Description of Work The Clinical Social Worker is responsible for facilitating therapeutic group sessions within the Hope and Wellness Center treatment mall program. The Clinical Social Worker develops and implements group-based interventions utilizing evidence-based modalities and techniques. This position must exercise clinical judgement to continually assess the needs of group participants, monitor group dynamics and adapt interventions as needed to target various treatment goals. This position monitors and documents on participants' responses, performance, and progress towards goals outlined in the comprehensive treatment plan. This position may also serve on a variety of Hope and Wellness Center and/or hospital wide committees and workgroups.     I.A.Primary Purpose of Organizational Unit This social worker position is assigned to the Adult Service Unit, which is part of the Service Division of a local department of social services which provides crisis assistance and on-going supportive services to eligible individuals and their families. B.Primary Purpose The social worker in this position has the responsibility of assisting aged, disabled, and other adults experiencing medical, social, and community problems to identify their problems. The worker strives to enhance the client's problem-solving and coping capacities with the purpose of providing services to enable the individual to live independently in his or her community. C.Work Schedule The normal work hours are from 8:00am until 5:00pm. The worker may have to work overtime on some occasions for which he/she will receive compensatory time. D.Change in Responsibilities or Organizational Relationship This position now has the added responsibility to investigate Adult Protective Service reports, assess competency of adults and make recommendations regarding the need for a guardian. Protective Services for Adults is a multi-factored service based on an awareness of the vulnerability of some adults to abuse, neglect, and exploitation. Abuse means willful infliction of physical pain, injury, or mental anguish or unreasonable confinement, or the willful deprivation by a caretaker of services which are necessary to maintain mental and physical health. Neglect refers to a disabled adult who is either living alone or not able to provide for himself or herself the services which are necessary to maintain mental or physical health or is not receiving services from his caretaker. Exploitation means the illegal or improper use of a disabled adult or his resources for another's profit or advantage. The protective service component consists of provisions in the state law for intervention in crisis situations in which adults are reported to be in need of protection. It is the role of the social worker to ensure that the agency's actions are consistent with statutory and state requirements. Adults who are in need of this protection are persons 18 years of age or older with disabilities or handicaps such as age, physical, mental or socioeconomic, which have incapacitated them to such a degree that they are unable to take care of themselves and who have no one able or willing to assist them with the care of their needs. Guardianship involves the provision of services to individuals who are alleged to be incompetent and their families. It includes legal proceedings in which an adult is declared incompetent by the court and another party is given the responsibility for duties relative to the adult's personal affairs and/or property. The nature and scope of a guardian's responsibility are determined by the court, based on applicable provisions of the law and the circumstances of the individual case. Guardianship ends with the death of the incompetent adult or with a judicial restoration of the adult's competency. The legal determination that an adult is incompetent means that the adult is considered incapable of making important decisions concerning his/her personal welfare and/or financial resources. The authority in this position is given to a guardian to make decisions such as where the adult will live, including the geographical area and type of living arrangement; how the adult's income will be spent; and whether the adult will have recommended medical treatment or surgery. To the extent that the guardian is given authority over these and other areas of the adult's life, the adult no longer has the right to make decisions in those areas. Because of the effect of declaring an adult legally incompetent and giving his/her authority to make decisions to guardian, guardianship should be considered only when other less drastic methods of assisting adults are not sufficient. Description of Responsibilities and Duties (written in order of importance) * denotes essential function. This worker will have the case management responsibility to provide supportive services to individuals and families residing in the county. The social worker receives case assignments through intake as assigned by supervisor and from applications taken in the field, or as referrals from current service recipients. Development/Treatment/Goal/Service Plan The Adult Services social worker, after gathering the information needed to complete the assessment, helps the client come to terms with the reality of his/her situation and pointing out the individual strengths and weaknesses which the client possesses to deal