Focusing more on the employees and their safety instead of always using them as scapegoats. The issues are deep seeded and management is a tough nut to crack. Most efforts to improve lasted a week or 2 then back to the old ways. Increase accountability for all and show people you care about them, the job they do for you and treat them as humans too and the culture could begin to improve.
My interview was back when paper applications were still mainstream. I knew nothing about the company or what they did but they hired me anyway. They used to love training people up and building them up from scratch. Not sure how the interview now
