A Brief Overview
Assists in coordinating, planning, implementing and facilitating programs and counseling services on an outpatient basis. The Outpatient Therapist's responsibilities may include several of the following areas: individual, group and family counseling, crisis services/evaluation, tele-therapy services, and current and future groups/programs. All of these areas may involve providing assessments, facilitating treatment and crisis intervention of clientele affected by mental health or dual diagnoses issues. Providing community/education services will be a part of this position and some travel is occasionally required.
What you will do
Coordinates a clinical treatment program for the diagnosis, treatment or referral of clients/patients presenting moderate to severe social adjustment problems.
Adheres to policies which assure the confidentiality of client communications. Provides for a multi-disciplinary, holistic approach to the treatment of clients, coordinating same with appropriate hospital departments.
Conducts individual, family, or group therapy sessions in accordance with accepted principles of the National Association of Social Workers.
Assesses progress of clients/patients and modify or extend treatment program accordingly. Coordinates care of clients/patients with referring physician or agency.
Documents therapy in accordance with hospital policies and procedures on medical records.
Intervenes as a crisis counselor in situations requiring immediate psychological treatment such as substance abuse, suicide attempts, psychotic episodes and other instances as outlined in hospital policy.
Refers client or patient to outside agencies where appropriate.
Communicates with family members/significant others in order to further the effectiveness of client/patient treatment and progress.
Coordinates client/patient billing and modify treatment program in accordance with insurance coverage, plan provisions, and individual ability to pay.
Provides educational programs on a variety of psychological topics to hospital/community groups as requested.

ESSENTIAL FUNCTIONS:

A Brief Overview
Provides evaluation and treatment for individuals of all ages who are suffering from an acute mental health crisis through telephonic, face to face, or interactive video communication. Ability to work in an intense, stressful work environment where you must perform rapid assessments, make swift treatment decisions and have the ability to multi task. Applies specialized clinical knowledge and advanced clinical skills in the areas of assessment, diagnosis, and crisis intervention of mental, emotional and behavioral disorders, conditions and addictions, including severe mental illness in adults and seniors, severe emotional disturbances in children and adolescents, and acutely intoxicated or withdrawing individuals. Helps facilitate referral or placement to appropriate level of care or other resource.
What you will do
Answers all inquiry calls in a timely fashion, offer and schedule assessments, complete the database and keep all information up to date. As the point of contact for all of BHC services, provides excellent customer service to all callers.
Completes the psychosocial assessment including suicide/homicide risk assessment, determine appropriate level of care and make appropriate referrals.
Manages the Behavioral Health Center emergency room by visiting with walk in patients, providing a medical screen, meeting all the EMTALA guidelines.
Assists the emergency room physicians, including E- assessment through the E-emergency program.
Works with law enforcement to petition or place Board of Mental Illness holds when needed, and with security regarding transports and Crisis Prevention Intervention when needed.
Provides crisis intervention and case management for local mental health clinics, medical clinics and psychiatry clinics. Provide EAP assessment and counseling including phone intervention. Manages the psychiatric inpatient beds through a system wide tracking board, manage the physician rotation assignments and on call schedules. Maintains current knowledge of admitting modules and the ability to register Clinical ARC patients and Pre-admitted patients, in a timely fashion.
Provides ongoing training/orientation for all students including psychiatric residents, nurses, counseling interns and paramedics. Provides mental health support services in the community and promote mental health services available through Avera Behavioral Health.
Obtains and completes all insurance pre-certifications and authorizations as determine necessity for pre-certification; coordinates with insurance companies, patients and clinical staff to obtain information; and maintains complete and accurate documentation. Must have proficient knowledge of insurance requirements, medical/clinical terminology, psychotropic medications, and medical necessity associated with certification.
Must maintain an updated working knowledge of the following: SD, Minnesota and Tribal Mental Illness Holds; Duty to Warn and Abuse reporting standards; Informed consent and competence to make treatment decisions; referral sources in a 4 state region; level of care criteria and pre-authorization requirements and processes; ASAM criteria; DSM criteria; ICD9 codes along with de-certification process and appropriate admission service types.

