Purpose and Impact:
This is complex professional work supervising and monitoring the work of an assigned group (unit) of workers under the employee's direct supervision. The employee in this position is expected to become familiar with agency contract goals and requirements and to ensure staff performance toward those goals. In addition, knowledge of Florida Child Welfare statutes, administrative code, and operating procedures as well as any additional policy provided by the local Lead Agency, Department of Children and Families Regional Quality Assurance staff or Department memo is necessary for the proper performance of this position. The Case Management Supervisor is required to demonstrate advanced skills in communicating with, motivating, training, and evaluating employees, and is expected to meet regularly with each worker under supervision to mentor and monitor performance. Additional responsibilities include the on-boarding of new staff, the coordination and tracking of required training hours for certification, as well as assessing progress of new case managers in preparation for child welfare certification. Supervisors are expected to participate in the recruitment and hiring of workers for their units and may need to create Employee Improvement Plans to initiate proceedings to address deficiencies in staff performance.
Essential Functions:
Develops Performance standards and job duty expectations with workers, reviews standards and plans for continuous improvement.
Communicates worker's compliance with these expectations on a regular basis.
Creates, identifies, and coordinates the activities of newly hired case managers including shadowing experiences, compliance with training requirements, and opportunities for field/classroom training.
Reviews each case in unit caseload at a minimum, on a quarterly basis. This review must include a one-on-one discussion with the assigned case manager, recommendations for case progress, documentation of the review and recommendations in the Florida Safe Families Network (FSFN) online database. Cases identified as high risk will need a more frequent level of oversight.
Monitors and directs the day-to-day work of case managers. This monitoring must include the completion of home visits within required time frames, the completion of required tracking tools, court documentation, staffing packets, family assessments, safety plans, case documentation in FSFN within 48 hours, file maintenance and other required case work in timely manner.
Attends/ensures preparation for all unit case staffings, reviews incoming cases, and determines how best to assign new cases within the unit.
Reviews and assists with complex cases, provides guidance and support.
Reviews work products such as family assessments, home studies, court reports, case plans and case documentation with case managers, and provides consultation and direction to them to ensure appropriateness, clarity, quality, and thoroughness.
Identifies performance needs of case managers and other assigned staff, and develops and implements training plans and/or other opportunities for performance improvement.
Provides guidance to case managers and other staff by coaching, motivating, training, and providing staff development activities.
Identifies and promotes outstanding performance.
Participates in court proceedings as necessary, and monitors worker performance in court.
Mentors all new case managers in the field each month by participating in a home visit with the case manager and providing the case manager with appropriate feedback after that home visit. These mentoring visits are to be documented by the Supervisor in FSFN within required time frames.
Keeps Assistant Program Director and Program Director advised of high risk or problematic cases and arranges for multi-Disciplinary staffings for cases as necessary.
Attends program Supervisor meetings as scheduled and shares information gathered with unit workers and other team members.
Conducts regular unit meetings with staff to share information, provide training, discuss cases and participate in team building activities.
Develops training and staff development plans with each staff person under his/her supervision.
Conducts all required performance evaluations with assigned workers in a timely manner. Submits evaluations to Program Director for discussion and review.
Provides community education through public presentations.
Participates in agency CQI (Continuous Quality Improvement) activities as required.
Other Functions:
All duties are performed in accordance with the following standards:
Courtesy: Treats customers, the public and staff with courtesy, respect and dignity and presents a positive public image.
Communication Skills: Keeps APD and Program Director fully informed of activities, pertinent issues, upcoming events, and potential problems. Demonstrates effective oral and written communication skills in daily work.
Teamwork: Supports the unit, department and/or organization and works with others to accomplish the goals of the unit, department and/or organization.
Safety: Employee makes a reasonable effort to adhere to established safety procedures and practices in the work area and ensures that unit workers do the same.
Training: Attends and successfully completes all mandated training courses, must attend and successfully complete “Supervising for Excellence” training curriculum. Must complete all agency requirements to become certified as a Child Welfare Supervisor within required timeframes.
Confidentiality: Adheres to all confidentiality rules.
On-Call: Supervisors are expected to be available and on call at all times. Supervisor carries an active cellphone during regularly scheduled work hours, as well as after hours and on weekends, in order to immediately respond to all calls.
Must demonstrate sensitivity to our service population's cultural and socioeconomic characteristics and needs.
Other:
Case Manager Supervisors are charged with ensuring that their case managers fully understand case ownership responsibility as the integrator of all services and supports identified for each child, including therapy, other mental health services, health and dentistry, developmental services, educational support, permanency and safety; as well as their responsibility to make trauma sensitive transitions when it is determined that a caretaker lacks the needed level of responsibility to care for their children.
Physical Requirements:
Must have a high level of energy, be adaptable to irregular hours, be flexible to cover on-call as needed, be able to travel as needed.
Education:
Must possess a bachelor's degree in human services field. Must have a Child Protection Certification or be eligible and pass waiver toward certification requirement. Master's Degree is preferred.
Experience:
Must have a minimum of three years of experience as a certified child welfare case manager, preferably in the Florida Dependency system. Prior supervisory experience preferred.

