The RN (Registered Nurse) Quality Assurance Coordinator is responsible for assisting in overall quality improvement activities for Nascentia Health CHHA and LHCSA divisions.
Job Requirements:
Reviews clinical records.
Participates in review of chart audits utilizing SHP reports (if applicable) and best practice following policies and procedures.
Responsible for completing all LHCSA audits
Reviews client records, orders and other communication for quality and safety of care.
Analyzes documentation and reports trends to Manager
Follows up with staff/management regarding issues identified in review process.
Assist clinical leadership in working with clinical staff on questions, OASIS education/clarification, documentation issues, and clinical care interventions.
Communicates with Manager regarding issues or concerns with clients and/or clinical staff.
Follows up with staff regarding documentation deficiencies and/or discrepancies with OASIS, coding and assessment.
Participates in formal and informal training opportunities for clinical staff.
Performs quality improvement activities as assigned by Manager of Quality Improvement.
Communicates agency policies to staff.
Promotes quality improvement as it relates to the activities of clinical staff in accordance with organizational policies and procedures.
Maintains knowledge of and adherence to agency policies, procedures, along with quality improvement standards.
Demonstrates knowledge and use of agency policies and procedures.
Demonstrates knowledge of quality improvement standards.
Demonstrates knowledge of OASIS assessments.
Has a working knowledge of Medicare/Medicaid and private insurance
Ability to learn and apply coding principles to home care.

Requirements:

The Enrollment Coordinator II will act as a point of contact to assist team members with unfamiliar or unusual calls, enrollment, and dis-enrollment scenarios, and ensure the highest quality of customer service and best practices. The ISNP Nurse Practitioner Medical Provider (NPMP) is a position that embraces a collaborative Team approach to enhance care. You will be supported by a great group of physicians and NP partners who are knowledgeable and extremely approachable. You will be overseeing member care as the NPMP, consulting regularly with the interdisciplinary staff and allied health providers to provide a seamless delivery of care and services in accordance with CMS guidelines.
As a key member of the clinical team, you will practice evidence-based medicine within your scope of practice, including: preventive screening and health promotion; diagnosis and treatment of a wide variety of common, acute, and complex problems; ordering and interpreting diagnostic tests; prescribing medications; referring to specialists and collaborating other providers as appropriate; providing basic emergency care if necessary; and providing education and resources to members, families, and staff.
Responsibilities
Clinical
* Perform comprehensive assessments and document findings in a complete, concise manner that is compliant with Nascentia documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
* Deliver cost-effective, quality care to assigned members
* Regularly evaluate the plan of care for cost effectiveness while meeting the needs of members, families and providers to decrease high costs, poor outcomes and unnecessary hospitalizations
* Manage medical and behavioral, acute and chronic conditions effectively, and in collaboration with a Care Team providers or specialty providers.
* Responsible for ensuring that all diagnoses are ICD10, coded accurately, and documented appropriately to support the diagnosis at that visit
* The NPMP is responsible for ensuring that all quality elements are addressed and documented per Nascentia and CMS guidelines
* The NPMP will perform scheduled medication reviews and post-hospitalization medication reconciliations
* Facilitate agreement and implementation of the members plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians
* Evaluate the effectiveness, necessity and efficiency of the plan in conjunction with the Team, making revisions as needed
* Utilizes evidence-based practice guidelines and protocols established by Nascentia and CMS
* Must attend and complete all mandatory educational and training requirements
* Must attend and provide evidence of population-based CME training annually
Care Coordination
* Understand the Payer/Plan benefits, CCM associate policies, procedures and articulate them effectively to providers, members and key decision-makers
* Assess the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members
* Coordinate and communicate plan of care as members transition through different levels of care and care settings
* Monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change
* Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the members needs and wishes
* Regular and effective communication with Care Team members, family or key decision-makers, nursing staff and specialists
* Actively promote and support the Nascentia Health ISNP program in assigned facilities by modeling professional excellence and partnering with key stakeholders (i e : internal sales function, provider relations, facility leader) to maintain and develop membership growth

