To provide nursing oversight to people that attend the Day Habilitation program, and support and guidance to the staff. Responsibilities Primary duties and responsibilities include but not limited to: Provides medical oversight and medical assessment of all people that attend the Day Habilitation to ensure their safety and well- being. Provide continuous training and support to the staff in order for them to efficiently meet each person's medical needs. Administer first aid as needed or instruct staff on necessary medical interventions. Reviews all medical records and paperwork, including annual physicals and medical visit appointments, on a regular basis and following doctor's orders as prescribed. Collaborates with the interdisciplinary team to ensure all medical concerns are addressed. Acts as a liaison with the family, residences and MD to coordinate care. Ensure that all medical documentation is comprehensive and conforms with agency policies and procedures and is maintained in the person's medical chart. Provide supervision to the LPN, and will be available to the LPN for any medical assessment or medical concern. Ensure implementation of all new medical policies and procedures by training staff and observation of practices. Ensure that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe or a potentially hazardous work environment must be immediately reported to supervisor. Performs additional related duties as requested by her supervisor. Physical aspects may include but not limited to lifting and transferring of an adult/child. Qualifications Qualifications: Associates degree in Nursing required. Obtain updated medication list on every visit, if any changes to medication noted; notify the provider. If no changes, check off medication reconciliation box in EHR. Collect from patient any forms or documents that needs review and sign off by the provider and inform provider. Prepare medical records for the clinician, include laboratory and diagnostic reports and stamped progress notes as needed. Obtain any pending imaging or testing that has been done after the last visit from patient or caregiver. Perform phlebotomy on adult and pediatric patients and obtain other specimens as per clinicians' order and in compliance with DOH and CLIA protocols 100% of the time. Perform EKGs and other office procedures as per clinician's orders. Complete vaccinations, medication standing orders (MSO), and PPD plantings or other task in scope of practice as ordered by the provider. Meet with patients and complete the required Intake packet/ intake updates and supporting documentation. Enter information in EHR as instructed. Assist with the examination of the patient, as directed by the clinician/nurse. Assist with therapeutic interventions to ensure patient/staff safety based on techniques learned in pre- service, during orientation and ongoing training. Assist with the check-out process as needed: print patient plans, referrals, or prescriptions. Refer patient to front desk to schedule return visit and any internal referrals. Enter prescription requests, form requests, etc., into the designated database and complete associated documentation (e.g., prescription fax forms). Update EHR accordingly. These systems should focus on individual health and safety, staff development and training in health management areas. Participate in the nursing on-call and weekend on-call systems. Under the direction of the Assistant Director of Nursing, the RN Supervisor is responsible for providing nursing oversight and case management to a designated home/caseload so as to ensure that all medical/nursing needs are being addressed. The RN Supervisor provides clinical direction and training to LPN staff and participates in rotating on call schedule so as to ensure 24-7 availability to LPN and AMAP staff in the program. Practices and promotes the agency's mission statement in every interaction. Responsibilities Primary duties and responsibilities include but not limited to: The RN Supervisor is responsible to manage all health care needs for a caseload of people in a particular residence or group of residences. This includes but is not limited to completion of quarterlies and annuals, tracking and completion of labs and appointments, quarterly nursing reviews, communication with medical providers. Perform MD rounds with the Medical Director and ensure that all MD orders are implemented timely and all recommendations/order are appropriately documented and completed. RN Supervisors provides clinical oversight to the LPN staff by providing training and guidelines for delegated nursing functions and ensuring that skills performed as in accordance with appropriate nursing standards and training provided. Includes periodic observation of medication passes and other delegated nursing functions. Provide feedback to AD of Nursing for inclusion into ongoing support meetings ad annual performance evaluation. RN Supervisor provides clinical supervision to AMAP staff including completing certification and re-certification, periodic inspection of delegated nursing functions.
