What You'll Do:
Initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points
Initiating the plan of care and revising as necessary
Providing services and / or treatments requiring substantial and specialized nursing skill
Counseling the client and family members in meeting nursing and related needs
Initiating appropriate preventive and rehabilitative nursing procedures
Preparing clinical progress notes
Provide coordination of services
Make referral to other services as needed
Informing physicians and other team members of changes in clients' needs
Evaluating outcome of care
Supervising LPNs and CNAs
Initiating and updating CNA care plans
Participating in orientation, onboarding, and education programs for new team members
Planning for the clients discharge from services
Adhere to all established Standard of Nursing Practices, my Potential at Home, and NLCS Policies & Procedures
Ensures continuity and quality care of assigned clients
Within the tour of duty accurately assesses clients' physical and psychosocial status and environmental needs of assigned clients. Identifies needs for other services/disciplines for clients and intervenes appropriately to secure these services. Documents admissions and completes ongoing nursing assessments on assigned clients
Collaborates and communicates, with tact and sensitivity, with other team members and departments to coordinate an interdisciplinary approach to client care. Identifies and interprets the psychological reaction of the clients and takes appropriate action within the time frame necessary. Documents nursing interventions and client response to care accurately, timely and completely
Performs other duties as assigned
What We're Looking For:
Degree in Nursing required
Registered Nursing licensed in the State practicing
Minimum of 2 years of experience in a hospital, long-term care facility or other related healthcare facility
Minimum 1 year of home health experience
Must possess strong written and oral communication skills; and ability to maintain confidential information
Must have and maintain a valid driver's license
Must have and maintain current CPR
Must be proficient in Microsoft Office

Carroll Hospital's Adolescent Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) offer structured, short-term treatment for adolescents who need more support than traditional outpatient therapy but do not require 24-hour inpatient care. Through daily group therapy, psychiatric support, and collaboration with schools and families, our programs help teens stabilize, build coping skills, and successfully transition back to home and academic life.
As an Adolescent PHP/IOP Social Worker, you'll be part of a compassionate, multidisciplinary team dedicated to empowering youth and families to achieve emotional wellness. You'll lead therapy groups, conduct psychosocial assessments, and coordinate care that fosters growth, resilience, and long-term success.
Our program is LGBTQ2S-affirming, gender-affirming, trauma-informed, and trauma-responsive, providing inclusive and culturally competent care for all adolescents and families. Clinicians utilize evidence-based treatment modalities, including Dialectical Behavior Therapy (DBT), Cognitive Behavioral Therapy (CBT), and other strength-based interventions.

What You Will Do
Conduct comprehensive psychosocial assessments with adolescents and families to identify emotional, behavioral, and social needs.
Facilitate daily group therapy sessions focused on skill-building, emotional regulation, communication, and healthy coping strategies.
Deliver care that is LGBTQ2S-affirming, gender-affirming, and trauma-informed/trauma-responsive, supporting each adolescent's unique identity and lived experience.
Collaborate with psychiatrists, nurses, and program staff to ensure coordinated, developmentally appropriate care.
Engage parents and caregivers through communication and education, reinforcing treatment goals and supporting continuity of care at home.
Incorporate DBT-informed and other evidence-based approaches to promote emotional regulation and interpersonal effectiveness.
Monitor progress and adjust therapeutic interventions in collaboration with the multidisciplinary team.
Document assessments, group sessions, and progress notes accurately and promptly in accordance with hospital, departmental, and legal standards.
Maintain confidentiality and uphold ethical and professional standards consistent with NASW, ACA, and AAMFT codes of ethics.
Participate in regular supervision, treatment team meetings, and clinical reviews, contributing to quality improvement and program development.
Coordinate discharge and aftercare planning, ensuring smooth transitions to community and school-based supports.
Support program initiatives and quality goals, identifying barriers to care and collaborating on solutions to enhance patient outcomes.
Qualifications
Minimum Requirements
Master's degree in Social Work, Counseling, Marriage & Family Therapy, or a related behavioral health field.
Active Maryland licensure
Fully Licensed: LCSW-C, LCPC, LCMFT strongly preferred
Associate/Provisional: LMSW, LGPC, LGMFT candidates will be considered based on experience
Preferred Experience
1+ year of clinical experience in a hospital, residential, IOP, or PHP setting.
2+ years' experience facilitating group therapy for adolescents.
Experience with systems-involved adolescents and families (e.g., child welfare, juvenile justice, or special education).
Familiarity with DBT and other evidence-based and experiential modalities relevant to youth and family systems.
Strong skills in clinical documentation, assessment, and treatment planning.
Knowledge of community mental health and educational resources for adolescents.

