The Supervisor acts as the primary designee in the absence of the Manager or Director, assuming additional responsibilities as delegated. This position is also responsible for serving as a liaison with clinical leadership and other stakeholders for decision-making, problem solving, and specific projects. The Supervisor serves as a role model and change agent, supporting the Manager or Director by supervising daily operations and providing timely feedback to support and maintain quality patient care in an efficient, productive and healthy work environment. In addition, the Supervisor of Care management may carry a modified patient case load, performing all the duties of a Hospital Social Worker, and ensuring compliance with CMS and other regulatory initiatives.
