2. Evaluation for consistency of data among MARS, Physician Medication Order Forms, and labels on medication.
3. Contacting the RN and Day Program Director if medications have expired or need to be refilled.
4. Verification of safe storage of medication
5. Verification of an appropriately stocked First Aid Kit
6. Responsible for documentation or verification of documentation of those records with which the LPN has had contact.
7. Documentation of any nursing care given to a participant.
8. Communicates immediately with Director, RN, PA, CLS, or house staff responsible for the participant any symptoms or behaviors that a participant may be exhibiting and may require medical intervention. Reports any injuries and Identify and communicate any intervention needed for the safety/ well-being of the clients to the Day Program Director, RN to Director, staff, RN, PA, and CLS
9. Train/supervise staff in the area of Medication Administration, tube feedings, and other delegable nursing care after RN approves training program. Documentation of training.

Position Title: Program Specialist/Direct Support Professional
Supervised by: Director of Day Services and Program Administrator
Qualifications: High school degree and one year's related professional experience Possess valid driver's license and good driving record and ability to drive a car/van.
Schedule: Monday through Friday 7:30am - 3:30 pm (times will vary per transportation route)

DUTIES OF POSITION
Individual Program Planning
Participate in preparation of the Person-Centered Plan (PCP) by completing initial and annual assessments, and present recommendations for development of goals to the Program Administrator/Activity Coordinator.
Work in cooperation with the Program Administrator/Activity Coordinator in development, revision, and implementation of each individual's PCP and all PCP recommended activities identified as the day program service's responsibility.
Assist the Program Administrator/Activity Coordinator in program development, using creativity to promote active engagement and independence.
Schedule and implement goals as indicated in PCP.
Follow any specialized program developed by the supportive staff (Plan of Care, Behavior Plan.)
Maintain open communication with families, caregivers, and residential providers.
Individual Related
Supervise individuals at all times keep actively engaged in different tasks/activities to include activities in the community.
Report any concerns to the supervisor immediately (injury, illness, unusual behaviors) and document on the appropriate form.
Provide additional support, when needed in order to maintain integration in the community.
Maintain the individuals' attendance alerting the supervisor of any concerns.
Read the individuals' daily logs and report any concerns to the supervisor; record in the log, as required.
Lift, transfer, toilet, push manual wheelchairs and feed individuals, as needed.
Administer medications to individuals as required.
Transportation Related
Transport individuals to the day program and other locations as directed by the Director of Day Services, Program Administrator, and Activity Coordinator
General
Maintain accountability to the Director of Day Services, Program Administrator and Activity Coordinator.
Acquire and maintain all required training certification including Medication Administration, CPR/First Aid, and Van Driving certification.
Attend all additional meetings, trainings, and in-services, as directed by supervisor, per regulations, and staff interest; adjusting schedule, as required.
Maintain a professional attitude with individuals, coworkers, caregivers, and other professionals.
Maintain a clean safe work environment.
Perform other duties and responsibilities as required within the scope of the position.
