Transporter - Valet Supervisor: Facilities Director * High School Diploma/GED * Must be 21 years old or older * Must have current valid Driver's License in the state of residence. The YHDP (Youth Homelessness Demonstration Project) Transitional and Rapid Rehousing Case Manager provides case management services to homeless and formerly homeless young adults (18-24) utilizing the Transition in Place housing model to facilitate rapid exit from shelter. The YHDP program uses evidenced based best practices such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, and Housing First. The case manager will work with participants to recertify program eligibility, conduct home visits, link to long-term supports, and follow up to assist in maintaining their housing and independence.
