Primary Purpose: In partnership with the Fairfield County Coordinated Access Network (CAN) and other community stakeholders, New Reach provides supportive housing services to individuals and families who, without supports in these critical times of transition and need, are at imminent risk of homelessness or are actively experiencing homelessness in the greater Bridgeport community. The services are provided primarily to low-income individuals and families experiencing homelessness and housing instability in the greater Bridgeport area, who are affected by significant and persistent mental health, substance use, or HIV/AIDS. The housing caseworker acts as the first point of contact to provide support and advocacy, housing counseling, and community linkages with the ultimate goal of housing stabilization. Other populations or housing support services may be assigned as secondary purposes when needed. Essential Functions: * Accept referrals from local and regional sources including but not limited to 211, the Coordinated Access Network (CAN), community medical case managers. * Complete program intakes, including initiating contact with referred households, description of program services offered, and a comprehensive, strengths-based assessment of needs to stabilize housing and health. * Co-create person-centered goal plans with clients and other parties involved in services. * Provide home and community-based case management services to address unmet housing and health needs through direct support, coaching, mentoring, crisis intervention, life skills training, advocacy, and linkages to community resources. * Engage with property managers/owners to provide information on service components, advocate for housing rights, and mediate housing concerns effectively. * Assist clients with stabilizing housing, including affordable housing search, tenancy skill building and responsibilities, fiscal preparedness for independent rent payment, and housing maintenance. * Assist clients with identifying unaddressed needs and encourage an array of integrated support/services related to benefits, employment and income, education, housing, medical conditions, mental health, substance use, HIV, family and/or community-based supports, and access to mainstream services. Refer and support engagement in appropriate services to meet the household's needs. * Work collaboratively and maintain positive, professional communication with program partners including client's household members and natural support networks, property owners/managers, housing authorities, legal entities, and community providers to support housing retention and stability. * Utilize the statewide HMIS database, Google Sheets, and Smartsheet to document HUD assessments, case notes, services, financial requests, and client outcomes in compliance with standards and expectations. * Maintain timely and accurate documentation for all client files per agency policy and procedures and funder requirements, including assessments, goal plans, client services, referral outcomes, housing inspections, rental assistance, and case notes. * Participate in internal, local, regional, and statewide meetings as assigned. * Develop a strong community profile with the homeless services, mental health, substance use, and/or HIV/AIDS community as an advocate for clients. * Other duties as assigned to assure successful operations. Qualifications: * BA/BS in Human Services or related field, OR associate's degree and 2 years of human services experience, OR recovery support specialist certification with 2 years of human experience. * Valid driver's license and access to reliable transportation with proof of insurance. * Highly motivated, with excellent interpersonal, oral, and written communication skills. * Ability to provide intensive and comprehensive services to households experiencing homelessness, including advocacy and mediation skills. * Ability to work independently and as part of a team, to support the mission of New Reach. * High level of organizational skills to prioritize and manage multiple projects simultaneously. * Proficiency in Microsoft Outlook, Word, Excel, and Power Point. Purpose Under the direction of the Director of Facilities, the Facilities Technician will be the point person in the absence of the Facilities Manager. The Facilities Technician collaborates with the Facilities Technicians, Environmental Service Workers and with Program Staff to safely implement the repair and maintenance of physical assets, grounds, facilities, equipment and utilities; to monitor the security of occupants and property, and to supervise building occupants and others pursuant to programs and operations to ensure goals are achieved and that the property and improvements are clean, safe, healthy, and may be fully used productively, comfortably, economically and safely, at all times by residents, staff, volunteers, visitors, and others. Facilities Technicians are mindful of the clients served by Columbus House and seek to maintain a physical environment that reflects the core values of Trauma Informed Service: Safety, Trustworthiness, Choice, Collaboration, and empowerment. Duties and Responsibilities Complete daily, weekly, and monthly checklists, maintain records of scheduled procedures, and identify and recommend improvements pursuant to maintenance, security, and programs and operations. Complete all required safety training on time as assigned by the company. Replace and repair building finishes such as ceiling tiles, door hardware, and can paint walls, piping, floors, and equipment etc. Perform minor repair and maintenance of all physical assets, buildings, facilities, grounds, and utilities. Inspect physical assets, buildings, facilities, grounds, and utilities and identify items in need of maintenance or repair; ensure assets and facility occupants are safe and secure. Clean and maintain grounds including but not limited to grass cutting, shrub pruning, snow removal and de-icing. Reliably and safely transport other staff, materials, supplies and tools as required throughout the state of Connecticut. Cleanup after yourself when completing a project. Assist with Environmental Services when required. Use the EMAINT work order system. Complete all required documentation and reporting. Respond immediately to emergency situations and customer concerns. Monthly on-call responsibilities. Use your own vehicle and have your own tools. On-call responsibilities. Primary Purpose: The Program Manager will oversee the day-to-day operations and services implementation of the Greater New Haven Youth (18-24 years old) Rapid Rehousing and Diversion teams. The Program Manager will supervise, train, lead staff, and coordinate, and ensure that all services are being delivered according to program specifics and in compliance with funder requirements. Essential Functions: * Oversee and manage all aspects of operations and service delivery of the Greater New Haven Youth Rapid Rehousing Program and Diversion programs, including but not limited to support needed for individuals and families. * Oversee and manage all aspects of the implementation of evidence-based models within program service delivery , including but not limited to Critical Time Intervention (CTI), Motivational Interviewing and person-centered planning * Support discussions, execution and successful completion of the organization's annual Strategic Planning directives and objectives * Conduct and/or participate in community training and meeting facilitation as a representative of NR to enhance community knowledge and skills associated with homeless service delivery and evidence-based practices * Prepare monthly, quarterly, and annual reports for agency and funding sources as required by contract and agency policy * Monitor and manage compliance of agency data implementation, tracking and dashboard metrics created to support the successful operations of services and annual goals * Advocate to the Chief Program Officer & Executive Leadership Team the needs of programs, including programmatic needs and operational needs; monitor and revise program policies and procedures, in coordination with other staff and community partners as necessary * Ensure compliance with all accreditation, certification and regulatory standards * Supervise staff per agency supervision standards, orient and train all staff to ensure that job responsibilities are being performed in a quality manner, consistent with policy and enforce disciplinary actions when necessary * Responsible for hiring, verification of time worked, monitoring and approval of PTO time, and annual performance evaluations of staff * Ensure the quality of data maintained in the Homelessness Management Information System (HMIS) database. Ensure maintenance and accuracy of all client records and program statistics/outcome measures * Act as NR's representative at community meetings and collaborative; create awareness; educate policy makers on issues relevant to the need of housing for families with special needs * Maintain 24-hour availability in the event of an emergency * Assist with special projects and complete other duties as assigned Qualifications: * Master's Degree in related field or BS/BA in related field and 3-5 years' management experience required. * Critical Time Intervention Instructor Certified Preferred. * Program administration/development skills required; with a strong ability to lead. ยท Experience managing housing, service programs, home visitor, and other similar programs. * Experience and education on issues of substance abuse, mental illness and homelessness; desire to promote equal access and opportunity for at-risk/homeless persons. * Highly motivated, creative with excellent interpersonal skills; strong advocacy skills.
