    Ensures the delivery of quality medical services to the individuals with Intellectual Disability / Developmental Disability living within the CLS IRA program, participates in the medical on-call system, and provides supervision of unlicensed direct care staff in the performance of nursing tasks and activities. The RN acts as the primary liaison between the medical service providers and the individual. Responsibilities Ensures medical care for the individuals within a designated cluster. Identifies and develops an individualized plan for nursing services for any individual who requires nursing care, including those who require medication administration for diagnosed medical conditions and updates annually Ensures all medical services including testing are completed at the frequency recommended by the provider. These consults will be tracked by the RN with all recommended testing completed and results obtained. Ensures prompt rescheduling of any cancelled medical appointments. Accompanies individuals on critical medical appointments in order to enhance communication and coordination of needed medical services Makes weekly visits to the residence for individual medical monitoring and review of all medical reports, visit forms, diagnostic tests and laboratory results. Completes a monthly site visit; including medication oversight and reconciliation. Completes nursing progress notes monthly and during an illness until resolved Tracks medical appointments using the agency tracking forms for each site in the residential cluster. Tracking forms must be reviewed with AD Nursing during monthly support meetings. Attends monthly support meetings with AD Nursing to review health status of individuals and discuss job related concerns and performance. Provides initial and ongoing supervision/training/support to unlicensed direct care staff on individual's chronic medical conditions and in all nursing tasks and or functions that they perform. The RN will periodically review that the performance of unlicensed staff is consistent with standards of care and training Conducts annual clinical performance evaluations for unlicensed direct care staff for procedures that include but are not limited to medication administration (AMAP pouring). This evaluation will become part of the employee's annual performance evaluation. Ensures delivery of pouring paperwork to the training department. Monitors and visits individuals who are hospitalized for medical or psychiatric reasons at least weekly and more if needed as determined by the RN with AD Nursing input. Coordinates the communication of all information between the hospital and the Habilitation Team. Acts as the Agency's lead contact for all discharge planning Participates in a 24-hour, 7 days a week on-call system that provides direction and support to the Residential Staff including verification of new medication orders. Responsible for delivery of on-call triage paperwork to the medical secretary within one week. Ensures back-up nursing coverage during use of accrued agency time and or for the on call system. Assists program with nursing coverage for any coworker's leave as needed. Responsible for providing supervisor with a daily nursing report regarding any change in individual's health status including new diagnoses, hospitalizations, ER and urgent care visits. Ensures pharmacy reviews are completed every 6 months for IRAs and quarterly for ICFs, and ensures follow up of recommendations of pharmacy reviews. Leads team meetings for an individual prior to any scheduled surgery, any procedure requiring anesthesia, or any other significant medical concern (end-stage cancer, renal failure, diabetes, etc.) Ensures Compass and State audits are prioritized, and changes schedule if necessary to attend all. Contacts family members to secure consent for medical procedures, pre-sedation, pre-medication and prior to the initiation of psychotropic medication and annually thereafter. Coordinates and presents medical information necessary for Surrogate Decision Making Committee review In the event the RN is unable to perform any part of her job responsibility on a weekly basis, she/he is required to notify their supervisor immediately. Requests time off as per the agency handbook. In the event of sickness and or schedule changes, supervisor must be notified, and a schedule change called in/e-mailed to the medical secretary. Is able to drive, as is necessary to perform his/her residential nurse job responsibilities by driving from one site to another and or to travel for in person meetings during the course of the work day. Performs other duties as requested by supervisor. This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check. Ensures that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee. Reports to Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect or a potentially hazardous work environment. Additionally, follows all agency incident policies and procedures. Adheres to all regulations, agency policy and procedures, the employee handbook, and agency program training requirements. Arrives to work by start of scheduled shift, adheres to agency policy and procedure in regard to call-offs and overtime to ensure quality support is provided to people each day. Qualifications NY State licensed Registered Nurse required Current CPR certification required. One year experience working with ID/DD population is preferred. Ability to drive is necessary to perform his/her/their job responsibilities. Excellent oral/written communication and organizational skills.

