PROGRAM DESCRIPTION:
The Philadelphia Model: Addressing serious mental illness (SMI) in long-term care (LTC) will provide highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care who also have serious mental illness and who have reached maximum benefit from the mental health resources available elsewhere in the community or hospital. The program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI that may include other complex behavioral health needs and/or prior criminal history, and need long-term skilled nursing services- can receive the care and services needed to address their holistic needs free of stigma. A multidisciplinary team will provide both long-term care and behavioral health services in this integrated program.

GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES:
Under the direction and supervision of the Program Director and the Compliance Regulatory Nurse, in collaboration with the Director of Nursing, the Registered Nurse will demonstrate a commitment in the assessment and management of residents with psychiatric, mental health and/or substance abuse diagnosis. The Registered Nurse assesses, plans, implements, and evaluates behavioral health nursing care provided to residents and their families within the context of the program's philosophy and goals. Will be expected to apply all nursing service policies when rendering nursing care and providing health education to patients and families.
Recognizing that nursing care is one portion of health care, the Registered Nurse is expected to function as a member of a multidisciplinary team, interacting collegially with staff, physicians, consultants, and interdepartmental personnel.
ESSENTIAL & CORE FUNCTIONS:
•Practices psychiatric-mental health nursing in accordance with the philosophies, goals, and objectives of both Behavioral Medicine and the assigned unit.
•Provides quality behavioral health nursing care to all residents as defined by ANA Standards of Psychiatric Mental Health Clinical Nursing Practice, ANA Standards of Professional Performance, and long-term care regulations. Models ANA Standards.
•Intervene with residents by providing health promotion and health maintenance, intake screening and evaluation, milieu therapy, therapeutic use of self, self-care activities and bio-psychosocial interventions, health education, crisis intervention, individual and group counseling, and resident advocacy.
•Contribute to the development and evaluation of a goal-oriented care plan continuously based on nursing assessments and nursing interventions with residents and resident representatives.
•Completes daily chart reviews.
•Reviews Care Plan and updates information as needed.
•Implements and delegates behavioral nursing interventions that are consistent with the care plan and the medical regime.
•Implements Nursing policies and procedures.
•Supervises and directs unit staff, designating and reviewing assignments of Behavioral Health Techs, on appropriate assignment sheet.

EDUCATION & EXPERIENCE:

About the Role
The Director of Development (DoD) is a key member of the leadership team, responsible for fundraising strategy, donor relations, sponsorship development, and campaign execution. The ideal candidate is proactive, creative, and relationship-driven, with a strong record of fundraising success, exceptional attention to detail, and a deep understanding of Chicago's philanthropic and golf communities. Reporting to the VP of Development & Operations, the DoD works closely with leadership, the Board of Governors, the Leadership Council and the Ambassadors Council to expand the organization's reach and impact. This role offers a unique opportunity to make a lasting impact while helping shape the next phase of First Tee - Greater Chicago's growth, including potential new facility development on Chicago's West Side.
Job Responsibilities
Develop and execute a comprehensive fundraising strategy, including major gifts, sponsorships, giving societies, and targeted campaigns.
Co-manage a donor portfolio of ~1,300, focusing on top donors and sponsors, while cultivating new relationships.
Develop and oversee foundation and/or corporate proposals, subsequent reporting and donor communications.
Plan and execute end-of-year giving mailer, including the storytelling, design, and overall execution.
Campaigns & Events
Co-plan and execute signature fundraising events such as the Corporate Challenge Golf Outing, Fore the Kids, Masters Watch Party, and other opportunistic events.
Lead digital and peer-to-peer campaigns via GiveSmart and Harness.
Communications & Strategy
Partner with the Marketing & Communications Manager to align messaging and fundraising materials.
Partner with the Program team and coaches to gather participant stories and data to demonstrate program impact.
Partner with the leadership team to ensure accurate reporting and use of financial statements, revenue tracking, and expense management.
Navigate stock gifts, ACH transfers, wire payments, and other money movement processes.
Actively participate in overall budget planning.
Engage and inspire the Board of Governors and the Development Committee, along with other fundraising task forces.
Represent First Tee - Greater Chicago at community, corporate, and donor events.
At a leadership level, partner with key stakeholders to enhance and elevate First Tee - Greater Chicago's impact. i.e. First Tee HQ, Western Golf Association/Evans Scholars Foundation, Chicago Parks Foundation, etc.
Database & Reporting
Maintain accurate donor records in Eleo, ensuring timely reporting and segmentation.
Analyze donor trends and campaign results to guide strategy.
Qualifications
Bachelor's degree required; advanced degree or CFRE preferred.
5+ years of nonprofit development experience, with success in major gifts, sponsorships, and donor engagement.
Knowledge of Chicago philanthropic networks and corporate community.
Proficiency with Eleo, Harness, GiveSmart, and Microsoft Office Suite (Word, Excel, PowerPoint).
Strong communication, storytelling, and organizational skills.
Passion for youth development and First Tee - Greater Chicago's mission.
Understanding of golf culture and the opportunities it presents is beneficial.
Primarily office-based, 2-3 days per week (typically Monday, Wednesday, occasional Friday).
Other workdays are flexible to a suitable space with tech/tools, internet, etc.
Secondary site visits, 3-5 days per month.
Collaborate with Program/Coach team.
Observe programming.

What you'll do:
As an Optometrist, you will provide exceptional care by conducting comprehensive eye examinations, which include refraction and fundus photography. You will use cutting-edge technology such as Virtual Visual Field, Visionix VX110 auto-refraction and keratomery, Visionix Eye Refract system, Topcon Maestro2, and Visionix VX40 autolensometer. You will also have a fully electronic EHR and Practice Management Systems using iPads within the practice.