The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency's core values.
Essential Functions:
Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.
Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.
Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.
Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.
Submit service requests to the lead agency Utilization Management Department and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.
Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.
Arrange for, attend, and participate in individual case staffing as necessary. Complete all required staffing and application packets.
Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary.
Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.
Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.
Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective caregivers as needed. Plan and facilitate parental and sibling visits as needed and appropriate.
Transport children as needed.
Ensure that all Independent Living functions are completed as required.
Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.
Function as agency on-call Case Manager as scheduled.
Works cooperatively with Program Directors, with other Case Managers, with placement staff, with Protective Investigators, with Child Welfare Legal staff and with agency support staff.
Effectively manages time to ensure that all home visits are completed as required, all documentation is entered into FSFN within 48 hours, court documentation is prepared according to specified time frames and court appearances are attended as necessary.
Follows Florida Statutes, Administrative Code, written policies and orders of the Dependency Court in managing cases toward goals recorded in case plans.
Organize, prioritize and complete all work assignments by the established deadlines.
All duties are performed in accordance with the following standards:
Courtesy: Treat clients, the public and staff with courtesy, respect and dignity and presents a positive public image.

Purpose & Impact:
Key responsibilities include the direct supervision of youth residents, providing quality care, ensuring a safe and secure environment and transporting and accompany/supervising residents on outside activities. This is a direct service delivery position at a primary location, but the position may work at any one of a number of regional group homes within a reasonable radius as shift needs arise.
These group homes house adolescent teens who have aggressive behaviors. The ideal candidate for this role is someone who would be comfortable working with at risk adolescent males and females. The ideal candidate would also not be intimidated by youth who have been impacted by abuse, neglect, and trauma. Essential Functions:
Provide supportive environment for youth.

Purpose & Impact:
The Early Childhood Service Coordinator will provide case management services for at least 30 open cases at any given time with families who have children ages birth to 6. Maintain case ownership responsibility for coordinating services and supports identified for each child and family member to include health, wellness and/or safety. This individual shall manage the support plan, as required, and continuously monitor and assess the family's condition and dynamics to make necessary assessment and coordinate supports.
Essential Functions:
Responsible for planning early childhood events at each Center to engage families and refer to St. Joseph's Hospital for developmental screenings.
Conduct home visits to meet the needs of the family and ensure successful enrollment in key services.
Work with parents/caregivers to develop individualized family support plan.
Provide families with timely referrals to services.
Partner with families to build strong parent-child relationships and strengthen their support networks with other parents and community members.
Maintain regular contact with service providers and document service progress.
Provides community education through public presentations.
Organize, prioritize and complete all work assignments by the established deadlines.
Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources.
Work collaboratively with volunteers and community resources.
Knowledgeable of Hillsborough County child serving systems and community resources.
Knowledge of Children's Board policies.
Accurately document all case activities and maintain the case file in accordance with the standardized case file format.
Develop and maintain relationships and partnerships with local programs and offer parents/caregivers information and materials regarding community programs.
Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by supervisor.
Organize, prioritize and complete all work assignments by the established deadlines.
Work flexible hours to include some evenings and Saturdays.

Must demonstrate sensitivity to our service population's cultural and socioeconomic characteristics and needs.
Understand the value and foundation of the Protective Factors framework.
Practice Family Support Principles.
Cross trained to perform daily operating procedures.

Must be trained in the Center data system: Attendance and Needs Tracking System and maintain member confidentiality.

Attend mandatory trainings and perform other duties as assigned.
Physical Requirements:
Must have a high level of energy, adaptability to irregular hours, able to travel within Hillsborough County as needed.
Education:
Bachelor's Degree in Social Work, Early Childhood Education or related field required.
Experience:
Must have experience in primary responsibility to provide case management for at least 30 open cases at any given time with families who children ages birth to 6.
Bilingual English/Spanish required.
Skills:
The coordinator will possess a strong knowledge base and establish relationships with community resources to successfully link families with needs identified in their family support plan.

Responsibilities
Work with home health patients in the community setting

Typical caseload includes approximately 5 one-on-one visits per 8-hour shift
Serve a diverse adult and geriatric population with neurological, cardiopulmonary, orthopedic, and general deconditioning diagnoses
Clinicians use laptop and phone with cellular data for documentation and communication
Evaluation, treatment, and documentation are performed independently in patient homes
Support provided by a fellow OT, rehab supervisor, home health manager, and scheduler
