Tracks Medicare resident benefits days, validates daily Medicare census and coordinates this information with accounting representative and therapy representative, to ensure accurate billing (if applicable)
Assures all quarterly resident assessments are completed
Assures all care plans completed and updated as needed
Coordinates daily PPS meetings and weekly Medicare meetings
Reviews nursing documentation of residents and provides guidance/training as needed, to ensure charting is appropriate and meets coverage guidelines
Responsible for completing case management to ensure adequate documentation for reimbursement
Coordinates discharge of residents
Works with Clinical Liaison to verify appropriateness of admission and admission diagnosis codes
Maintains weekly communication with insurance case manager to receive approval for resident treatment and communicate with physician on insurance resident admission for timeliness of physician visits
Communicates with rehab department on routine basis, to discuss therapy services
Communicates with Life Enrichment and Social Services to coordinate assessment in a timely manner
Recruit, hire, train, coach, and evaluate departmental staff (management staff only)
Monitor residents for changes in condition, ensure MD, and family, are notified and appropriate documentation is reflected in the medical record
Assess residents initially and annually and complete resident service plans on an annual or as needed, basis
Maintains the Residents Roster and Acuity Levels within the department (i.e., State Boards, QA monitor)
Review and follow-up all consultant reports in a timely manner
Reviews monthly physicians orders and ensures accuracy
Monitors psychotropic behavior sheets and makes recommendations as needed
Complete performance evaluations of all licensed nurses, mediation aides and CNAs

\tIs responsible for health care management of the campus to include, independent and assisted living, health unit and special care.
\tMakes rounds in all buildings, ensures staffing is appropriate and residents are taken care of.
\tEnsure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
\tMaintain good attendance record
\tRecruit, hire, train, coach, and evaluate departmental staff (management staff only)
\tAssessment of high risk resident for pressure sores, restraints, weight changes, and other preventative care as outlined in the resident plan of care
\tAssess residents and completes MDS according to facility policy\tMonitor residents for changes in condition, ensure MD, family, and Director of Nursing are notified and appropriate documentation is reflected in the medical record
\tAssess and make recommendations for significant change in conditions for MDS TILE and resident plan of care
\tMaintains the Resident's Roster and Acuity Levels within the department (i.e. State Boards, QA monitors)
\tReview and follow-up all consultant reports in a timely manner
\tComplete evaluations of all licensed nurses and assist with CNA evaluations on assigned unit
\tNotify Director of Nursing and Director of Healthcare of incident reports concerning abuse
\tThoroughly investigate any complaints and concerns and report to Director of Nursing
\tAttend all assigned committee meetings, according to Director of Nursing directives
\tReview incident report documentation and ensures follow-up according to facility policy
\tMake rounds to verify clinical compliance and quality of care and provide couching and disciplinary action as indicated within the facilities policies and procedures
\tEnsures all life enrichment programs/QA monitors are implemented on assigned unit
\tPerform charge nurse duties, in cases of staffing emergencies, as required to meet the resident's immediate needs
\tOther duties as assigned MINIMUM QUALIFICATIONS:\tValid Texas RN license
\tDemonstrated supervisory and managerial skills
\tKnowledge of current professional nursing theory and practice

ESSENTIAL DUTIES:

Ensure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
Maintain good attendance record
Makes beds, straightens rooms and keeps resident care areas in an organized and clean manner, ensuring the entire nursing floor is kept orderly
Respects resident's rights by: (knocks on doors before entering room, addresses resident by name, allows resident to make own decisions when appropriate, promotes independence)
Provides safe resident transfers involving bed, wheelchair and /or shower chair, uses the gait belt with all transfers, to protect resident from complications and injuries
Takes and records temperature, pulse, blood pressure and respirations as required
Assists residents with simple range of motion or other exercises as directed by nurse or therapist and ambulates resident safely according to his/her endurance levels
Applies medical or adaptive devices as ordered by the appropriate supervising nurse
Makes rounds to ensure resident's safety, comfort, medical attention every hour or as required by the resident
Collects specimens as directed
Completes weights for each resident as required
Sets up meal tray and assists with feeding as necessary. Assesses for choking and responds appropriately
Ensures residents and equipment are clean after meals, including hands, and removes clothing protectors
Accurately documents care given to residents including the documentation of reportable resident problems (i.e. ADL sheets, B&B sheets, meal percentages, I&O, s/s of skin breakdowns, etc.)
Reports changes in resident condition to the supervising nurse and /or Director immediately (i.e. changes in skin conditions, urinary changes, etc.)
Reports problems with completion of assignments
Handles confidential information appropriately
Demonstrates flexibility with changing work demands
Reports to charge nurse: equipment and supply needs, repair/maintenance needs, safety hazards and unusual occurrences or observations
Attends and participates in appropriate in-service and staff meetings to improve knowledge and skills and to maintain competency and certification
Wears prescribed uniforms, follows prescribed dress code, wears name tag appropriately and maintains a professional image to the public and residents
Assists the home department and other departments when required in other duties and tasks when asked
Provides appropriate interventions for resident behavior per care plan, utilizes distraction and validation techniques as appropriate and encourages resident independence
Assists residents to and from activities of choice and encourages the resident to participate in activities of choice
Assists activity director as needed with activities and if necessary leads activity
Other duties as assigned
MINIMUM QUALIFICATIONS:
Education: High school graduate or GED
Possession of a valid Texas Certified Nurse Assistant certification
Ability to read, write and effectively communicate in English
Six months experience in providing personal care in a long-term care facility preferred
Knowledge/ability to learn computer skills
Ability to work independently
Ability to adjust to changing conditions, frequent demands and stressful situations
Ability to acquire new knowledge or perform additional or more difficult tasks
Ability to maintain a positive, stable disposition in performing, working in association with staff and residents
Ability to maintain a high level of performance in an environment of frequent demands and changes