PHC is unique in its quality of medical, dental, mental health, rehabilitation and specialty care provided to individuals with disabilities and their families. Key/Essential Functions & Responsibilities: Report to the nurse manager or nurse supervisor, at Premier HealthCare (PHC). Meet on a weekly basis for supervision to review duties and responsibilities. Accountable for the day-to-day operations of the PHC nursing station, ensure all workflows and polices are implemented and followed as directed. Assume duties of the nurse supervisor when needed and provide coverage for and assistance with PCMH and other grants. Supervise nursing staff; consist of Medical Assistant/s and medical secretary if applicable to site. Is courteous and uses proper PHC greeting according to policy script 100% of the time. Ensure that each provider on site have an assigned LPN or Medical assistant to assist each provider with daily workflow. It is understood that you will document all activities in the electronic medical record system once you are trained and the system is operational. Check that assigned PCP is correct in EHR, update as needed. 100% accuracy in obtaining vital signs, height, weight and BMI as assigned and document all information in the patient's medical record. 100% accuracy in documentation of all pertinent information in the patients Electronic Health Records, including reason for visit, medical history, medication updates, allergies, all screenings and PCMH related information, etc. Complete the PCMH Tab in EHR with Nursing questions, ensure education sheets are given out to patient in the high-risk groups, reinforce to the patient all goals set by the provider during the visit. Obtain updated medication list on every visit, if any changes to medication noted; notify the provider. If no changes, check off medication reconciliation box in EHR. Collect from patient any forms or documents that needs review and sign off by the provider and inform provider. Prepare medical records for the clinician, include laboratory and diagnostic reports and stamped progress notes as needed. Obtain any pending imaging or testing that has been done after the last visit from patient or caregiver. Perform phlebotomy on adult and pediatric patients and obtain other specimens as per clinicians' order and in compliance with DOH and CLIA protocols 100% of the time. Perform EKGs and other office procedures as per clinician's orders. Complete vaccinations, medication standing orders (MSO), and PPD plantings or other task in scope of practice as ordered by the provider. Meet with patients and complete the required Intake packet/ intake updates and supporting documentation. Enter information in EHR as instructed. Assist with the examination of the patient, as directed by the clinician/nurse. Assist with therapeutic interventions to ensure patient/staff safety based on techniques learned in pre- service, during orientation and ongoing training. Assist with the check-out process as needed: print patient plans, referrals, or prescriptions. Refer patient to front desk to schedule return visit and any internal referrals. Enter prescription requests, form requests, etc., into the designated database and complete associated documentation (e.g., prescription fax forms). Update EHR accordingly. Obtain Prior Authorization for non-covered medications or feeding supplies for patients as needed. Notify patients of any lab results as directed by the provider and document in EHR as indicated. Maintain overall cleanliness and organization of treatment rooms and other areas of the practice site as assigned. Remove filled sharps' disposal units and other biohazardous waste and prepare for removal by the transporter. Replace units as needed. Promote continuity of care and serve as a liaison between PHC and the patient, family, caregiver and other agencies involved in the care of the patient. Participate in the telephone triage process to ensure that requests are appropriately made, prioritized correctly and have the necessary supporting documents prior to the first visit. Document all Hospital/ Emergency Room follow up in the designated template in the EHR. Ensure Hospital discharge summary is obtained prior to each visit. Once summary is obtained, meet with providers to review whether visit was avoidable and document in patients' chart. Follow workflow as directed. Participate in Quality Improvement activities as directed by the Practice Administrator or designee. Resupply and inventory as assigned by the nurse supervisor/manager Comply with OSHA/CLIA regulations in the exercise of all clinical and related functions. Report all safety and health hazards to appropriate personnel in a timely manner. Assist with all emergencies as directed by the charge nurse/ designee or clinician. Comply with NYSDOH regulations and protocols as stated in the PHC Policy and Procedures Manual in the exercise of all clinical and related activities. Ensure refrigerator and freezer temperatures are checked daily. Ensure Daily emergency equipment are checked daily and signed off 100% of the time. Ensure Emergency cart is checked once monthly. Responsible for ensuring crash cart and all emergency medication are up to date. Check vaccines inventory and expiration date weekly or as directed by supervisor. Ensure all vaccines are labeled correctly and all Vaccine information statements are updated. Maintain competencies through on-site training, continuing education programs, and review of reference materials. Endorses and receives information before and after going on lunch or break.