with the situation. This enables the social worker and client to develop realistic goals and service plans. These goals and plan are usually time structures and task specific. Individualized goals are directed toward allowing the client to remain in their own home at his/her highest level of functioning and the prevention of problems. This may include providing the client with a mechanism to establish links with needed community resources. The goals and plans are monitored and reviewed at least quarterly and more often if needed. A complete reassessment is done yearly. Counseling/Therapy Examples of techniques utilized by the Adult service worker places emphasis on the family systems approach. These include problems identification, client advocacy, reality therapy, crisis intervention, problem solving, ego enhancement, environmental manipulation, positive reinforcement, and psycho-social assessment. The worker, at times, seeks community resources to assist the client in attaining and maintaining goals. Because some of the situations the client faces are very sensitive, the social worker offers supportive counseling. Implementation The Adult Service social worker on initial visit begins assisting the client in formulating and implementing the service plan by assessing appropriate and available services, presenting options to the client and encouraging client decision-makingwith the focus on problem solving and independent actions. The involvement of other services and/or other professionals is initiated at the discretion of the social worker in consultation with supervisor. Monitoring/Maintenance The Adult Service social worker monitors that the services plan on an on-going basis for progress, appropriateness and changes if needed. Changes may occur if there are changes in client family composition, health (i.e. physical or mental), economic, social environment, or etc. * 5%6.Consultation Most consultation is centered on client related problems and is provided for family and other informal support systems and for other professionals, agencies and community resources. The purpose of such consultation is to advocate for the client, to promote the goals of the service plan and to provide the needs of the client. * 3%7.Community Liaison The social worker, by advocating for the client, allows him/her to establish linkage to community resources. Work in this unit entails: l. Counseling and planning with the child's family intended to strengthen the ability of families to protect and care for their own children, to minimize harm to children and youth, and to ensure permanence in children's lives. 2. Arranging for provision of and assisting families in utilizing appropriate services and community resources. 3.Obtaining the cooperation of law enforcement agencies as required obtaining suitable care and services for the children in or out of their family origin 4.Initiation of court action when necessary. Primary Purpose of the Position This agency is mandated to provide services to children who have been identified as neglected, abused, or dependent. The primary purpose of the Child Protective Services In-Home Social Worker, Investigations, Assessment and Treatment, (SW IA&T) is to execute this mandate. This is an advanced, professional level casework position, providing counseling and case management services for children who are identified as neglected and/or abused or dependent. This is done so by assessing family situations to determine the strengths and weaknesses of the family system and to evaluate the level of risk the child may face in the home. The social worker also provides other services to families with children under the age of 18 years who have requested such services from the agency in conjunction with protective services. Treatment and immediate responses will be provided to crisis situations for at risk children in the most complex and hostile of cases. Work Schedule Regular working hours are from 8:00 a.m. until 5:00 p.m. Monday through Friday. However, in order to make the mandated number of face-to-face contacts in some situations, clients may need contact after hours and overtime may be required. Employees are given the option of flexing their workday to accommodate their clients' schedule without putting the social worker in an overtime situation. Employee serves on a rotation basis for periodic radio coverage. Change in Responsibilities or Organizational Relationship None Following is a description of responsibilities and duties of this position listed in order of importance. All responsibilities and duties are essential to the effective performance of an In-Home Services SW IA&T. Intervention developed The In-Home Services SW IA&T is responsible for reviewing the case and becoming familiar with the family dynamics, identified problems and strengths of the family. Utilizing the Child and Family Team process, a Family Services Agreement is devised jointly with the family based on the Risk Assessment and Initial Case Plan. The Family Services Agreement contains objectives that are measurable, time limited, describe specific desired outcomes and identify necessary behavior changes. Identified needs, objectives and activities are directed toward alleviating the problems that lead to the abuse, neglect or dependency and addressing underlying issues that may contribute to the need for protective services. Once the