Per-Diem Meals on Wheels Delivery Driver
Delivering meals with care to make a difference in someone's life! As a Meals on Wheels Delivery Driver you will be delivering meals to older adults and adults living with disabilities in the community. This also provides our homebound consumers with an opportunity to socialize and see a friendly face.
Schedule: This is a per-diem position and can be up to 15 hours/week. Monday - Friday between the hours of 9am -1pm.
Compensation: $22.50 per hour.
This is a multi-route position and you can be assigned to deliver meals in any of the following communities: Chelsea, Everett, Malden, Medford, Melrose, North Reading, Reading, Revere, Stoneham, Wakefield, and Winthrop.

The Resident Services Coordinator works within the framework of the Executive Office of Aging & Independence (AGE) regulations. Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid (in-person & remote, required 1-2 days in office) work schedule (once training is complete.) No nights or weekends.
Compensation: Starting annual salary range is $54,922.14 - $59,166.74. Job Activities
Key activities of the position are below. Additional duties may be assigned as required.
Top Key Activities:
Assess consumer history, current status, living situation, support system, and if applicable, caregiver's history, and identify unmet needs, which inhibit independent living.
Work in collaboration with the Housing personnel and Supportive Housing Team Managers to ensure residents have access to community services and public benefits
Is available and provides information, referrals, service coordination, activity planning, and follow-up to all building residents
Collaborate with contracted provider agencies to coordinate and assign service staff to consumers/residents.
Case manages and supports residents and community members based on individual needs and the organization's mission and goals.

Qualifications:

The Role

The Chief Financial and Operating Officer (CFOO) will succeed a long‑tenured, well‑respected leader and will inherit an exceptional team of committed and experienced staff. The CFOO will join MHP's five‑member Steering Committee (senior management team) and will ensure that MHP has the infrastructure and systems in place to achieve its mission effectively, be financially and operationally sustainable, and mitigate risk.
Key Responsibilities Leadership and Strategy
Oversee short‑ and long‑term budget planning, forecasting, and resource management in alignment with MHP's mission and strategic plan; identify opportunities to increase net income, unrestricted net assets, and to secure program funding where appropriate and as opportunities arise.
Regularly undertake business modeling, planning and stress testing; and align resources to meet agreed upon organizational objectives.
Regularly assess organizational performance against the annual budget and financial forecasts and make recommendations on financial strategy, staffing, and business goals.

Carry a case load of approximately 50 unemployed and unenrolled program participants
Conducted at minimum 25 participant interactions a week including bi-weekly coaching sessions and/or weekly non-compliance/re-engagement attempt meetings
Coordinate and develop relationships with all external Providers (including Secondary Service Providers, District Agency Providers, School, Employers, and Community Providers) as well as Teammates and ensure that participants are making meaningful progress toward employment.
Review DHS-administered assessments, develop and amend participants' Detailed Individual Responsibility Plans (DIRPs), and help participants overcome barriers to program engagement and participation.
Develop trusted working relationships with assigned TANF participants and monitor participant engagement and progress up through participants first year of employment.
Provide counseling, intervention, and program action planning and delivery.
Facilitate the successful utilization of job readiness training, rapid job searching, job retention services, and/or education services.
Document activities, participation, compliance, and employment progress in the Electronic Case Management (ECM) systems.
Stress urgency and ownership, guiding participants through a learn-by-doing process during their service planning and delivery.

Coordinate multiple activities outlined in the participant's DIRP and assist participants in reporting work participation.
Ultimately responsible for the engagement of Participants and works with all staff to identify appropriate activities, resources, and supports for participants.
Performance Management and Reporting
Consistently meet or exceed performance targets and KPIs, including job placements, employment retention, school completions, case closure due to earnings, customer satisfaction and audit scores.
Ensure 100% accuracy in internal data systems, including timely recording of employer activity, jobseeker interactions, and placement outcomes
Work closely with internal teams to maintain service consistency and accountability, improving overall efficiency in services

Receive and investigate allegations of abuse, neglect, financial exploitation, and self-neglect of an older adult (60+) in a timely manner as defined under the regulations governing the protective services program.
Provide case work services to protective service consumer population, by responding to crisis situations with field visits to assess an older adults physical, social, emotional and environmental status
Conduct written assessments of risk, decisional ability and functional abilities. While also providing short-term case management and making referrals to alleviate substantiated allegations.
Work closely with community agencies and organizations including first responders, community safety personnel and medical professionals to identify needs, locate resources, and set up service network to meet needs.
Develop and maintain ongoing service plans to assist in the resolution of the Protective Services casework.
Maintain case records; collect information for program monitoring and evaluation.
Assume 24-hours on-call responsibility as determined by Protective Services Supervisor.

The ASAP RN is responsible for performing the duties required by the Clinical Assessment and Eligibility (CAE) Program established by the Executive Office of Elder Affairs (EOEA) and the Office of Long-Term Services and Supports (OLTSS) and in contract with MassHealth, and the MA State Home Care Program. The RN must be knowledgeable about the programs and services available to consumers throughout the 16 communities in the Minuteman Senior Services service area. The RN is part of an interdisciplinary care management team and participates in interdisciplinary meetings with community service providers including care managers, nursing home staff, physicians, nurses and therapists from home health agencies, hospice providers, nutritionists, housing managers, mental health professionals, and other community home and health care professionals.
ESSENTIAL FUNCTIONS:
Review referrals provided by Care Managers for each consumer determined to be in need of home care services, participate in Case Interdisciplinary Reviews, and consultations with Care Managers as needed.
Conduct initial home visits to assess consumer's health status, determine need for service and appropriate level of service needed, and develop a personal care plan if needed.
Provide yearly reassessments as deemed appropriate by state guidelines and interdisciplinary conference.
Serve as a consultant to our Protective Services program on health-related issues.
Provide pre-admission screening of Medicaid consumers seeking long-term care services with the consideration of a community-based alternative plan which may require an in-home assessment.
Track consumers who have been deemed ineligible for nursing home placement either due to alternative community services or not meeting the Medicaid regulations for long-term nursing home placement.
Process cases of all screening requests according to MassHealth and EOEA provisions.
Provide on-site meetings at skilled nursing facilities to meet the requirements of the Long-Term Care Options program for possible discharge and ASAP service coordination.
Assist with provision of health care related in-service training to ASAP personnel.
Assess consumers for clinical assessment and eligibility (CAE) for all MassHealth and State Home Care Programs.
