Job Description

The primary purpose of the Trauma Patient Navigator is to serve as a role model and community liaison to individuals who are enrolledin the Jersey City Medical Center Hospital Violence Intervention Program (HVIP). Trauma Patient Navigators are expected to developrelationships based on trust with patients and families, provide reliable information, appropriate community resource referrals, andemotional support to victims of violence who are being treated in the hospital and have agreed to participate in the program. Thisindividual will function as a core member of the trauma patient care team.

Additional Requirements

Bachelor's degree required, BA in counseling, social work, psychology, forensics or related field preferred. NJ Driver's license required. Spanish speaking required. Successful completion of training including hospital-wide orientation, trauma informed care training, orientation to the trauma service,and shadowing of community outreach organizations required. Previous mentoring experience is desired. Participation in community organizations and firsthand understanding of issues related to interpersonal violence preferred. Sensitivity to a substance abuse environment preferred. Must have a basic understanding of computer use and software applications, and the ability to perform basic internet searches. Must have demonstrated interest in serving the Jersey City community. Must be able to work independently and seek guidance when necessary. Must have good listening skills. Must have a demonstrated ability to work in a team environment and have the ability to work with individuals of a diverse background. Must have own transportation to accompany patients to appointments and connect with community resources, as needed.

The Patient Navigator is a patient advocate and will guide patients through the health care system, ensuring appropriate appointments are given in a timely manner through guidelines agreed upon with the Medical Director of the Practice/Department. The Patient Navigator will assist with access issues, develop relationships with referring health care providers and ensure the complete and efficient management of all referrals, including the acquisition of necessary patient-care materials and medical records. In addition, the Patient Navigator will track all new patient requests, provide monthly statistics and referring patterns. The Patient Navigator works closely with the patient care team and assists in the coordination of patient care by removing barriers that would otherwise prevent patients from receiving health care services.

Bachelor’s Degree in Social Sciences or a related field required. Bachelor’s Degree in Social Work preferred. BLS/CPR certification by the American Heart Association is required. Two (2) years of relevant work experience in a healthcare environment dealing directly with patients required, including in Behavioral Health discharge planning and in Outpatient Behavioral Health Services. Sound knowledge of the healthcare system and where to find reliable information. Candidates must be experienced and competent in a patient advocacy and/or customer service role. Some experience in ana outpatient setting preferred. Familiarity with medical terminology required. Excellent verbal and written communication skills. Effective organizational, planning, negotiating, coaching, and conceptual thinking skills. Bilingual skills (English/Spanish) a plus. Years of experience and/or navigator training may be substituted for the degree requirement on a year-for-year basis.

Responsibilities

The Family Navigator (FN) is a Social Worker with comprehensive knowledge of trauma treatment modalities, crisis intervention, and case management. The FN demonstrates critical thinking and utilizes social work ethics to assess and meet the needs of patients and their families/caregivers by providing care coordination throughout the care continuum.

FN assists in improving the clinic care experience and preparedness for treatment by orienting and educating patients, families, and caregivers to the healthcare system and multidisciplinary team member roles and resources. FN will assess and address patients' barriers to care, partner with patients/families to address their concerns, issues and needs, and facilitate appropriate referrals. FN will serve as the central point of contact to the patient, facilitating collaboration among all team members and ensuring an individualized approach throughout the length of the patient's needs. This includes clinical service directly after the initial intake call, during the forensic evaluation and post-evaluation, providing support and engagement with outside resources. FN will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse patient needs , and demonstrate ability to prioritize utilizing organizational skills and clinical judgement. FN will promote a consistent, holistic plan of care to enhance the quality of life of patients, families and caregivers through assessment, teaching, communication, support, and the practice of family-centered care. FN will work to influence both positive patient outcomes and system outcomes through improved interdisciplinary communication, patient retention, and community partner collaboration.



Qualifications

Education:

Master's degree in mental health field, including Social Work, Psychology, Counseling, or Marriage and Family Counseling.

Experience: Minimum one-year post-MSW experience in one or more of the following areas required: Healthcare, family service, child maltreatment, case management, or coordination of home-based services. Bilingual in English and Spanish is strongly preferred.

Specific knowledge and experience working with African American communities highly preferred. Demonstrates an understanding of their social networks, community assets, and traditions. Understands the historically rooted challenges and distrust these communities may have interfacing with social welfare and health care systems; systemic advantage and disadvantage; generational racial trauma, and other factors related to the non-dominant culture affecting mental health and treatment.

Skill Requirements:

Broad knowledge of the principles of Social Work and the ability to apply them.

Demonstrates strong organizational skills, including an ability to effectively triage patient and family needs.

Practices with strengths-based practice focus.

Engages in culturally competent practice.

Demonstrates excellent verbal and written communication skills for effective and appropriate interactions with patients, families, staff, and community partners.

Effectively engages in discharge/transition planning.
