PROGRAM RESPONSIBILITIES: Determines participant eligibility for program services in accordance with Agency, State and Federal guidelines and policies and procedures. Formulates and implements nutrition care plans for individual participants utilizing Value Enhanced Nutrition Services, Getting to the Heart of the Matter framework and Participant Centered Education. Provides nutrition counseling to pregnant, postpartum, and breastfeeding women, infants and children participating in the WIC/FMNP programs in accordance with federal and state guidelines and policies and procedures. Completes and maintains required nutrition education documentation, referrals, and other program data as required by the state contract in the StarLINC computer system. Provides required nutrition education contacts to high-risk WIC participants according to state and federal policies and procedures. Completes intake procedures, anthropometrics, and blood work in accordance with state and federal program guidance and regulations. Ensures clinic flow is maintained by assisting other staff as needed with completion of intake, distribution of vouchers and anthropometric measurements. Assists with the design, implementation and evaluation of the program nutrition objectives established annually for the state workplan. Develops and implements nutrition education demonstrations and exhibits that are conducted at all permanent sites and mobile clinic sites. Assists with the preparation, publication, and distribution of the newsletters for WIC and CSFP as assigned. Develops appropriate nutrition education materials for use by WIC and CSFP participants according to nutrition education schedule. Assists with implementation of special projects (i.e., breastfeeding promotion, incentive distribution, Learn the Signs Act Early, Fit WIC, oral health, etc.) and related goals and objectives. Participates in quality improvement activities including completing reviews on participant records and programmatic requirements to ensure compliance with state and federal guidelines; makes efforts to correct all quality improvement findings. Refers participants to appropriate health and social service resources as needed. Completes participant intake eligibility process to maintain clinic flow. Issues electronic benefits to participants and caregivers and provides information on the appropriate use of the vouchers at WIC approved vendors. Assists with set up of each clinic and transports necessary records and equipment to effectively operate clinics. Assists with general clerical duties including filing, preparing for clinic including appointment reminder calls and follow-up on no-shows by telephone and/or writing to reschedule participant appointments. Assists with development of outreach plan and conducts outreach throughout the assigned service are to promote the WIC/CSFP programs. Responds to walk-in and telephone requests for information about WIC/CSFP services and other agency and community services. Develops and maintains knowledge and working understanding of StarLINC system and its applications and completes necessary computer procedures to maintain StarLINC and equipment as required by state. Attends staff meetings and other educational training and conferences for professional development and to maintain licensure or other certifications. Maintains knowledge of federal, state and agency policies and procedures and current nutrition developments and findings effecting women, children, and elderly. Skilled in the use of office equipment, software applications and computer programs. Education: Bachelor's Degree in Food and Nutrition Science, Home Economics or related field required. Master's Degree preferred. If applicable, current licensure as a Registered Dietician from the American Dietetic Association and state of New Hampshire. License/Certification: Successful completion of criminal record check. Valid driver's license with access to reliable transportation for required travel.
