RESPONSIBILITIES & QUALIFICATIONS
Responsibilities:
Identifies on-going needs and services of residents in promotion of the highest quality resident services to be delivered.
Ensures proper follow through, participates with evaluation and documentation for residents with a change in health care services and status.
Assists Resident Care Director (RCD) in conducting resident evaluations and data gathers information at time of move in, 30 days post move in, semi-annually and with any change in status.
Conducts monthly wellness visits for all residents.
Monitors, and follows up with residents who have experienced any fall, accident, or incident; documents and reports as required and directed.
Documents all pertinent information in the resident wellness file.
Maintains communications with resident's family and or responsible party to ensure medical needs for the resident are being met.
Contacts resident's attending physician when necessary and/or upon family request.
Ensures weights and vital signs are obtained monthly for each resident.
Assists in maintaining wellness files according to Sunrise policies and federal and state regulations.
Maintains medical supplies and emergency kits for the community.
Provides clinical support and assistance to community team members as needed.
Understands and follows infection control practices.
Partners with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks.

Medication Administration:
Provides the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about residents.
Receives medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Ensures that medications are passed according to times.
Documents and initials as medications are given and ensures that appropriate documentation is completed for refusal or missed doses.
Maintains confidentiality of all resident information including resident medication among other residents.
Reports all resident concerns made while administering the medication to the RCD or Wellness Nurse.
Restocks medication cart after all medication passes.
Assists in checking medication regardless of packaging system.
Assesses the residents to determine need for "as needed medication" and appropriately documents and reports to supervisor.
Counts all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift.
Maintains and cleans the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follows re-fill process for medications.
Helps residents maintain independence and promotes dignity and physical safety of each resident adhering to the Sunrise Principles of Service.
Strives to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns, history, and basic human needs.
Practices routinely good standard care precautions of cleanliness, hygiene, and health.
Resident Care Communications:
Notifies RCD of any resident and/or family concerns through the LEAD process.
Attends and actively participates in daily Cross Over meetings facilitated by the LCM.
Participates in the development of the Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
Risk Management and General Safety:
Partners with community team to ensure community is in compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Reports all accidents/incidents immediately.
Reports all unsafe and hazardous conditions/equipment immediately.
Ensures any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.
Complies with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.
Understands and practices the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.

The Senior Skilled Nursing Administrator, reporting to the Vice President of Operations, is responsible for overseeing eight skilled nursing communities. This role provides strategic leadership, operational oversight, and ensures compliance with regulatory requirements while maintaining quality care and efficient operations across all locations. • Conduct regular audits of community operations, documentation, and practices to identify and address deficiencies.

• Support communities in preparing for surveys, inspections, and implementing corrective action plans when necessary.
• Provide support during federal and state surveys.
Quality Care and Resident Satisfaction
• Monitor the delivery of resident care across communities to ensure it meets organizational and regulatory standards.
• Oversee the implementation of quality improvement initiatives to enhance resident outcomes and satisfaction.

Qualifications:

Job Summary
Employed unlicensed assistive staff members who have successfully completed at least one clinical rotation and are actively enrolled in an accredited RN program. Accountable to, and work under the direct supervision of a professional registered nurse or APRN provider to implement delegated aspects of nursing care. Provide patient care, support independent and dependent activities of daily living, vital signs, mobility and ambulation, phlebotomy, point of care testing, EKG testing, peripheral IV removal, specimen collection, and foley catheter insertion and removal, and comply with patient safety practices under the direction of the professional RN nurse. Under general supervision, the successful candidate will perform intermediate professional work providing direct clinical service to clients involving intake, assessing client needs, developing treatment plans, providing prevention services , managing cases, providing therapy, maintaining records and files, preparing reports, and related work as apparent or assigned. Essential Functions
Performs triage of referrals, intakes, client assessments and diagnostic interviews; conducts mental health and substance use disorder assessments and monitors level of functioning.
Manages all clinical needs of CSU clients on assigned shift.
Assists client in identifying self-determined goals and jointly prepare individualized service plan.
Develops and facilitates psycho-educational groups; provides individual, family and group counseling; implements treatment plans; coordinates daily treatment schedule.
Coordinates case management services; coordinates and links client to community resources and medical services; interfaces with other service providers and client support system.

Provides consultation, education and prevention services.

