, and provide Residents with the highest quality of life and quality of care in a household model of care. Residents will be provided by a team of household staff working in an environment that supports their personal growth and job satisfaction. The Clinical Nurse Leaders and Household Coordinators will form a Neighborhood Council which will serve all households in a guiding and supporting role under the guidance of the Vice President, Nursing and the Administrator.
The Clinical Nurse Leader performs nursing duties within the scope of practice as defined by the State of Connecticut and provides nursing care to household Residents in accordance with the Core Values of Mozaic Senior Life and practices within the scope of the role of RN/LPN. The Clinical Nurse Leader assumes the responsibility for the overall delivery of care and supports the household team to ensure quality of care and quality of life.
Hours Per Week - (30 hrs. & 37.5 hrs. available) 3:00pm - 11:00pm shift, must be available to work every other weekends.
