Description of Work
The Social Worker for the Blind provides Independent Living Services, counseling, Medical Eye Care support, and specialized rehabilitation services to blind and visually impaired individuals of all ages residing in Lee, Moore, and Chatham Counties. This role focuses on helping consumers achieve or maintain independence by teaching techniques for daily living, personal care, home management, and self-advocacy.
The Social Worker for the Blind collaborates closely with community partners, medical providers, and other professionals to ensure consumers have access to the resources and support needed to reach and sustain their highest level of self-sufficiency.

Position Specific Information
Mecklenburg County is the Lead Agency for Coordinated Entry (CE) for the Charlotte-Mecklenburg Continuum of Care. Coordinated Entry is the intake for the local homeless response system. It is part of the process to access homeless resources and shelter in Charlotte-Mecklenburg.
This Social Worker I position conducts Coordinated Entry (both in person and telephonically) with individuals and families experiencing homelessness, or those at imminent risk of homelessness, to efficiently connect them to the best available (scarce) homeless shelter, temporary housing, or homeless prevention options in the community. It may require the provision of short-term Systems Navigation support to clients until which time they are connected to a homeless services case manager and/or their situation resolves.
This position also engages clients within a strengths-based, solution focused framework to identify their service needs and goals, and to facilitate referrals to community resources so that clients have a plan of action toward enhanced self-determination.
CE clientele is diverse and brings with them a wide range of experiences and concerns; and may be in different stages of mental health, substance use, and trauma recovery.
In addition to clients, the Social Worker I requires the ability to effectively engage with community partners to provide on-site CE to clients wherever they may be (in places not meant for human habitation, hospitals, treatment centers, day centers, shelters, etc.)

Position Summary

DUTIES AND RESPONSIBILITIES
ESSENTIAL FUNCTIONS OF THE POSITION:
* Performs and completes screening for substance misuse in order to identify goals that remove barriers for the participant to have successful reentry into the community.
* Performs case management services.
* Completes follow up of participants by remaining in contact with attorneys, court professionals, providers, and families for re-entry needs.
* Maintains referral lists and serves as a referral specialist by monitoring and responding timely to referrals from the medical provider or other justice professional and following up on referrals submitted.
* Identifies and facilitates access to services needed for each participant, such as developmental, behavioral, mental health, and substance misuse needs.
* Functions as a substance misuse information and referral specialist.
* Meets with participants prior to release to finalize their reentry plan that assists with connection to services that address identified needs as well as connects with community programs responsible for post-release services.
* Coordinates re-entry services as identified to include but not limited to housing, transportation, insurance, etc.
* Assists our medical provider within the Detention Center with continuity of care of MAT services as well as substance misuse and mental health needs.
* Facilitates harm reduction strategies, such as Narcan training.
* Performs accurate record keeping of all encounters, digitally with hard back-up.
* Provides analysis data tracking for quarterly outcome reporting.
* Prepares appropriate reports for public information related to substance misuse and opioid misuse for Harnett County.
* Tracks inmate progress accurately through use of Excel Spreadsheets and/or data tracking software.

* Attends required meetings and/or trainings focused on MAT services, as needed.

This position also provides temporary supportive and specialized services to non-Protective Services classified elderly and/or disabled adults, encouraging customers to develop healthy coping skills through linkage with local community resources. This program provides emergency and essential services to disabled adults who are in need of protective services. This position is responsible for investigating all screened-in referrals within twenty-four or seventy- two hours depending on the immediacy of the situation. Services focus on the safety of the customer and the possibility of remaining in his/her physical and social environment utilizing the strengths of the customer, his/her family, or social network to create a positive alternative to the current situation.

