Provides oversight of care management through a collaborative process of assessment, planning, facilitation, and advocacy to meet members' health needs. Job Requirements: The Care Manager Supervisor supervises care management practice within the team concept to ensure a high quality of professional service and maintenance of Nascentia Health Options standards and policies. Provides direct oversight of the care manager s daily operations of utilization review practice, process and procedures ensuring accurate member care needs are met and documented per the managed long term care program (MLTC) regulations. Completes Quality/Utilization chart reviews per agency policy. Completes member care conferencing with care manager per agency standard. Provides oversight to care management team ensuring that the case management process is being facilitated and services provided to meet member s health needs. Ensures care management staff have completed all assessments, reassessments, and member contacts within regulatory time frames. Utilizes care standards and strategies when reviewing care plans. Participates in Quality Assurance and Improvement activities. Coordinates and assigns new members to the care management team. Oversight of member s hospital and skilled nursing facility admissions/utilization. Routinely communicates changes/updates to staff, other departments, and Regional Director of Care Management as necessary Requirements: 1. Strong communication skills (written and oral). 2. The Enrollment Coordinator II is responsible for assisting the Enrollment Supervisor in managing incoming calls, processing consumers inquiries/referrals, and processing all Managed Long-Term Care Plan (MLTCP) enrollments and disenrollments. Serves as an expert consultant for plan enrollments and dis-enrollments in addition to handling department escalations and audits. Through the direction of the Enrollment Supervisor, provides guidance and delegates necessary tasks to a team of Enrollment Coordinators. Assisting and overseeing daily Enrollment Coordinator responsibilities, which include regularly coaching and observing calls, answering incoming inquiries and referral calls. Processing plan enrollments and dis-enrollments. Communication and coordinating with internal teams as well as outside vendors, Local Department of Social Services (LDSS), and Maximus (sub-contractor for the Department of Health), Assistance. Processing Referrals, Prospective Enrollments, and Consumers in Zoho. Properly notating Guiding Care and Zoho with notes and appropriate alerts. Performance Responsibilities and Standards: Provide in-home consultation for policy and program development and operation and evaluation of nutrition services. Your responsibilities include: Deliver safe, patient-centered physical therapy interventions in the home Perform comprehensive patient assessments using best clinical practices Develop, implement, and update individualized plans of care Serve as a key member of the interdisciplinary care team, communicating patient progress and care needs Educate patients and caregivers on mobility, safety, and therapeutic exercises Ensure accurate, timely, and clear documentation (including OASIS, within regulatory timeframes) Supervise and provide oversight of paraprofessional staff, including home health aides, as related to the patient s plan of care Maintain compliance with infection control, quality improvement, and regulatory standards Manage a diverse adult caseload while meeting productivity expectations Stay current with clinical best practices and participate in ongoing education and in-services What We re Looking For! Current New York State Physical Therapy license Registered with New York State and the American Physical Therapy Association (as applicable) Valid NYS driver s license and automobile insurance One year of physical therapy experience preferred (home care experience a plus) Strong clinical judgment, organization, and ability to work independently Excellent communication and documentation skills Physical Requirements: Ability to sit or stand at a desk 90% of the day Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs. Ability to drive in ALL weather conditions (rain, sleet, snow, wind). Visual/hearing ability sufficient to comprehend written or verbal communication Ability to use telephone and communicate over the phone Frequently uses a laptop computer to enter orders and obtain product information Bloodborne Pathogens Exposure Category I - Employee has tasks who routinely involve exposure or contact with blood and body fluids.
