The Physician Assistant provides medical care to all age groups in accordance with the State of Washington Department of Licensing and professional scope of practice. In addition, the Physician Assistant provides diagnostic and therapeutic medical services to CHC patients, while making reasonable and sound medical assessments and treatment plans. Knowledge, Skills & Abilities * Reads, speaks, understands and writes proficiently in English. * Works independently and is self-directed. * Works effectively in a team environment. * Organizes, prioritizes, and coordinates multiple activities and tasks. * Works with initiative, energy and effectiveness in a fast-paced environment. * Remains calm and effective in high pressure and emergency situations. * Problem-solves with creativity and ingenuity. * Knowledge of medical terminology. * Proficiency in the use of Microsoft Office applications; Word, Excel and Outlook. Preferred: * Bilingual skills. Education * High school graduate or equivalent. * Bachelor's degree as a Physician Assistant. Preferred: * Education and training in specialized care such as prenatal care, family practice, pediatrics, obstetrics or prenatal. Experience Preferred: * Not less than a 0.5 Full Time Equivalent (FTE) of patient care within field of training within the past year. * CPT-4 coding experience. * ICD-10 coding experience. * Healthcare information systems, such as electronic health record and practice management systems experience. * Experience in a multiple provider medical practice. * Working with low income, multi-ethnic populations. Credentials * Physician Assistant license, with prescriptive privileges, with the State of Washington. * DEA controlled substances registration. * Physician Assistant certification by the National Commission on Certification of Physician Assistants (NCCPA). * Basic Life Support for Healthcare Providers (CPR/AED) certification - must be obtained within 45 days of employment. * Successful completion of CHC's credentialing and privileging review upon hire and re-credentialing as appropriate. Other * Driver's license with the State of Washington. * Motor vehicle insurance liability policy, a certificate of deposit, or a liability bond to the required limits. Job Specific Functions/Performance: * Provides medical care to all age groups in accordance with the State of Washington Department of Licensing and professional scope of practice. * Provides diagnostic and therapeutic medical services to CHC patients. Makes reasonable and sound medical assessments and treatment plans. * Obtains complete medical history and physical data from patients. * Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed. * Consults with physicians when indicated in managing acute and chronic medical needs of the patients. * Orders appropriate laboratory and diagnostic procedures. * Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention. * Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs. * Instructs patient and family regarding medications and treatment instructions. Promotes medical care through direct patient care and education. * Identifies special needs of CHC patients and works with staff to address those needs. * Participates in Quality Improvement and Utilization Management programs and activities. * Documents encounters in an accurate and timely manner. * Provides therapeutic intervention following accepted guidelines or protocols. * Coordinates delivery of care with appropriate clinical staff to ensure quality of care is met. * Based on operational needs, position may or may not have responsibilities for managing a patient panel. * Based on operational needs, position may have defined portion of FTE dedicated to support inbox management for other providers. * Adheres to attendance standards in order to perform the job functions for daily operations and/or continuity of patient care. The Department of Medicine at the VA Northeast Ohio Healthcare System is seeking a full-time Physician Assistant (PA) interested in an Acute Medicine position working in our Community Living Center (CLC) Admiral Point neighborhood/ unit. The CLC is a 174-bed post-acute care facility that provides Veterans with skilled nursing care, skilled physical and/ or occupational rehabilitation, and dementia care. The Admiral's Point unit is a 14-bed unit that provides such care for residents with advanced respiratory needs (chronic respiratory failure - excluding mechanical ventilation, tracheostomies, laryngectomies, advanced heart failure, pulmonary hypertension, etc.). Primary responsibilities include comprehensive medical care of residents as part of their interdisciplinary team, including but not limited to the following: * Provide clinical care management to CLC residents through weekly visits/ encounters: perform resident interviews and physical examination, obtain appropriate workup and expert consultations as needed with medical and/ or surgical subspecialists, coordinate care with other specialties and disciplines to elaborate comprehensive disease and medication management care plans, complete history and physical as well as progress notes. * Provide episodic acute care as dictated by the needs of the Veterans: identify changes in condition, complete assessment, initiate treatment, and initiate transfer and transfer documentation when a Veteran moves to a higher level of care, readmission documentation upon their return, and discharge planning and documentation. * Recognize and understand resident medical and psychosocial needs and identify resources to meet those needs and achieve desired outcomes pertinent to their CLC stay and post-CLC disposition, in collaboration with mental health specialists, case management and social work, as well as CLC leadership. * Participate in a cohesive interdisciplinary model of care through collaboration with other CLC disciplines within defined venues such as weekly interdisciplinary team rounds, discharge meetings, bioethics meetings, mental health meetings, wound care meetings etc. * Educates patients and families on numerous health care topics and issues. * Identify opportunities for improvement and initiate performance improvement processes using interdisciplinary performance approaches to improve care delivery within the CLC. * Participate in performance improvement projects as assigned, as well as cover for care planning meetings. * Assume a leadership role in utilizing problem solving methodologies to resolve system and/or process problems that impact Veteran care, staff performance, and/or systems improvements.