Critical Care Paramedic: Flight and Ground

Provides comprehensive emergency medical care to the critically ill and injured during helicopter and ambulance transport from accident scenes, and referring hospitals, under the direction of medical control physicians. Receives direct supervision from the Operations Managers. May at times receive functional supervision from the VP Director, AVP Clinical Operations, and Medical Director. Must maintain a weight not to exceed 250 pounds in gear. Required to work in confined space with exposure to extreme temperatures, vibration and altitude; and to stand for extended periods of time. Required to physically lift and transfer patients, and lift and move equipment used for patient care and other sensory facility to provide patient care. Communicates directly and uses aircraft headsets, radios and telephones with patients, physicians, and other staff in emergency and daily patient care activities. May be exposed to personal safety hazards in both air and ground transport.
Primary Duties and Responsibilities
Delivers expert patient care in helicopters and ambulances to multiple patient types including scene work, adult subspecialties, pediatrics, and high-risk obstetrics. Assesses patient care needs, establishes priorities and delegates tasks to peers and ancillary support staff as appropriate.
Assesses the needs of patients with highly complex problems. Identifies current and potential complex problems, critically analyzes trends and changes in patient status, and plans appropriate interventions directly or indirectly through other nurses and paramedics
Performs basic and advanced airway techniques and treatment modalities, including oxygen therapy, airway maintenance, assisted ventilation, insertion of supraglottic airways, nasal and orotracheal intubation, rapid sequence pharmacological induction to facilitate intubation, surgical cricothyroidotomy, and needle thoracostomy.
Administers and maintains pharmacologic agents and intravenous fluids at the scene and during inter-hospital transfer.
In collaboration with the Critical Care Transport Specialist - RN, monitors and evaluates invasive and non-invasive techniques/procedures and their therapeutic effects, including the monitoring and operation of the intra-aortic balloon pump (IABP), Ventricular Assist Devices (VAD), and intracranial monitoring devices during transport. Troubleshoots these devices, recognizes problems, and makes necessary corrections. Recognition and intervention of abnormal parameters includes potentially lethal cardiac dysrhythmias requiring electric cardioversion and defibrillation.
Performs a variety of clinical duties including, but not limited to: insertion of nasal and oral gastric tubes and management of autologous blood transfusion systems. Initiates and maintains venous access and intraosseous lines.
Obtains patient information, including detailed reports, when appropriate, and disseminates all necessary information to appropriate medical staff, per protocol. Responsible for monitoring and recording all patient data and completion of all appropriate clinical documentation in a timely manner.
Maintains program readiness and ensure cleanliness and appropriate par levels of supplies used by MedSTAR Transport. Ensures helicopter or ambulance readiness with daily review of equipment and supplies and records findings on daily vehicle checklists.
Ensures system readiness post-transport. Ensures that par stock levels of equipment at remote bases are maintained and informs MedSTAR leadership of supply needs, as necessary.
Establishes and maintains professional radio communications with MedSTAR Communications Center and all other agencies, adhering to all FCC protocols.

Position Requirements
Minimum: Bachelor's degree in social work, psychology, rehabilitation counseling or human services - related field from an accredited college with a least one year experience working with the seriously mentally ill; must be able to submit official transcripts; must have evidence of 1,500 hrs of supervised experience obtained within 5-yr period. Two year's experience working with dually diagnosed clients preferred. Must qualify has a QMHP-A. Two years of experience working with dually diagnosed clients preferred. Assists in providing safe and effective client care in assigned areas, to include assisting with personal care, close monitoring, acting as part of multi-disciplinary treatment team and facilitating psycho-educational groups. Assists with patient recreational activities. Full time position must be able to work all shifts, and every other weekend.

Summary of Job Duties : The On-Call Community Care Aide (CCA) is responsible for helping frail, older adults (age 55+) with their activities of daily living (ADLs) and with performing personal care services at their place of residence. This position requires the selected candidate to be able to respond to calls and be available to cover care during the on-call time frame for their designated areas. A reliable vechicle and valid PA driver's license are required.
Required Education: High school diploma or equivalent.
Required Experience: One or more years of verifiable experience working with the frail/elderly population is required. Experience as a personal care aide, CNA, home health aide or nursing assistant is desirable. Essential Functions
Assesses client service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment.
Develops an individual service plan of care and services appropriate to the evaluation with the client/family consensus, reviews and updates service plans.
Makes referrals and linkages to appropriate agencies for services, coordinates client services and treatment with multiple service providers and agencies.
Evaluates the quality of services provided and changes in client's condition and counsels clients as necessary; evaluates client's environment for safety, security, negative factors and productivity; compiles and analyzes data relating to complaints; identifies and works to resolve problems.
Assists clients with activities of daily living including hygiene/bathing.
Serves as liaison to public agencies and provides information regarding Authority programs and services.