General Definition of Work:
The Kinship Navigator position aims to address the unique challenges faced by relatives who step in to care for the children of their family members. These caregivers often navigate complex systems and encounter barriers that leave them unsupported, especially within foster care structures where their contributions can be overshadowed. The Kinship Navigator role is designed to bridge these gaps, providing resources and advocacy to ensure these families receive the recognition and assistance they deserve.
The goal is to increase kinship caregivers' ability to provide safety, stability, and permanency for the children placed in their home. Purpose of the position:
The primary purpose of this position is to ensure child safety in compliance with state and federal statutes regarding child protective services.
Essential Functions of the position:
Assess the risk and safety of children in response to actual or suspected child neglect, abuse, or dependency through conducting a thorough investigation of the home, school and other locations or persons involved, conducting such assessments requires the use of a personal vehicle and a valid driver's license.
Develop and monitor service plans with families involved in Child Protective Services and provide or refer services as needed. Make recommendations to the court, including services needed, progress and barriers to permanent plan.
Provide direct services to children and their parents/caretakers through which a timely and comprehensive assessment is conducted to determine if abuse/neglect/dependency exist and provide for ongoing intervention if warranted.
Ensure safety planning for children in compliance with state and federal statutes and policies. If children cannot remain safely in their home a petition to the court and/or non-secure custody is sought.
Assess, support, supervise and counsel families involved with child protective services to ensure child safety, wellbeing and permanence. Evaluate available information to determine the appropriateness for intervention and level of services required.
Prepare legal documents, court reports, and court testimony when warranted in compliance with state and federal statutes and policies. Ensure all required documentation, forms and reports are completed and maintained in a timely and consistent manner in accordance with local, state and federal requirements.
Provide transportation on a limited basis to families when necessary to access services to address safety and wellbeing issues.

DUTIES AND RESPONSIBILITIES
Responsibilities and Duties:
Variety and Scope - Employees provide social work services in the areas child protective services, custody and adoption, and foster care work which involve the assessment of individual and family dynamics and needs, serving as expert witness at court hearings, crisis intervention, and counseling. Work involves explaining diagnosis and treatment alternatives to patients and families, counseling and participating in education endeavors for physicians, psychologists, nurses, other health disciplines, students and social work interns, and participating in research activities.
Intricacy- Employees use considerable judgment in conducting individual assessments, selecting and providing treatment techniques or dealing with psychosocial aspects or researching catastrophic or terminal diseases. Employees provide in-depth assessment of family dynamics, determine the extent of neglect or abuse, and provide counseling for clients with complex needs. Employees usually refer the most severe cases, such as severe phobias or unusual violence patterns to a higher level social worker or therapist. Employees educate and consult with other staff and community professionals for information purposes and joint case planning. Some didactic consultation may also be involved.
Subject Matter Complexity - Work requires a thorough understanding of social work assessment techniques and treatment approaches primarily of a crisis, supportive, and behavioral nature. Considerable knowledge of legal and administrative aspects of programs. I. Assessment/Evaluation - 30%
Assessment of the family's service needs and evaluation of the appropriate services to be provided is an essential component of this employee's responsibilities. Tools used include Mental Health and psychological evaluations, interviews with family members and collaterals, and structured assessment instruments. Input from inter-agency review teams is also used.
2. Development of Treatment Goals and Service Plans - 20%
The employee assists families in developing service plans and setting goals. Service plans are designed to prevent further abuse or neglect and to ensure safety of children within the family setting. If child is removed from the home, goals are designed with purpose of reuniting families. The purpose is to prevent further maltreatment and resolve problems in family unit. Written service and visitation agreements are developed between employee and client. These written agreements provide direction and clarification for parents who are sometimes overwhelmed by the agency and often unsure of the agency's expectations. Service and visitation agreements specify obligations for both parents and the agency. They formalize the parents' agreement and input for the treatment program. This approach is supportive of parents and keeps them involved in planning for their child's future.
3. Case Management and Service Plan Implementations - 20%
As case manager, employee designs and obtains appropriate services to meet needs of clients. Case management ensures a cohesive approach in the delivery of services. Employee is responsible for ensuring agency and court reviews for children in the custody or placement responsibility of the agency. This employee provides Medicaid-at-risk case management services for clients meeting medical criteria. Assists client in grasping relationship between current situation and desired goals and advocating for needed resources and services for client.

