The Behavioral Health Consultant (BHC) will work to support the mission and vision of Neighborhood Healthcare by providing direct patient care for behavioral health services. This role will meet with individuals and families to provide brief behavioral psychotherapy services to patients and consultations primary care providers using an integrated care model.
This is a hybrid position.
This position is eligible for a $10,000 signing bonus!
Responsibilities
Direct Patient Care
Provides short term targeted psychotherapy for problems identified by primary care providers
Performs initial and ongoing behavioral health assessment to develop patient care plans
Evaluates patients to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical care
Provides brief psychotherapy to patients and families
Addresses and factors co-occurring disorders into patient's care plan
Responds promptly and with caring actions to patients and employees
Provide crisis management and psychoeducation for patients and families
Conducts “warm hand offs” at medical provider requests
Acknowledges and honors patients' psychosocial, spiritual, and cultural beliefs
Utilizes BHC staff protocols for referral, follow-up, and quality assurance
Provider & Team Support
Consults with primary care providers about behavioral aspects of their patients' medical treatment
Contributes to the success of the organization by participating in quality improvement activities
Maintains professional working relationships with all levels of staff, clients, and the public
Cooperates in accomplishing department goals and objectives
Participates in case conferences, trainings, and departmental or agency meetings, as directed
Regulatory Compliance
Follows BHC protocols, policies, and procedures
Provides personal information and participates in the credentialing process with managed care companies
Attends continuing education programs offered to upgrade professional skills and maintain licensing requirements
Documents legible and legally defensible medical records
Qualifications
Education/Experience
Education
Doctorate degree in psychology with a valid CA Psychology License, or
Master's degree in social work with a valid CA LCSW License, or
Master's in degree in counseling with a valid CA LPCC License
Valid BLS certification from approved American Health Association provider required upon hire
Two year of pre/post licensure psychotherapy experience with children, adults, and/or adolescents required; child/adolescent experience required
Experience providing health psychotherapy services, such as smoking cessation, weight management, stress management, or work with chronically medically ill patients
Bilingual (English/Spanish) preferred
Additional Qualifications (Knowledge, Skills and Abilities)
Knowledgeable about and experience with displaying of normal signs of human development and ability to assess and provide age-appropriate care
Ability to work well in an interdisciplinary team within medical care.
Ability to operate one or more word-processing, spreadsheet, and/or electronic medical records systems
Ability to do some charting simultaneously while in session
Excellent verbal and written communication skills, including superior composition, typing, legible handwriting, and proofreading skills
Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
Ability to successfully manage multiple tasks simultaneously
Excellent planning and organizational ability
Ability to work as part of a team as well as independently
Ability to work with highly confidential information in a professional and ethical manner

Salary Ranges
Psychologist: $110,000-$160,000 per year, depending on experience.
LCSW/LPCC: $95,000-$130,000 per year, depending on experience.

Family Health Centers of San Diego (FHCSD) is a 501(c)(3) nonprofit organization rich in history and tradition. In 1911, the San Diego Industrial School was built on the site where our first clinic now stands. This school was originally a settlement house that provided Barrio Logan's immigrant residents with social services. The community grew around this location, and by 1940, Barrio Logan had become one of the largest Mexican-American communities on the West Coast.
In October 1970, Laura Rodriguez and a group of community members sought to bring much-needed medical services to Barrio Logan. The day the group began occupying the building, Laura had attended a Barrio Logan Community Action Committee meeting where she passionately advocated that it should function as the site of a community health clinic. When the request fell on deaf ears, the protest was initiated that evening. Other community members, including several fearless abuelitas, participated in the building occupation by remaining inside overnight. When the police arrived, none dared arrest the grandmothers.
After eight days of occupation, succeeded in negotiations with the City of San Diego and the community. This led to the gifting of the building to the community of Barrio Logan and the establishment of the Chicano Free Clinic. Medical care delivery began two evenings a week, and the health center saw approximately 150 patients each month.
This building eventually became Logan Heights Family Health Center, the flagship clinic of Family Health Centers of San Diego (FHCSD). FHCSD has since transformed into the tenth largest health centers in the country, providing care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured.

