Provides clinical leadership to promote increased compliance with a range of quality and cost measures and standards of care. Manages service delivery of inter-professional and para-professional team members working on an individual case or population of cases. What You Will Do
Manages and provides full scope of advanced nursing practice for targeted patient populations. Evaluates patient responses to therapy / interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes.
Identifies need for new/revised clinical protocols. Collaborates with physicians and others within the practice to develop protocols and provides training as appropriate.
Manages and provides comprehensive advanced nursing care including physical examination, comprehensive history, screening for physical and/or psychological conditions, emergent interventions, pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling and patient education. Establishes a treatment plan based on clinical findings and. Determines when further evaluation by collaborating physician, specialist or emergency care is warranted.
Collaborates with patients, families, primary care physicians and other team members to provide assessment and care planning. Assesses, plans, and provides intensive and continuous care management across client settings.
Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements.
Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required
Valid driver's license, as determined by operational/regional needs may be required
Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans required
Maintains NPI, Medicaid and Medicare provider numbers preferred
Maintains a collaborative practice agreement with a physician in compliance with New York State regulations preferred
Must be certified by ANCC or another accrediting Nurse Practitioner body - in order to bill Medicare and meet credentialing requirements required
For Psychiatric Nurse Practitioners only:
Current PMHNP-BC certification required
Education:
Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) required
Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required
For Psychiatric Nurse Practitioners only:
Master's Degree in psychiatric-mental health Nurse Practitioner required
PhD in psychiatric-mental health Nurse Practitioner preferred
Work Experience:

JOB SUMMARY:
The MRSS Qualified Mental Health Specialist (QMHS) will provide Mobile Response and Stabilization Services (MRSS) to youth (age 2-20) and families who have called the crisis hotline with a family identified crisis. The MRSS Qualified Mental Health Specialist (QMHS) will respond face-to-face (in person or via video conferencing) with another MRSS Team member to high risk crisis situations within an hour to assess the youth and stabilize the family. After the initial 72 hours, the family will receive on-going stabilization and care coordination services from the MRSS QMHS and other members of the MRSS team in various community settings. The team works to provide support - including education and consultation for parents and/or caregivers, case coordination, and symptom management and monitoring for up to 42 days.
This is a Part-Time Position with flexible work hours. 2nd shift availability - (12pm - 8:30pm) is required for this position
ESSENTIAL DUTIES:
Provide mobile response to a family identified crisis within one hour.
Engage youth and families in de-escalating the crisis and assess for risk.
Facilitate hospitalization for clients as necessary.
Assist with the completion of the CANS assessment and help develop a safety plan and a MRSS plan.
Provide ongoing stabilization services with the youth and families for up to six weeks.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
Respect the privacy of clients and hold in confidence all information obtained during the client's treatment according to HIPPA guidelines. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
Recognize the significance of the parent and/or caregiver in the client's life and demonstrate the skills to engage the client, primary family, and other supportive adults in the treatment planning process.
Accept assignment of cases from the supervisor to provide QMHS Services for clients, their families, and/or significant others as needed, including: community organizations, schools, other health and social service professionals, the justice system, and other agencies involved with the client.
Provide QMHS Services including, but not limited to:
development/review of the client's treatment plan.

Responsible for delivery of high quality services and the functioning of the multidisciplinary team through the provision of specific treatment modalities. Under the direction and supervision of the Manager, assumes responsibility for the case management of those clients assigned by the manager. **Essential Functions**

+ Provide accurate and timely documentation of services
+ Utilization review of assigned clients
+ Provide education to community and other team members regarding substance abuse, chemical dependency and tobacco dependency
+ Maintain patient safety standards, departmental policies and procedures, and performance improvement standards

**Job Summary:**
The Addictions Counselor provides direct patient care under the supervision of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health (BSMH).
**Essential Functions:**
+ Responsible for Substance Use Disorder (SUD) group facilitation.
+ Co-facilitates psychoeducational intervention and psychotherapy groups, participating in family meetings and treatment team meetings.
+ Offers patient/family support by providing advice, tools, and community resources.

Counseling Intern (PAID INTERNSHIP)

SUMMARY: To provide a variety of diagnostic and therapeutic services to a varied client population. Services are provided in-home, in-office or within the community. The position is primarily responsible for the provision of direct client care services under a LPCC-S. Provides individual counseling, group counseling, family/couples counseling, and case management and support to clients engaged in outpatient treatment services with the counseling department.
ESSENTIAL RESPONSIBILITIES:
* Conduct psychiatric diagnostic assessments to determine medical necessity.
* Develop, implement, and update Individual Service Plans with the client in accordance with OAC standards and FCS policy/procedure.
* Provide psychotherapy/counseling services to clients through individual, group, and/or family methods.
* Review and update progress, and diagnoses in accordance with OAC standards and FCS policy/procedure to ensure continued provision of proper and necessary services.
* Maintain client records in accordance with OAC standards and F&CS policy/procedure.
* At a minimum, achieve and maintain the Agency's established productivity standards.
* Coordinate and communicate with other community agencies to insure effective inter-agency cooperation.
* Maintain current knowledge of all applicable FCS policies/procedures and function according to these standards.
* Enter and maintain daily schedule into computer as required.
* At a minimum, achieve and maintain the Agency's established productivity standards. Counseling Intern will be responsible to maintain a minimum of 50% productivity.
* Attend and participate in staff meetings, in-service training, supervision, various committees as assigned including Performance Improvement, workshops, and other meetings to receive information and instructions as well as to remain current and accurate on procedures, policies, and techniques.
* Complete all documentation, including consumer outcomes, in a legible and timely manner according to the policies of Family and Community Services and the Ohio Department of Mental Health and provide necessary data for statistical and billing purposes.
* Maintain effective working relationships with co-workers and make every attempt to resolve work-related difficulties with the co-worker in a timely fashion.
* Communicate all concerns and questions regarding the program functioning and client care to the immediate supervisor.
* Maintain current knowledge of all applicable Family and Community Service policies and procedures and function according to these standards.
* Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.

KNOWLEDGE, SKILLS & ABILITIES:

MINIMUM QUALIFICATIONS:
* Education: Bachelor's degree - Required
* Experience: Minimum two (2) years - Preferred along with knowledge of and adherence to all pertinent federal, state and local laws, standards of the Ohio Department of Mental Health, and appropriate professional ethical standards.
LICENSES or CERTIFICATIONS REQUIRED:
* Valid State of Ohio - Counseling Trainee (CT) Licensure
* Valid Ohio drivers license