Employee uses various team approaches in providing services to clients. Permanency Planning Reviews review information provided regarding children in the placement authority of this agency. Employee plans and organizes information presented to team for purpose of making decisions and plans regarding continued need and appropriateness of child's foster care placement. These are held quarterly throughout the time the child is in foster care.
The number of cases assigned varies. Employee usually carries fifteen to twenty cases at a given time. Foster care cases are assigned immediately after adjudicatory court hearing or immediately after family signs voluntary placement agreement. Primary Purpose of the Position

This is an advanced, professional level casework position, providing counseling and case management services for children who are identified as neglected and/or abused or dependent. The social worker also provides other services to families with children under the age of 18 years who have requested such services from the agency in conjunction with protective services. Work Schedule
Regular working hours are from 8:00 a.m. until 5:00 p.m. Monday through Friday. However, in order to make the mandated number of face-to-face contacts in some situations, clients may need contact after hours and overtime may be required. Intervention developed
The In-Home Services SW IA&T is responsible for reviewing the case and becoming familiar with the family dynamics, identified problems and strengths of the family. Utilizing the Child and Family Team process, a Family Services Agreement is devised jointly with the family based on the Risk Assessment and Initial Case Plan. The Family Services Agreement contains objectives that are measurable, time limited, describe specific desired outcomes and identify necessary behavior changes. Identified needs, objectives and activities are directed toward alleviating the problems that lead to the abuse, neglect or dependency and addressing underlying issues that may contribute to the need for protective services. Once the case plan is devised, the helping process is defined.
Providing/arranging for counseling or supportive services
The In-Home Services SW IA&T provides counseling to the client directed toward the development and utilization of family support systems including recognition of develop-ment and utilization of strengths, overcoming personal, social, and environmental barriers, and utilizing agency and community resources. The worker seeks community resources to assist the client in attaining their goals.
Monitoring and assessment
The In-Home Services SW IA&T must monitor the Family Services Agreement and complete a Risk Assessment at various intervals in all neglect, abuse and/or dependency cases requiring continued mandated services. After a Family Services Agreement is negotiated, ongoing monitoring and assessment is necessary to determine if the plan continues to be appropriate and that the plan of intervention continues to address the identified needs of the parent and children. This monitoring includes regular interviews with the client in the home and requires collateral contact with other public agencies such as mental health, medical professionals, day care providers, health department personnel, and relatives and friends of the client. Updating the Family Services Agreement is necessary when changes in the family occur and it becomes necessary to revise the plan in order to implement an appropriate plan of protection for the child. Court action is used in the event the client does not meet the objective and the safety of the child is at risk. It is the worker's responsibility to provide the supervisor with a factual assessment in order for the joint decision to be made regarding court action. Prior to court action, the worker must complete the Risk Assessment, which indicates the risk and/or harm to the child by remaining in the home.
Use of the court and court preparation
Court activity is very much a part of the Social Worker's duties. Prior to the decision for court intervention or in the case of an emergency, immediately after obtaining a non-secure custody order, the worker will present her case to the Child and Family Team which makes recommendations as to alternatives to court or the children entering into agency custody. In the event that out of home placement is necessary or, to ensure permanence in a child's life, the Social Worker initiates court action through a non-secure custody order and/or petition, assesses placement options (relatives or foster care) and conjointly with the foster care worker develops an appropriate plan for the child. The worker prepares the child and family for separation, placement and the court process. The worker prepares a written assessment for the court, solicits witnesses when necessary, shares information with the child's Guardian ad Litem and testifies in court making the agency's recommendation. The worker must also follow up with written reports for court review as ordered or required. The worker may conduct an investigation of suspected abuse and when evidence of abuse is found the worker is required to report such information to the District Attorney and law enforcement within a specified time frame both verbally and also in writing. Law enforcement and the Case Manager interview the victim and perpetrator. Should the perpetrator be prosecuted, the worker may be a witness during court proceedings. Social Workers may be subpoenaed to testify in both civil and criminal proceedings due to their involvement with their clients.
Monitoring, reviewing, maintaining
The In-Home Services SW IA&T is responsible for monitoring, reviewing, and maintaining cases through various means. This is accomplished through frequent contact with the client by visiting them in their home, interviews in the agency and telephone contact, collateral contacts and in some situations periodic court reviews. Both face to face and collateral contacts are made monthly according to state standard. The Social Worker formally assesses the client's progress by updating and re-signing the Family Services Agreement on a quarterly basis and whenever there is a major change in the family, reassesses the risk to the child(ren) by completing the Risk Assessment form. These reviews assess the progress of the client in alleviating the factors that lead to the abuse, neglect or dependency and whether the client has been able to demonstrate they can provide a safe environment for their child (ren). Further services may be identified at this time. The Social Worker is required to present cases in the agency's foster care review team when a child comes into agency custody. This team closely monitors those children whose custody has been removed from his parents or whose custody has been placed with the Department of Social Services.
Interpretation of Child Protective Services
The In-Home Services SW IA&T is a member of the agency's public forum and occasion-ally is called on to speak at meetings of community agencies and organizations in an effort to emphasize the preventive nature of the agency's services and to familiarize the commu-nity with the reporting laws. The worker assists in providing information to other agencies and local citizens to better help them understand the functions of the agency. This allows for better rapport that leads to better delivery of services and cooperation from other disciplines.