case plan is devised, the helping process is defined. Providing/arranging for counseling or supportive services The In-Home Services SW IA&T provides counseling to the client directed toward the development and utilization of family support systems including recognition of develop-ment and utilization of strengths, overcoming personal, social, and environmental barriers, and utilizing agency and community resources. The worker seeks community resources to assist the client in attaining their goals. Monitoring and assessment The In-Home Services SW IA&T must monitor the Family Services Agreement and complete a Risk Assessment at various intervals in all neglect, abuse and/or dependency cases requiring continued mandated services. After a Family Services Agreement is negotiated, ongoing monitoring and assessment is necessary to determine if the plan continues to be appropriate and that the plan of intervention continues to address the identified needs of the parent and children. This monitoring includes regular interviews with the client in the home and requires collateral contact with other public agencies such as mental health, medical professionals, day care providers, health department personnel, and relatives and friends of the client. Updating the Family Services Agreement is necessary when changes in the family occur and it becomes necessary to revise the plan in order to implement an appropriate plan of protection for the child. Court action is used in the event the client does not meet the objective and the safety of the child is at risk. It is the worker's responsibility to provide the supervisor with a factual assessment in order for the joint decision to be made regarding court action. Prior to court action, the worker must complete the Risk Assessment, which indicates the risk and/or harm to the child by remaining in the home. Use of the court and court preparation Court activity is very much a part of the Social Worker's duties. Prior to the decision for court intervention or in the case of an emergency, immediately after obtaining a non-secure custody order, the worker will present her case to the Child and Family Team which makes recommendations as to alternatives to court or the children entering into agency custody. In the event that out of home placement is necessary or, to ensure permanence in a child's life, the Social Worker initiates court action through a non-secure custody order and/or petition, assesses placement options (relatives or foster care) and conjointly with the foster care worker develops an appropriate plan for the child. The worker prepares the child and family for separation, placement and the court process. The worker prepares a written assessment for the court, solicits witnesses when necessary, shares information with the child's Guardian ad Litem and testifies in court making the agency's recommendation. The worker must also follow up with written reports for court review as ordered or required. The worker may conduct an investigation of suspected abuse and when evidence of abuse is found the worker is required to report such information to the District Attorney and law enforcement within a specified time frame both verbally and also in writing. Law enforcement and the Case Manager interview the victim and perpetrator. Should the perpetrator be prosecuted, the worker may be a witness during court proceedings. Social Workers may be subpoenaed to testify in both civil and criminal proceedings due to their involvement with their clients. Monitoring, reviewing, maintaining The In-Home Services SW IA&T is responsible for monitoring, reviewing, and maintaining cases through various means. This is accomplished through frequent contact with the client by visiting them in their home, interviews in the agency and telephone contact, collateral contacts and in some situations periodic court reviews. Both face to face and collateral contacts are made monthly according to state standard. The Social Worker formally assesses the client's progress by updating and re-signing the Family Services Agreement on a quarterly basis and whenever there is a major change in the family, reassesses the risk to the child(ren) by completing the Risk Assessment form. These reviews assess the progress of the client in alleviating the factors that lead to the abuse, neglect or dependency and whether the client has been able to demonstrate they can provide a safe environment for their child (ren). Further services may be identified at this time. The Social Worker is required to present cases in the agency's foster care review team when a child comes into agency custody. This team closely monitors those children whose custody has been removed from his parents or whose custody has been placed with the Department of Social Services. Interpretation of Child Protective Services The In-Home Services SW IA&T is a member of the agency's public forum and occasion-ally is called on to speak at meetings of community agencies and organizations in an effort to emphasize the preventive nature of the agency's services and to familiarize the commu-nity with the reporting laws. The worker assists in providing information to other agencies and local citizens to better help them understand the functions of the agency. This allows for better rapport that leads to better delivery of services and cooperation from other disciplines. Employee