JOB SUMMARY:
Nation's Infusion at Home is seeking a full-time, exempt Pharmacy Manager for our office in Owings Mills. Candidates will need to have a Maryland license and be willing to apply for a DC license if not currently held. The Pharmacy Manager supports the day-to-day operations, ensuring clinical excellence, and driving operational efficiencies across the infusion pharmacy. This role plays a key part in optimizing workflows, implementing Lean Six Sigma principles, enhancing patient outcomes, and supporting business growth. The Manager will also serve as a mentor to pharmacists and technicians, develop clinical programs, engage referral sources, and contribute to strategic initiatives. Schedule will be Monday-Friday, 8:30 AM - 5 PM with a rotating on-call schedule that includes weekends.
REQUIREMENTS:
At least five (5) years of relevant experience, preferably in a home infusion environment
Strong knowledge base in sterile IV admixture procedures, medication therapy management, and home infusion services
Competent in the use of pharmacokinetic principles & formulas and assessing patient laboratory values.
Ability to problem solve and work in a high stress environment
Experience with procurement and inventory management of medications
Ability to lead and supervise staff
Proficient with software including, but limited to, O365 and EMR systems, and in technology solutions for system enhancements
Excellent problem solving, organization, and communication skills
Ability to exercise sound independent judgment
Knowledge of Performance Improvement Principles and Practices
Current Maryland Pharmacist license in good standing and the ability to maintain required licensure; ability to receive DC license promptly after employment begins
Understanding of regulatory and accreditation requirements
Range $140 to $160K depending on experience

About the Role:
The Inpatient RN Case Manager at Sinai Hospital works with the clinical team and medical provider to coordinate and implement safe discharge plans for patients. Their main goals are to improve patient well-being, outcomes, and ensure healthcare services are used efficiently and timely.
This position is scheduled Every Weekend Sat. & Sun. for 12-hour shifts.*
*This position offers a potential of up to $6,000 sign-on bonus*
Key Responsibilities:
Assessment & Planning: Conducts initial and ongoing assessments to determine patient needs for care coordination and discharge, then develops a focused discharge plan, especially for high-risk patients.
Intervention & Collaboration: Works closely with the clinical team and medical providers to put the discharge plan into action.
Continuous Improvement: Stays current with healthcare trends, regulations, and payer requirements related to patient care, discharge planning, and benefits.
Requirements:
Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field
BSN preferred; ADN required
3-5 years related experience; Prior experience with inpatient case management/discharge planning required
Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license
Case Mgmt cert preferred within 3-5 yrs of hire

Primary Duties:
Provides evaluation services including review of pertinent historical information, performance of specific assessments, and determination of recommendations and documentation of results. Explains evaluation findings and treatment plan to patient and family and incorporates patient and family in the goals setting.
Demonstrates proficiency in implementation of treatment protocols based upon evaluation findings, the patient's presentation, and the evolving health care environment. Consults with other healthcare professionals as indicated. Recognizes and documents changes in patient's condition requiring modification of the plan of care and/or the need for medical/rehabilitation consultation.
Completes appropriate discharge planning and follow-up care. Accurately and timely documents and charges for patient care activities considering our evolving health care environment, painting a clear picture of the patient's course of care including focused initial evaluation/plan of care that establishes baseline and goals, daily encounter notes, interim progress summaries, and discharge evaluation.
Demonstrates effective time management and meets productivity expectations. Manage an accurate schedule. Demonstrates a professional commitment to provide frictionless patient access to care through flexibility, adaptability, creativity, and actions/behaviors that display empathy in our patient consumer driven environment.
Takes initiative in pursuing and directing continuing education for professional growth and competency for self and organization. Serves as a clinical instructor to assign students. Assists with orientation and mentoring of inexperienced staff, students, volunteers, and technicians.
Qualifications:
Bachelor's, Master's, or Doctoral degree in Physical Therapy.
Graduate of an accredited school of Physical Therapy.
Maryland, District of Columbia (DC), and/or Virginia licensure required as deemed necessary for your specific location(s).
American Heart Association's BLS (Basic Life Support) for Healthcare Providers CPR (Cardiac Pulmonary Resuscitation) certification.
