This opportunity is also available in Tawas City, MI & Alpena, MI. Sault St. Marie, MI : Primarily: Outpatient, floats to acute care. Shift Notes: 9-5:30 with flexibility. Rotating weekend, 4 weekends within a year, 1 holiday every 3ish years. Tawas City, MI : Primarily outpatient, with some acute care coverage. Shift Notes: 8-5:30 M-Th and 8-12 on Friday with no weekends, no holidays Alpena, MI : Primarily outpatient but do float to acute care sometimes. Shift Notes: Monday-Friday, roughly 9-5:30pm, weekend rotation (1 per month) Physical Therapist I : This position delegates responsibility to the Physical Therapist Assistant and Aides. Activities of this position are patient focused and utilize assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and governing policies and procedures. ESSENTIAL DUTIES AND RESPONSIBILITIES: Evaluates the patient's function by performing formal/informal testing measures to determine the skeletal, neurological, respiratory, and cardiovascular status. Develops a goal specific treatment regime based on assessment data and the patient's tolerance to carry-out the treatment plan as prescribed. Submits a written treatment regime to be included as part of the plan of treatment authorized by and coordinated with the physician. Updates plan of care as needed to reflect changes in patient status. Co-signs verbal orders received by physical therapist assistant as necessary. Devises a schedule of treatment, which is frequency and duration specific to be discussed with patient/family/caregiver and coordinated with clinical support staff. Educates patient, family, and/or caregiver regarding pathology or condition, patient alternatives, prognosis, and rationale for treatment through forms of rest, exercise, modalities, and other indicated interventions. OTHER DUTIES AND RESPONSIBILITIES: Provides supervision for Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides and students per affiliate and regulatory standards. Recommends adaptive/supportive equipment based on assessment; contacts appropriate representative and/or supplier to place order and assists in securing equipment for patient. Documents the patient/family/caregiver response to treatment and changes in patient condition per affiliate documentation policies and/or standards and in compliance with regulatory agencies. Completes daily documentation necessary for billing procedures per affiliate policy and in compliance with regulatory agencies. Provides treatment intervention and demonstrates competence to facilitate or restore optimal functioning specific to therapist's patient care area. Educates patient/family/caregiver in home education program using relevant and effective teaching methods. Demonstrates effective oral and written communication with all internal and external customers to maximize quality of patient care including coordination with interdisciplinary team members. Promotes education and development among staff, students, and other health care providers using relevant and effective teaching methods. Promotes process improvement. Assists with program development, Participates in team conferences and interdisciplinary meetings as appropriate. Promote physical therapy within the facility and service area, participating in and providing community education. Function as case manager for assigned cases. Demonstrates commitment to facility and specific affiliate mission as evidenced by customer service, quality of service, and teamwork. Maintains confidentiality of patient and affiliate information in accordance with facility and affiliate policies and procedures. Other duties as assigned. PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS: Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Job Title: Intake Therapist Department: Integrated Health Reports To: Team Lead Intake Therapist FLSA Status: Exempt Centralized Intake provides Intake screening to Detroit Wayne Integrated Health Network (DWIHN) and Certified Community Behavioral Health Clinic (CCBHC) consumers. This position assesses consumers for outpatient mental health services and Intellectual and Developmentally Disabled consumers (I/DD) by evaluating the nature and severity of an Individual's mental health problem determining which service response would best meet the needs of the patient, doing an eligibility assessment, and providing a referral to an appropriate program. Requirements and Duties: Knowledge of Clinical assessments and clinical writing skills are essential. Knowledge of the use of DSM-V. Knowledge and ability to diagnose in assessment and treatment planning. Knowledge of common signs and symptoms specific to the I/DD population. Knowledge of common mental health conditions, substance abuse conditions and treatments/supports interventions. Knowledge of behavioral and cognitive techniques and therapeutic treatment interventions. Ability to effectively assess and intervene during crisis situations, including suicidality.