will participate in disaster related programs, including shelter duty, as needed. Other duties as assigned and required. In all duties outlined above, all forms of documentation required by the North Carolina Division of Social Services, North Carolina General Statutes, and Nash County Department of Social Services is included. Death Benefit - Alamance County Government has elected to contribute towards a Death Benefit associated with the Retirement System for all employees that are contributing Retirement System members. Once an employee has met at least one (1) year as a contributing member to the retirement system, the employee will have a benefit payable to their beneficiary of no less than $25,000 and no more than $50,000 based on the employee's salary. Benefit is payable if the employee is actively working and has not reached 70 years of age. Life Insurance - Basic term life insurance is the amount of one times the annual salary (no greater than $70,000) is provided for all FT employees. Additional coverage is available for purchase through payroll deductions for employee and eligible dependents. Retirement Benefits - Membership in the NC Local Government Employees Retirement System (NCLGERS) is a requirement for all full-time and regular part-time employees (working more than 1,000 hours annually). Employees contribute six percent (6%) of their annual compensation before it is taxed. If you leave the County service before retirement, you may withdraw your retirement contributions or elect to freeze your contributions in anticipation of future local government employment. If you have at least five (5) years of service, you are considered 100% vested in NCLGERS. Alamance County will be contributing a percentage as required by NCLEGRS on your behalf as well. Time Off Accruals: Vacation Time - All Alamance County full time employees immediately earn a bank of vacation hours that can be used after 6 months of employment (exhausting their probationary period). Vacation time accrues based on the full-time hire date (most recent hire date is used in the case of rehires). Employees with a hire date before the 15th of the month will receive vacation credit for that month; otherwise, time will not be earned. Time will be contributed to the employee's time off bank on the 15th of the month based on a monthly proration of the schedule below. Years of Service - Days of Vacation Annually 0-4 years of service 10 days equivalent to 80 hours 5-9 years of service 13 days equivalent to 104 hours 10-14 years of service 16 days equivalent to 128 hours 15-19 years of service 21 days equivalent to 168 hours 20 years or more 25 days equivalent to 200 hours Sick Time - All Alamance County full-time employees will earn a bank of sick hours to be used when they are away from work for instances related to their own illness or the illness of an immediate family member, death of an immediate family member or when they (or an immediate family member) need to attend a health-related appointment. All County employees earn 12 days of sick leave per year with an earnings of 8 hours per month if employed on or before the 15th of the month. County Observed Holidays - Alamance County will observe the State of North Carolina Holiday Schedule. Holidays will include the following and will be paid at 8 hours holiday pay. Exceptions have been made for certain departments given the nature of the services provided. Wellness Benefits & Programming: Employee Assistance Program: There are times when everyone needs a little help or advice. The confidential Employee Assistance Program (EAP) through a local provider can help you with things like stress, anxiety, depression, chemical dependency, relationship issues, legal issues, parenting questions, financial counseling and dependent care resources. Best of all it's free for County employees and available for your household to utilize. Utilization of the EAP is completely confidential and participation isn't recorded. Help is available 24/7/365 by telephone. Alamance County Employee Health Center opperated by Everside Health: Available to employees and spouses who are covered under the Alamance County Insurance Plan and their covered dependents, 18 years of age and older. Retired employees under age 65 are also eligible to use the clinic. Depending on insurance coverage elected, care at the clinic ranges from free to a fair market value cost. Services Available: Treatment of injuries or illnesses such as Flu, Sinus Infection, pink eye, sprains, strains or fever. Monitor chronic conditions (Diabetes, Hypertension, Hyperlipidemia, Health Disease, Asthma, etc...) Assistance with finding a Primary Care Provider Health Education and Counseling Triage and referrals to specialists when appropriate. Alamance County provides a variety of additional benefits including bereavement pay, Paid Parental Leave (up to 4 weeks), Wellness Leave to focus on work-life balance initiatives, Volunteer Time Off (paid time) opportunities to be a part of the Alamance Community, to opportunities for professional certification pay. Employee Fitness Center: Alamance County encourages all employees to maintain a healthy lifestyle both at home and at work. In support of this philosophy an Employee Fitness Center is available for all County employees to use at their convenience during non-work hours and is open 6:00 AM to 9:00 PM Monday-Sunday.
