Job Overview:
The autonomous nature of the Licensed Clinician role lets clinicians function like a private practice without the hassle of managing a back office.
Ability to work within a 40-hour work week! The majority of your time will be clinically focused, with the remainder focused on patient documentation and facility correspondence
You would be an on-site clinical provider working with residents and their families.
Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents' emotional, social, and psychological needs.

Provide initial assessments, utilize psychotherapy to address mental health issues and offer ongoing support
Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
Key Requirements:
We're looking for someone with an entrepreneurial spirit accompanied by strong medical knowledge, understanding of documentation (CMS, Medicare, Medicaid) and nursing home/ hospital facility experience.
Have a master's degree and a valid LISW/LMFT/LMHC license in the state for which you are applying.
Two years of counseling and therapy experience
Ability to work as a member of a team with a great attitude
Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient's family, nursing staff, facility social services team, other MPAC providers, etc).
High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met
Comfortable with the ever-changing environment of a skilled nursing facility

Responsibilities
Reviews requisitions for physical therapy and evaluates patient's physical condition and needs.Establishes PT treatment goals for each individual patient, based on the findings of the initial evaluation.Develops a program of corrective exercise and treatment for assessed patients.Supervises the treatment and recording of patient's progress.Determines proper equipment usage, instructs the patient in appropriate program, and modifies program as needed.Teaches patient, other staff members, and student affiliates in passive, active assistive, or resisted exercise, ambulation, and other physical movements in order to articulate and demonstrate principles of rehabilitative therapy.Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.Enhances professional growth and development through participation in educational programs, current literature, workshops, and inservice meetings.Documents as required by the profession and department, such as evaluation results, treatment plans, progress reports, and other such reports.Collaborates with support personnel according to legal standards and ethical guidelines.Establishes a PT Plan of Care based on functional deficits and patient goals. Completes objective testing for range of motion, strength, and performance of daily functional tasks. Completes therapeutic modalities, as appropriate, including ultrasound, iontophoresis, and electrical stimulation. Administers corrective exercise or manual techniques for stabilization, facilitation, or inhibition of movement. Completes daily documentation and charging in Therapy EMR. Communicates with the referring provider on patient progress and treatment recommendations. Provides supervision to Physical Therapy Assistants, in accordance with regulatory agency guidelines. Utilizes standardized outcome measures in a uniform manner to ensure functional performance is accurately reflected.

As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:

Working under the supervision of the PACE Center Manager, the PACE Social Worker is independent in the application of social work knowledge and skills. He/she is responsible for providing direct social work care management services to the participants of the North East Medical Services PACE program and works collaboratively with the PACE Interdisciplinary Team (IDT) to manage long-term care needs.
ESSENTIAL JOB FUNCTIONS:
Conducts psychosocial assessments to determine PACE Participants' care needs, preferences, and goals.
Participates as an active member of the IDT for Participant care planning.
Documents accurately and thoroughly in the Participants' electronic health record in accordance with industry and departmental standards including time requirements.
Communicates effectively and openly with other Interdisciplinary Team members on participant status and needs.
Maintains current and thorough written case management records, including periodic reassessments of program Participants.
Provides supportive counseling to Participants and/or their family members, as indicated.
May develop and lead group counseling, support groups, and/or other relevant activities.
Refers Participants and families to appropriate community agencies or facilities, acts as a liaison with such organizations, and as an advocate for participants.
Consults with and advises staff members regarding the social, emotional, and cultural factors associated with health and medical care, and as to the availability of social services in the community.
Works with the IDT to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.)
Participates in program and policy development related to social work.
Understands and demonstrates respect for Participants' rights and utilizes established mechanisms for management of ethical issues in Participant care.
Demonstrates active participation in Quality Improvement activities, staff training, and meetings.
Performs home visits, as necessary.
Works closely with the Enrollment Team to support enrollment of prospective Participants in the PACE program.
Provides care to Participants in-person, via telehealth, and/or via telephone, as indicated.
Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
Performs other job duties as required by manager/supervisor.
May provide after-hours on-call coverage to support PACE participants when the PACE Center is closed as needed for support.
QUALIFICATIONS:
Required:
Master's degree in Social Work from an accredited college or university required.
Current BLS and First Aid required
Preferred:
Current CA licensure as a Licensed Clinical Social Worker (LCSW) preferred
Minimum 2 years of experience in a health-related area preferred
Minimum one year of experience working with a frail and/or older adult population preferred
Valid California Driver's license with good driving history within the last 5 years.
LANGUAGE:
Must be able to fluently speak, read and write English.
Fluency in Chinese (Cantonese and/or Mandarin) is required.
Fluency in other languages is an asset.