The ISNP Nurse Practitioner Medical Provider (NPMP) is a position that embraces a collaborative Team approach to enhance care. You will be supported by a great group of physicians and NP partners who are knowledgeable and extremely approachable. You will be overseeing member care as the NPMP, consulting regularly with the interdisciplinary staff and allied health providers to provide a seamless delivery of care and services in accordance with CMS guidelines.
As a key member of the clinical team, you will practice evidence-based medicine within your scope of practice, including: preventive screening and health promotion; diagnosis and treatment of a wide variety of common, acute, and complex problems; ordering and interpreting diagnostic tests; prescribing medications; referring to specialists and collaborating other providers as appropriate; providing basic emergency care if necessary; and providing education and resources to members, families, and staff.
Responsibilities
Clinical
Perform comprehensive assessments and document findings in a complete, concise manner that is compliant with Nascentia documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
Deliver cost-effective, quality care to assigned members
Regularly evaluate the plan of care for cost effectiveness while meeting the needs of members, families and providers to decrease high costs, poor outcomes and unnecessary hospitalizations
Manage medical and behavioral, acute and chronic conditions effectively, and in collaboration with a Care Team providers or specialty providers.
Responsible for ensuring that all diagnoses are ICD10, coded accurately, and documented appropriately to support the diagnosis at that visit
The NPMP is responsible for ensuring that all quality elements are addressed and documented per Nascentia and CMS guidelines
The NPMP will perform scheduled medication reviews and post-hospitalization medication reconciliations
Facilitate agreement and implementation of the member s plan of care by engaging the facility staff, families/responsible parties, primary and specialty care physicians
Evaluate the effectiveness, necessity and efficiency of the plan in conjunction with the Team, making revisions as needed
Utilizes evidence-based practice guidelines and protocols established by Nascentia and CMS
Must attend and complete all mandatory educational and training requirements
Must attend and provide evidence of population-based CME training annually
Care Coordination
Understand the Payer/Plan benefits, CCM associate policies, procedures and articulate them effectively to providers, members and key decision-makers
Assess the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members
Coordinate and communicate plan of care as members transition through different levels of care and care settings
Monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change
Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the member s needs and wishes
Regular and effective communication with Care Team members, family or key decision-makers, nursing staff and specialists

As a Care Coordinator, you play a vital role in supporting patient care by ensuring services are authorized, scheduled, and coordinated efficiently. Working closely with Care Managers, clinical teams, and community agencies, you help keep care plans on track and patients supported often behind the scenes, but always essential.

About the Role
The Care Coordinator provides administrative and technical support to the Care Coordinator Supervisor and interdisciplinary team, including the members. This role focuses on the operational aspects of care coordination, including telephonic communication and coordination of services, authorization processing, service coordination, and ongoing communication with Members and care partners.
This position is onsite at a Nascentia Health office, Monday Friday, 8:00 AM 4:30 PM.
What You ll Do
Support efficient, accurate care coordination through strong organizational and communication skills.
Key responsibilities include:
Review, process, and enter authorization data accurately into computerized systems
Monitor and manage expiring authorization reports to ensure continuity of services
Assist Care Managers with scheduling and coordinating Member services as directed
Collaborate regularly with clinical team members and participate in team meetings
Act as a resource to Care Managers, supporting scheduling and tracking of Member contacts, visits, and UAS assessments
Answer and triage phone calls, respond to questions, and escalate issues as appropriate
Communicate with Members on a regular basis to support care coordination needs
Perform clerical and administrative tasks including filing, copying, faxing, typing, and message-taking
Participate in performance improvement and continuous quality improvement (CQI) initiatives
Perform other duties as assigned

High school diploma required; Associate s degree in a related field preferred
Two or more years of experience in healthcare or completion of a Medical Assistant program
Strong computer literacy with the ability and interest to learn new systems and technologies
Proficiency in Microsoft Office, internet-based tools, and healthcare software applications
Strong analytical and problem-solving skills
Ability to work independently while collaborating effectively with a team
Excellent verbal and written communication skills
Demonstrated discretion in handling confidential and sensitive information
Ability to adapt to changing priorities, deadlines, and workflows

What You ll Do as an Adult Homecare Physical Therapist
Provide high-quality, evidence-based physical therapy services through assessment, treatment, education, and care coordination. You ll be trusted and empowered to use your clinical expertise to improve patient outcomes.
Your responsibilities include:
Deliver safe, patient-centered physical therapy interventions in the home
Perform comprehensive patient assessments using best clinical practices
Develop, implement, and update individualized plans of care
Serve as a key member of the interdisciplinary care team, communicating patient progress and care needs
Educate patients and caregivers on mobility, safety, and therapeutic exercises
Ensure accurate, timely, and clear documentation (including OASIS, within regulatory timeframes)
Supervise and provide oversight of paraprofessional staff, including home health aides, as related to the patient s plan of care
Maintain compliance with infection control, quality improvement, and regulatory standards
Manage a diverse adult caseload while meeting productivity expectations

What We re Looking For!
Current New York State Physical Therapy license
Registered with New York State and the American Physical Therapy Association (as applicable)
