The basic function of the Outpatient Care Coordinator is to provide clients with support and linkages to community services designed to restore clients to a basic life responsibility functioning level. Responsibilities Assist clients with housing, educational, social, prevocational, vocational, rehabilitative and / or community services. Apply clients for Medi-Cal or My Health LA benefits as needed. Coordinate auxiliary services to provide individualized connection, referral and linkage to community-based and governmental services. Facilitate necessary transition in SUD / LOCS. Coordinate with physical and mental healthcare providers, and community-based health clinics. Coordinate with state and county entities, such as DPSS, DCFS, Probation, Courts, and housing providers. Create a proactive care plan for clients. Monitor and follow up client care, and respond to changing client needs. Help clients with transitional care. Work to align resources with client needs. Document all care coordination in PCNX. Maintain a minimum of 65% DMC billing productivity per month. Maintain professional rapport with all networking agencies Perform other tasks as assigned by Outpatient Program Manager. Skills / Knowledge Must have excellent computer skills. Must have strong organizational, follow-up and time management skills. Must be able to work well within a team structure. Must have excellent oral and written communication skills. Must be professional at all times. Education / Training Position requires individual be registered or certified with one of the state-approved addiction counselor certification entities (e.g., CADTP, CAADE, or CCAPP).     Support Technician - Bilingual (English / Spanish) Substance Use Disorder Treatment Basic Function The basic function of the Support Technician is to ensure the safety of every client, and to provide leadership and supervision to clients. Responsibilities and Duties Ensure that clients report to their respective groups on time. Supervise and direct clients in the performance of their job assignments. Actively check on clients' completing their assigned tasks. Complete all necessary documentation, including observation logs, maintenance reports, incident / information reports, urinalysis reports, and other reports as assigned. Complete a discharge form for clients who are discharging, and forward the discharge information to all appropriate parties. Conduct routine urinalysis drug testing, skin checks, no-contact personal searches, eye checks, and searches of clients and their belongings. Attend shift change meetings and stay informed of activities occurring on your shift, i.e. new clients, discharges, illnesses, emergencies, etc. Read all communications to the Support Technicians, and review communications. Stay current on policies and procedures. Chart on at least three clients per shift. Tour the facility on a continual basis, ensuring that a clean, safe environment is maintained and report all issues that might compromise the maintenance of a safe, secure and clean facility to the Chief Support Technician. Perform other tasks as assigned by supervisors. Responsibilities Keep a schedule of regular counseling appointments with clients and be available for non-scheduled counseling or crisis counseling when needed. Develop treatment plans, based on the initial ASAM assessment conducted at intake, including goals and objectives. Write progress notes and other relevant clinical documentation, as clinically necessary and at least once weekly, detailing clients progress, or lack thereof, towards completing his or her treatment plan goals. Facilitate groups sessions and educational classes as assigned by Program Manager and document accordingly in each clients file within 48 hours. When necessary, update ASAM assessments for clients. Attend weekly Case Conferences to discuss clients progress in relation to their treatment plans. Identify problems or achievements through staff discussion. Conduct and document case management between clients and other service agencies involved in clients treatment, including but not limited to: Probation, Parole, DCFS, DMH, DPSS, DOR, along with referring case managers and insurance providers. Attend, and actively participate in, monthly in-service training sessions. Provide each client with an individualized Continuing Care Plan. Complete all necessary documentation within contractually established timeframes: this includes P.O. reports, progress notes, discharge summaries, aftercare plans, etc. Provide family support and education through conjoint sessions as well as family groups. Communicate and coordinate effectively with the treatment team regrading client treatment issues. Maintain a minimum of 65% DMC billing productivity per month. Perform other duties as assigned by the Program Manager. Skills / Knowledge Must be able to accurately document clients individual sessions and group participation. Must be able to communicate effectively both orally and in writing. Must have knowledge and ability to use counseling techniques to educate, elicit feelings, facilitate self-understanding and motivate the client. Must be skilled in using Evidence Based Practices, including CBT and MI. Must have the ability to act as Case Manager, coordinating a continuum of treatment and / or services for clients. Due to the program philosophy, all clinical staff must have a good understanding of the Narcotics Anonymous and / or Alcoholic Anonymous programs (the Twelve Steps). Basic Function The Accounting Manager oversees the daily operations of the accounting department and ensures accurate and timely financial reporting in compliance with GAAP, nonprofit regulations, and internal policies. The Intake Specialist at our Substance Use Disorder Treatment Center conducts screenings and facilitates client admissions into CRI-Help's residential programs. Responsibilities Handle inquiries from individuals seeking treatment and treatment information. Schedule and conduct screenings to determine individual's eligibility into the program. Assess applicants to determine level of care and the appropriateness of the program for the individual. Admit client into treatment, accurately completing all required documentation, in a timely manner. Utilize assessment tools (including, but not limited to, ASAM, CalOMS, Mental Status Assessment, Suicide Risk Assessment, Brief Trauma Assessment, PrePare, Tobacco Assessment, etc.) during the intake process. When identifying “red flags,” assess incoming clients carefully, and provide crisis intervention when necessary. Provide case management services, as needed. Utilize the Client Orientation Handbook, when coordinating new clients' admission into our programs. Introduce prospective clients to Twelve Step programs. Work directly with other CRI-Help staff members (including Clinical Director, Nurses, Technicians, etc.) in order to provide the highest level of care for clients. For private insurance, verify benefits and eligibility, and conduct pre-certifications. Verify Medi-Cal benefits, and be able to understand Master Aid Codes. Assist clients applying for Medi-Cal benefits. Search clients' belongings. Conduct client urinalysis tests. Provide referral sources when needed. Basic Functions Reporting to the CEO, the Director of Client Placement & Admissions is responsible for creating and maintaining efficient systems for clients accessing substance use disorder treatment services at CRI-Help, ensuring timely clinical assessments for admissions to CRI-Helps residential, residential withdrawal management, and outpatient programs. Responsibilities Provide leadership to the Intake Department and cultivate the function of the Department as an integrated team. Perform supervisory duties, including recruiting, hiring, training, coaching, and evaluating Intake Department staff. Train all staff members who complete admission inquiries. This includes but is not limited to over-the-phone screening, answering pre-admission/general intake questions from potential client, families, or their referral sources, scheduling intake, gathering and then communicating insurance information. Supervise and train all Intake Specialists who are responsible for conducting clinical assessments. Responsibilities Keep a schedule of regular counseling appointments with clients and be available for non-scheduled counseling or crisis counseling when needed. Develop treatment plans, based on the initial ASAM assessment conducted at intake, including goals and objectives. Write progress notes and other relevant clinical documentation, as clinically necessary and at least once weekly, detailing clients progress, or lack thereof, towards completing his or her treatment plan goals. Facilitate groups sessions and educational classes as assigned by Program Manager and document accordingly in each clients file within 48 hours. When necessary, update ASAM assessments for clients. Attend weekly Case Conferences to discuss clients progress in relation to their treatment plans. Identify problems or achievements through staff discussion.
