SECTION 1: Job Summary
An Optical Technician will achieve a world class Total Patient Experience that both maximizes retail sales while also supporting the patients' medical needs. This team member is trained to dispense and fit glasses and contacts, working from the prescriptions written by Optometrists and Ophthalmologists. POSITION OVERVIEW: Program Navigator:
The Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support, and services to current and prospective patients/clients of Spectrum Medical who wish to access PrEP to prevent HIV infection. Program Navigators are expected to:
Complete screenings to identify readiness for PrEP Program services and other barriers to care.
Assess benefits and insurance, and if needed, provide assistance to access payor programs.
Provide Program-specific education and counseling, as applicable.
Work collaboratively with community partners to provide testing and linkage to care services in the community/offsite.
Provide ongoing follow up to ensure medical and/or Program adherence.
Maintain appropriate patient files and timely documentation of services utilizing the designated electronic health record (EHR) software.
Establish working relationships with appropriate community resources in the area
who seek to send patients to Spectrum for PrEP
Assist the clinical team with individualized risk-reduction counseling and support, as well as adherence strategies for targeted outreach interventions to optimize follow-up.