Under the direction of the Infectious Disease and Special Programs (IDSP) Behavioral Health Program Manager and clinical supervisor of the Behavioral Health department, the Licensed Mental Health Clinician (MHC) is responsible for providing individual and group counseling with person living with or at risk of HIV. The MHC performs thorough psychosocial assessments of incoming clients. The MHC represents the IDSP department in the community, interacting and networking with community service agencies that provide support to persons living with HIV, substance use, and metal health co-occurring disorders. Cultivates and maintains communication and networking with diverse organizations in the HIV community. The MHC serves as a role model for cultural and linguistic competency for other staff to emulate. Maintain client confidentiality at all levels.
Essential Functions of the Job:
Provides individual, couples, family and group mental health and substance use counseling to persons living with HIV and co-occurring disorders. Service must be delivered in a gender and culturally appropriate fashion. Counseling will be providing in a fashion consistent with the State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.
Conducts thorough psychosocial assessments of incoming clients.
Works with clients to develop treatment plans; performs crisis intervention as needed.
Assesses and identifies client's needs for supportive services on an ongoing basis.
Acts as a resource, assists n obtaining and/or making appropriate referrals for necessary or requested services.
Facilitates group sessions and leads specialized educational classes; collaborates with other service team members and other appropriate individuals on clients' status; within scope of responsibility and expertise, provides guidance and instruction to other staff members regarding appropriate treatments for clients.
Establishes and maintains contact within community resources to ensure continuity of care (e.g. case managers, rehabilitation and detoxification centers, psychiatric units.)
Maintains a case load as assigned by contractual obligations.
Maintains professional records and documentation, including progress notes, treatment plans and ongoing evaluation of client progress, consistent with agency expectations and ethical standards.
Participates in team Case Conference meetings; HIV Multi-Disciplinary meetings; weekly case reviews; quarterly chart reviews.
Additional Duties and Responsibilities
Care Coordinator/Program Support:
Requests assistance and guidance from Clinical Supervisor and professional staff when necessary to address client issues and problems.
Knows the terms, conditions and scopes of work of contracts and grants that fund the mental health services.
Ensures thorough monthly monitoring, adequate progress towards contract goals and objectives.
Maintains, updates, and implements policies and procedures for mental health services.
Conducts quality improvement and evaluation activities for mental health services.
Completes monthly and quarterly narrative and statistical reports in an accurate and timely manner.
Participates in weekly IDSP Department staff meetings and staff development trainings, as needed.
Maintains flexible schedule to allow for some after hour and/or weekend work, as well as emergency coverage of shifts to meet the needs/demands of clients, service, and/or department related activities and events.
Assists leadership in the promotion and implementation of client activities, educational presentations, and special events.
Represents San Ysidro Health to other organizations and community groups. Network with other SYHealth programs and HIV/AIDS service providers.
Job Requirements
Education Required:
Successful completion of terminal graduate training program in a psychotherapy-related field: M.A., or M.S. in Social Work, related fields at an accredited university or college.

Auto-ApplyUnder the direction of the Chief Behavioral Health Officer, the Licensed Clinical Social Worker is responsible for performing psychological assessments and providing brief evidenced-based treatment using the primary care behavioral health model to adults, older adults, children and their families in a primary care setting. This licensed clinician works closely with primary care providers to develop and implement sound treatment plans. Responds to and provides crisis intervention services when needed; Ensures behavioral health services are delivered in a culturally appropriate fashion and is consistent with State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines.
Essential Functions of the Job:
Evaluates, triages and provides evidence-based, brief psychological/behavioral health interventions to patients with physical health, mental health, and/or substance abuse diagnoses.
Provides consultation to clinical staff and primary care providers (PCPs) as needed regarding the patient's care, treatment recommendations and progress via documentation in the client's record, face to face or telephonic consultation.
Performs brief assessments of patient needs and, with patient and PCP, develops a negotiated care plan. Evaluates and/or revises care plan as indicated with appropriate documentation in progress notes and communicates this to relevant multi-disciplinary team members.
Maintains accurate and timely documentation in the electronic medical record of all patients seen, completes forms and documents as required by the department, company, state, and federal agencies, and produces reports of evaluation and/or treatment, as required.
Performs crisis interventions as needed including but not limited to conducting suicide assessments and makes recommendations to medical providers and patients determines if crisis response services are appropriate for patient safety.
Works in coordination with the Chief Behavioral Health Officer and Director of Behavioral Health Operations to provide information to healthcare providers about mental illness, behavioral health services, and other topics as requested.
Participates in meetings and committees, including but not limited to case conferences, treatment team meetings, peer review, multidisciplinary team meetings, etc. Participates in committees and work groups as directed.
Actively participates in quality assurance activities of the department, including regular peer review and development and revision of policies and procedures.
Maintains productivity expectations in accordance with clinic and program guidelines.
Works closely with all providers to establish full integration of services and team-based approach to care.
Establishes and maintains contact with community resources to ensure continuity of care (e.g. case managers, rehabilitation and detoxification centers, psychiatric units.)
Establishes and maintains effective working relationships with clients, family members, other professional staff and the public.
Performs any other job-related duties as needed, assigned, or requested.

Job Requirements
Education Required:
Successful completion of terminal graduate training program in a psychotherapy-related field: Master's in Social Work or an equivalent program at an accredited University or College.
Experience Required:
Combination of education and years of experience as specified above. Sensitivity to multi-cultural concerns and the psychotherapeutic process required. Experience with community mental health preferred.
Certifications Required:
LCSW issued by the state of California and a valid California driver's license.

Bilingual in English/Spanish preferred