We are seeking a Licensed Professional Counselor- Mental Health (LPC-MH) to join our integrated clinical team. In this role, you will work onsite in one or more long-term care facilities within your local community, providing therapy, assessment, care planning, and behavioral health support. What You'll Do
As a key member of the interdisciplinary team, you will:
Clinical Care
Provide individual, family, and group therapy to LTC residents.
Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in LTC settings.
Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care.
Interdisciplinary Collaboration
Work closely with nursing, social services, rehabilitation, and medical providers to coordinate treatment and improve resident outcomes.
Participate in care plan discussions, facility meetings, and case collaborations as needed.
Documentation & Compliance
Maintain complete and timely documentation using our Geriatric Notes Tool (GNT).
Follow ethical, professional, and regulatory guidelines for clinical documentation, confidentiality, and care delivery.
Facility Partnership
Build strong, professional relationships with facility staff and leadership.
Serve as a behavioral health resource and trusted partner in supporting residents' mental and emotional needs.

**Job Summary**

Summary
The Integrated Health Therapist (IHT) serves patients as an integral member of a Patient-Centered Medical Home (PCMH) or Primary Care Clinic (PCC) and is dedicated to helping patients get well and stay well within an inter-professional environment. The IHT is an important resource for patients and team members for issues related to mental and behavioral health, chemical health, and psychosocial aspects of health and disease, and lifestyle management to support optimal patient functioning.
Job Description
The IHT has a solid understanding of human behavior; will be familiar with legal issues, ethical issues, insurance regulations and protocols for proper referral or application for various programs related to behavioral health. The incumbent must also be knowledgeable in medical care and emergency services and typical operations of ambulatory care centers. Must be comfortable providing coverage across all potential age populations and patients with co-morbid medical conditions. Commitment to an inter-professional environment and recognition of a bio-psychosocial conceptualization of patient health concerns is necessary. Must also possess skills that promote excellence in communication, flexibility, team-based care and decision making.
The IHT conducts behavioral health services in a primary care clinic setting by providing diagnostic assessment, onsite crisis assessment and intervention, determining need for ongoing care according to level of clinical ability, medical and psychosocial complexity. The IHT is competent in multiple counseling modalities (ranging from very brief to long-term), making appropriate referrals, and providing educational services across the continuum of care. The IHT follows up with referrals to specialty care and ensures those patients are receiving appropriate behavioral health management. Serves as the primary team consultant to promote understanding of the relationship between health and psychological/behavioral factors and promote this understanding to patients, families, other team members and the community. Takes active leadership in universal health screening for behavioral and chemical health conditions that negatively impact overall health and wellness and works collaboratively with other team members to ensure quality delivery of care. Provides education and training to staff regarding management of behavioral emergency situations and current behavioral health information and how this interacts with overall health. Delivers empirically validated, short-term behavioral health care services (assessment, consultation, diagnosis, and treatment) to ambulatory primary care patients via both traditional face-to-face visits and innovative care delivery models such as telehealth when necessary. Works closely with other PCMH or PCC team members to address psychosocial and cultural elements of health and disease pertinent to the patient's ability to manage their acute and chronic conditions and appreciate other factors that influence health care (developmental, economic, familial, religious, etc.) and wellness. Assists the team in effectively managing populations of patients, tracking costs and quality associated with innovative integrated care models, and implementing the highest standards of behavioral health care. Systemically evaluates the quality and effectiveness of behavioral health care and actively participates in quality improvement activities. Documents in a responsible, accountable and ethical manner to promote safe, reliable patient centered care. Participates in a population health management approach to health care by thinking broadly and innovatively and leveraging multiple resources to assist with direct care of patients with both medical and co-morbid behavioral conditions.
Credentialing through the Allied Health Staff of the organization where you will be working will be required.
Qualifications