When conducting investigations, employee sometimes finds it necessary to use anatomically correct dolls. Employee must have immediate access to a vehicle to perform essential functions of the job.
Visual attention, mental concentration and manipulation skills
Close visual attention is required when operating a motor vehicle. Intake, investigations, and case management duties all require visual attention and mental concentration. Body language, eye contact, and behavior of family members assist the social worker in assessing the family situation and must be viewed carefully. Preparation of court reports, development of protection plans, and completion of risk assessments require very intense mental concentration, as does reviewing placement options for children. The social worker must carefully document times, dates, and other factual information so that accurate assessments and recommendations are made. Services for maltreated children are aimed at strengthening family life by supporting and improving parental/caretaker abilities, which will assure a safe nurturing home for children. Services are home-based and are preventive, rehabilitative and non-punitive with efforts directed toward identifying, providing or arranging services as needed. Social workers may provide group, family or individual treatment to clients of any age and any developmental, medical, mental, substance abuse, financial or family problem. Child Protective Services is an advanced professional level of casework. Casework is accomplished through parent/caretaker cooperation and consent or if the child's safety is at eminent risk, through court petition as the agency's responsibility foremost to protect the child. The worker will receive referrals that meet the NC legal definition for neglect, abuse or dependency and, following the Multiple Response System, will conduct a family or investigative assessment to determine if the family needs further services to ensure the protection of children. The process involves reviewing agency records to be aware of family history, interviewing the parents, children, and collaterals, completing safety assessments, risk assessments, and strength/needs assessments throughout the fact-finding process. The worker will complete a comprehensive assessment and document all of the aspects of the family's life including their social activities, economic situation, environmental issues, mental health needs, activities of daily living, physical health needs, and a summary of strengths. (SEEMAPS) The work involves visits to the homes of clients, schools, medical and mental facilities, prison facilities and other agencies. The worker will involve law enforcement when investigating cases of physical and sexual abuse, or at other times when deemed necessary. When safety concerns are discovered, safety plans will be developed with the family to ensure safety of the children. Decisions are often made under stressful and sometimes dangerous conditions. F. Variety and Purpose of Personal Contacts
Employee has contact with unit staff, other units within the agency, clients, Health Department, other Social Services departments, Guardian Ad Litem Program, Mental Health, medical personnel, crisis organizations, churches, civic organizations, judicial system, law enforcement, Emergency Services personnel, and the general public. Contacts are often used as a means to assist in planning and/or meeting the needs of clients as a part of the human delivery system and to aid in assessing client's current situation.
G. Physical Efforts
Employee must be able to move within and without the agency and climb stairs. Employee occasionally lifts small children, camera, suitcases, toys, car seats, etc. in performance of duties. Employee must travel to meetings within the county as well as workshops and meetings out of the county. Overnights stays are sometimes necessary.
H.Work Environment and Conditions
Work is performed in a county social service agency with contacts in the field. Employee encounters hostile clients, some of whom are dangerous. Clients include mental patients, alcoholics, and substance abusers who are sometimes verbally abusive and threatening. All staff are also trained in the use of fire extinguishers. Field contacts are sometimes in adverse weather conditions such as snow, sleet, extreme heat and natural disasters. Employee may be exposed to infectious diseases and all staff are aware of precautions to take when exposure occurs.
I.Machines, Tools, Instruments, Equipment, and Materials Used
Employee uses laptop, iPad, printer, copier, calculator, automobile, telephone and cell phone, and car seats. When conducting investigations, employee sometimes finds it necessary to use anatomically correct dolls.
J.Visual Attention, Mental Concentration, and Manipulation Skills
Close visual attention is required when operating a motor vehicle. Intake, investigations, and case management duties all require visual attention and mental concentration. Body language, eye contact, and behavior of family members assist the social worker in assessing the family situation and must be viewed carefully. Preparation of court reports, development of protection plans, and completion of risk assessments require very intense mental concentration as does reviewing placement options for children. The social worker must carefully document times, dates, and other factual information so that accurate assessments and recommendations are made. Employee must have physical dexterity required to successfully and efficiently operate vehicles.
K.Safety for Others
Employee makes decisions daily that affect the lives of clients. Employee devises Safety Plans, completes Strengths and Needs Assessments and Risk Assessments in investigations. Employee also makes recommendations regarding the removal of children when homes are not safe and the return of children when homes are safe. Employee must carefully evaluate the needs of children in placement responsibility and their caregivers to determine the best plans for children. Worker abides by confidentiality guidelines to protect clients and uses caution when operating a motor vehicle when transporting clients. See #8 above for issues confronting the employee and for precautions.

