The Extended Care Worker is responsible for supervising and engaging students after and/or after school hours in a safe, caring, and structured environment. The worker supports the physical, emotional, and social development of students by facilitating recreational activities, providing homework support, and ensuring a smooth transition between the school day and extended care programming.
Key Responsibilities:
Supervise students during after- and/or after-school care, ensuring their safety and well-being at all times.
Maintain a positive, respectful, and inclusive environment for all students.
Plan and implement age-appropriate activities such as games, crafts, and outdoor play.
Assist students with homework as needed.
Monitor student behavior and follow school disciplinary procedures as necessary.
Maintain accurate attendance records and incident reports.
Communicate effectively with parents, staff, and administrators.
Ensure compliance with school policies and state childcare regulations.
Clean and organize the care area at the beginning and end of each shift.
Perform other duties as assigned by the Extended Care Coordinator or school administration.

Position Title: Full Time Weekend Direct Support
Supervised By: Community Living Supervisor or Program administrator
Qualifications: AA Degree or 60 semester hours in a related field of study or 15 semester hours and ones year's related professional experience or high school degree with 3 years experience. Friendly, organized, good verbal and written communication ability. Positive attitude, driver's license and good driving record, and car/ability to drive car/van is a must.
Schedules: Every Wednesday 3 PM - 11 PM and every other weekend -OR- every weekend Friday 7 AM through Monday 11 PM (includes sleep overnights). NO financial compensation is provided for sleep time.
Essential Job Functions
A. Program Planning
• Participate in the completion of assessments.
• Participate in team meetings and staff meetings.
• Implement all plans as written.
• Implement behavior change programs.
• Meet with behavior management consultant as directed.
• Complete all required data on time and accurately.
• Complete all incident reports on time and accurately.
B. Household Management
• Implement petty cash system according to procedures, guidelines.
• Oversee and document resident finances.
• Maintain cleanliness, attractiveness, comfort of home.
• Arrange for repairs on a timely basis.
• Supervise repair personnel on site.
• Ensure all household/resident bills are paid on time.
• Involve residents on above.
• Complete financial records, on time, accurately.
• Complete budget sheets, on time, accurately.
C. Supervision of Assistant Counselors
• Arrange for all staff coverage as needed.
• Participate in screening and evaluations.
• Orient to forms, policies, and procedures.
• Coach while assessing performance.
• Maintain supportive rapport with weekend counselors.
D. Health and Safety
• Arrange and participate in all routine and emergency medical appointments on time.
• Ensure accurate administration of medication and other treatments.
• Ensure nutritious menu planning and meal preparation.
• Encourage and instruct residents in good hygiene and personal practices.
• Include residents in all of the above.
• Complete medication administration forms on time, accurately.
• Follow up and document all medical recommendations and referrals.
• Conduct and ensure proper lifting and transfers of residents. Must be able to lift up to 50 lbs.
E. Community Participation and Leisure Skill Development
• Ensure frequent consistent outings and activities.
• Ensure that residents dress appropriately.
• Ensure that residents are properly groomed.
• Ensure the consistent use of generic community resources.
• Ensure that residents have opportunities to entertain friends, family at home.
• Ensure that residents are provided opportunities to participate in religious activities.
• Ensure that household maintains positive, friendly rapport with neighbors.
• Ensure that household and residents maintain appropriate contact with family members.
• Assist residents in developing appropriate at-home leisure activities.
• Complete monthly activity records on time, accurately.
• Provide an organized approach to activity planning.
• Ensure that each resident has an opportunity to take vacation.
• Ensure that residents receive transportation assistance.
F. Communication
• Develop an interactive and positive relationship with the individuals you are working with.
• Develop ongoing relationships with the individual's family members. Provide family members with current information as they desire.
• Provide the resources to the individuals you are working with to develop relationships outside of their home.

POSITION SUMMARY
As part of the management team of the department, the Community Living Professional Manager provides leadership, supervision, training, and overall coordination of activities at various assigned locations. This assignment is based on the needs of the clients of the organization.
ESSENTIAL DUTIES AND RESPONSIBILITIES
* Affirm the success of team members assigned by recruiting, interviewing, hiring, scheduling, and training new talent including Individual Support Plan (ISP) training: providing constructive and timely feedback through performance evaluations or during everyday interactions to assigned employees; and handling discipline and termination of employees in accordance with company policy.
* Projects a positive image of the Center for Human Services by interacting with staff members, external agencies, parents or guardians, and providers in a professional manner.
* Creates a culture of communication by attending and leading as needed all administrative staff meetings, keeping the management team informed of developments or concerns; working with staff members to "problem-solve" issues or concerns, as they arise and participating in all individuals served ISP meetings or other team meetings regarding individuals served.
* Maintains integrity of records as needed by ensuring Medication Administration Record (MARs) are accurate; writing implementation strategies related to ISPs for individuals served; ensuring discharge summaries are completed for all individuals served by the Community Living Program and reviewing and approving documentation.
* Maintains the health and welfare of the individuals served by administering prescription and PRN medications, as prescribed by physician; collaborating with the Registered Nurse on the individual's medical needs; ensuring physical examinations, are completed annually or as needed; and
* Carries emergency phone, in a rotational basis, as directed by Assistant Director. During this time, the manager must be at least 20 minutes to no more than1-hour drive response time and not be cognitively impaired. Responds immediately to requests for assistance.

QUALIFICATIONS
REQUIRED EDUCATION AND EXPERIENCE:
* Bachelor's degree (BA or BS) from an accredited university and/or a combination of education and direct experience equal to four years. (Direct experience or a combination of education and experience may be considered in lieu of
education.)
* Three years of managerial and supervisory experience in a professional environment
required.
* Two years of experience working in the field of intellectual or developmental
disabilities required.
