Organization facts:
Bozeman Health, governed by a community board of directors, is responsible for the flagship Bozeman Health Deaconess Hospital, Bozeman Health Medical Group, Highland Park Medical Campus, and Bozeman Health Big Sky Medical Center. The integrated health system also includes Bozeman Health Hillcrest Senior Living with independent and assisted living facilities, an outpatient services center, a same-day surgery center, a clinical research group and Bozeman Health Urgent Care. Today, Bozeman Health is the largest private employer in Gallatin Valley with more than 2400 employees.
The hospital is an 86-bed facility with 8 ORs, 20 bed Critical care unit, and 25 bed ED. Approximately 28,000 ED visits per year. A new NICU opening later this year as part of a comprehensive women and children's care system.
The hospital has level III ACS Trauma Center designation and is DNV accredited. There are approximately 260 physicians on the medical staff representing a broad spectrum of both medical and surgical specialties.

Overview

Randall Children's Nephrology at Legacy Emanuel is recruiting a full time, BE/BC pediatric nephrologist to join an established team of two pediatric nephrologists, nephrology nurses, and renal dietitian. Candidates with strong interests in patient care, quality improvement, clinical research, and teaching are encouraged to apply. Our program offers a full range of inpatient and outpatient services that encompasses general nephrology, renal replacement therapy, and pre- and post- kidney transplantation care.

Randall Children's Hospital (RCH) is a regional leader providing exceptional family-centered care for children throughout Portland and the Pacific Northwest. RCH offers services along a comprehensive spectrum of pediatric subspecialty and ancillary care. Our division partners closely with our three board-certified pediatric rheumatologists to ensure seamless multidisciplinary care for children with rheumatic disease. We also provide renal services to an active pediatric ECMO program.

Randall Children's Hospital is a state-of-the-art, 9-story freestanding Children's Hospital affiliated with Legacy Emanuel Hospital. Our facility offers a 177-bed free-standing facility that includes a 24-bed PICU, 45-bed NICU, 25-bed Children's Day Surgery Unit, 14-bed Children's Day Treatment Unit for outpatient infusion, 22-room Children's Emergency Department, and the region's only inpatient rehabilitation facility . This is 100% nephrology, with the ability to connect with CKD and ESRD patients needing urgent care and engagement with patients who are seeing a nephrologist for the first time. Monogram nephrologists are quality-driven physicians who apply their clinical expertise in a managed care, population health context. By practicing at Monogram Kidney Care, they serve patients across the country. They also engage in population health management in a sophisticated, outcomes-driven manner that promotes palliative care where clinically appropriate and focuses on patient education, autonomy, and quality of life.

Monogram nephrologists share a passion for raising quality outcomes for health plan members suffering from kidney disease. They practice in a way that promotes patient engagement long before ESRD, resulting in delayed disease progression. In the case of kidney failure, the focus is informed patient choice, permanent vascular access and planned dialysis starts where clinically appropriate, and close medical management of non-dialysis patients where dialysis is not the clinically appropriate choice. They treat patients and follow health plan members with kidney disease and related co-morbidities.

Monogram nephrologists may work remotely from home, unless needed to see patients in person locally. As part of working at Monogram’s national practice, they will be licensed in multiple states after joining the company. Monogram nephrologists work on an employed basis, full time, five days per week.

Roles and Responsibilities

  • Treat and diagnose patients with CKD and ESRD
  • Apply evidence-based criteria in determining the appropriateness and necessity of care; in particular, in determining whether dialytic or non dialytic care is the correct evidence-based pathway
  • Reduce unnecessary or preventable episodes of care, including hospital readmissions
  • Identify and make practical, pointed interventions to improve member care
  • Review Annual Wellness Visit assessments to ensure proper coding and provision of medical diagnoses across multiple comorbid conditions
  • Assist in the training of NPs and other providers to provide high quality care
  • Conduct meaningful end-of-life and treatment options discussions with patients
  • Other duties as assigned

Position Requirements

  • 1+ years nephrology practice experience
  • Desire to practice in a forward-looking renal care environment focused on population health management
  • Demonstrated experience applying evidence-based clinical criteria
  • Experience in renal care, geriatrics and/or emergency medicine
  • Comfort and skill in end-of-life discussions and advance care planning
  • Experience with high-need Medicare Advantage and managed Medicaid populations, as well as commercial populations
  • Experience integrating acute and behavioral health care and appreciation of social determinants of health and the value of social workers
  • Expertise in managing medication panels for complex patients
  • Required Licensure
    • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare. Willingness to become licensed in multiple states as company and patient needs require.
