Performs initial and interim assessment and evaluation of the client's level of functioning. Problem identification and goal setting. Develops the plan of treatment in consultation with the physician. Provides physical therapy in accordance with the plan of treatment. Revises the plan of treatment as needed. Interdisciplinary coordination and adherence to the plan of care. Provides patient/family education as appropriate. Qualifications What's Required Graduate of a physical therapy curriculum approved by the American Physical Therapy Association. Licensed by the State of Michigan. We are partnered with Mary Free Bed, so rest assured we provide the best to our employees so they can give the best to our patients. In this role, you will assume full responsibility for assessing patient needs for physical therapy which includes planning and executing individual treatment plans in accordance with the State of Michigan licensure and PT scope of practice Along with providing quality treatment to our patients, you will provide education and support to both patients and their family including written instructions as necessary. You will be part of a great team! You will work closely in department and interdepartmental efforts to improve process and clinical treatment. Qualifications What's Required Bachelor's degree graduate from an accredited school of Physical Therapy. State of Michigan Physical Therapy License Additional Information Are you Munson Material?
