Support the Business Solution Managers(BSM) and Program/Project Managers during readiness discussions by guiding them in establishing alignment at critical checkpoints throughout the project lifecycle (e.g. PLAN, Kickoff, Project Closure) Attend PM CoE and Scrum/Kanban Communities of Practice. Participate in other shared learning events and activities with the community. Facilitate Quarterly Product Planning (QPP) and Quarterly Business Reviews(QBRs) events. The ICOACH will be responsible for maintaining a safe and supportive environment for members through remote and in person coaching, while ensuring that members are treated with dignity and respect. The ICOACH will support the interests, culture, and needs of the people we serve, and to promote respectful interaction with all individuals. The ICOACH will work successfully within a team as well as professionally represent Waypoint, supporting individuals using in person coaching and the remote coaching platform. The ICOACH will provide remote support as defined in the member's Personal Care Plan. The ICOACH will exemplify professional values, ethics, responsibility, and confidentiality. Requirements Activating and connecting to each location monitored by cameras and sensors by establishing a connection to each site at the beginning of every shift. Reconnecting to each location and checking the status of the cameras and sensors on an hourly basis or as assigned (24/7). This check may be performed more frequently if requested by the individual living arrangement due to the specific needs of members. Monitoring the presence of staff and compliance with minimum staffing ratios in homes/programs and contacting supervisors when staffing shortage is observed. When needed, dispatching staff to a location to resolve a situation as detected by the system or due to monitoring pre-scheduled needs of the consumers living at each residence. These needs may include, but are not limited to, turning, or repositioning a consumer at certain times, assisting with teaching interactions, and responding to safety or behavior issues Problem solving with each individual team regarding ways to best support the needs of individuals served in each living arrangement by providing support and guidance to program staff as needed and keeping accurate records regarding issues occurring throughout their shift. Ensuring the safety, and rights of persons served and responding to requests for assistance from program staff and/or persons served utilizing a variety of required communication tools, implementing home security measures, and using sound judgment to react to unplanned and emergency situations. Remotely supporting persons served to increase their independence and to exercise their rights by remotely and in person prompting and teaching skills in various activities of daily living as natural teaching opportunities arise, using informal teaching procedures. Provide support and guidance for members as outlined in their Personal Care Plan. Exercise sound judgment to ensure the safety and welfare of members. Notify supervisor of any situation developing or occurring in the home which could have a negative impact on the home's operation or member's welfare and safety. Acknowledge and respond to software notifications within appropriate timeframes, while prioritizing alert responses appropriately. Fosters enhanced awareness/understanding of athletic training, sports medicine, and clinic/hospital programs to the department, hospital, schools and other community organizations. Job Descriptions/Duties The Family Success Network Family Coach provides direct services to families including administration of screening tools, home visiting, parent education programs, resource management and referral, concrete supports, financial literacy and family coaching. Specific activities include, but are not limited to, the following: Provide ongoing supportive consultations and prevention case management services delivered either virtually or through a combination of phone calls, emails, skype, or in-person through individual meetings, including but not limited to: Assessing and administering universal screenings, financial literacy training, and/or concrete supports. Universal screenings will assess families for trauma, substance use, and mental health. The tools utilized may include but are limited to: the Adverse Childhood Experience (ACEs) Survey; the Child Trauma Assessment Checklist (CTAC); UNCOPE (substance abuse screening); and a mental health screening tool. Provide short-term, intensive, in-home services: Provide interested families a series of one to two home visits each month and access to the full array of the six pillars (family coaching, parent education, basic life skills, financial literacy, service referrals, and concrete supports) of service delivery. Serve as a Family Coach to develop a prevention services plan, outlining the families' goals and will utilize evidence-based programming to enhance the families' protective factors and increase the family's stability. Provide parenting programming and facilitate access to Life Skills training, as well as Family Coaching, which will incorporate Motivational Interviewing to assist the family in moving towards behavioral change. All families engaged in Family Success Network services will work with a Family Coach who will complete the following tasks, as agreed upon by the family: assess the strengths and needs of families provide concrete support in times of need develop and work with families on family success plans provide financial literacy utilize trainings to engage families and address identified needs, including but not limited to Triple P, Transition to Independence Process Model, Motivational Interviewing, and Strengthening Families Framework provide services directly to families in their own homes or neutral setting Manage an ongoing caseload of up to 30 families at one time, engaged in and receiving services. Attend meetings amongst collaborative members. Develop rapport with local social service partners and community resources to build and maintain a professional, responsive relationship with key community partners. Document services consistent with requirements of OCTF Program Manager and FSN Program Manager. Submit regular programmatic and fiscal reports to the Program Manager, as well as to other identified partners requesting data. Actively engage with family and child serving systems, including attendance at community meetings, such as Family and Children First Council. Represent the OCTF at any required national and local trainings and meetings. Serve as point of contact for the county of service; and Other duties, as assigned. Qualifications Minimum high school diploma is required; college preferred. Experience working with families for a minimum of one year. Must have a valid driver's license and reliable transportation. Must be flexible, knowledgeable of community resources, and a team player. Ability to work with diverse population is required.