Child Protective Services is periodically affected by changes in methodology, guidelines, legislation, and shifting program emphasis.

Essential Job Functions
Serves as a crisis response specialist when responding to law enforcement calls for service. Primary responsibilities in the field will be de-escalation through crisis intervention and appropriate referrals.
Acts as a liaison between patrol officers, partner agencies, and the public.
Determines the best course of action and provides short-term crisis intervention services in situations that may include domestic violence, child abuse, sexual abuse, suicide, serious or fatal accidents, other cases involving emotional trauma, and people experiencing a mental health crisis.
Maintains records and confidential case files.
Compiles and maintains statistical data related to law enforcement calls for service related to social work needs and highlights gaps in available services.
Builds rapport and effective working relationships with city staff.
Provides input to Chief of Police and senior leadership of Police Department related to social work incidents and awareness.
Informs victims of available resources and acts as a liaison in obtaining services.
Assesses and reviews crime reports and ensures victims are contacted within established time frames.
Maintains working knowledge of available resources in the community.
Performs needs assessments, coordinates referrals and collaborates with external partners to provide comprehensive support services.
Complete all necessary training sessions.

QUALIFICATIONS
Education and Experience:
Bachelor of Social Work (BSW) required, Master of Social Work (MSW) preferred. One (1) to three (3) years' experience in the social work field is preferred. Experience working in the law enforcement field that provides the required knowledge, skills, and abilities is preferred, but not required.
Special Qualifications:
Possession of a valid driver's license to operate a motor vehicle. Requirement exists at the time of hire and as a condition of continued employment.
