The RN must be knowledgeable about the programs and services available to consumers throughout the 16 communities in the Minuteman Senior Services service area. The RN is part of an interdisciplinary care management team and participates in interdisciplinary meetings with community service providers including care managers, nursing home staff, physicians, nurses and therapists from home health agencies, hospice providers, nutritionists, housing managers, mental health professionals, and other community home and health care professionals. ESSENTIAL FUNCTIONS: Review referrals provided by Care Managers for each consumer determined to be in need of home care services, participate in Case Interdisciplinary Reviews, and consultations with Care Managers as needed. Conduct initial home visits to assess consumer's health status, determine need for service and appropriate level of service needed, and develop a personal care plan if needed. Provide yearly reassessments as deemed appropriate by state guidelines and interdisciplinary conference. Serve as a consultant to our Protective Services program on health-related issues. Provide pre-admission screening of Medicaid consumers seeking long-term care services with the consideration of a community-based alternative plan which may require an in-home assessment.
