Compensation: $100,000 - $105,000 plus a 15% bonus potential!
GENERAL SUMMARY:
The Resident Health Services Director is responsible for evaluating residents needs and assisting residents in navigating the available services that create the bridge between residential and assisted living and nursing care. This person is responsible for the programming designed to meet these needs and to develop future programs to help meet the wellness needs of the residents (including but not limited to intellectual, occupational, social, environmental, spiritual, emotional, nutritional and physical).
PRINCIPLE DUTIES:
* Provide nursing evaluation of the residents, coordinate additional care, and communicate with medical personnel and family members, and follow-up after the emergency.
* Coordinate the residents return including transportation, medications and a light meal upon return. Update plan of care
* Provide individual and group educational opportunities for residents and staff in the area of disease prevention and management in the holistic spirit of the Lifestyle and Health Services Wellness Program.
* Facilitate with other leaders in the continuum of care to identify and recommend additional services or a physical move within the continuum of care for residents whose needs have changed.
* Anticipate and coordinate with the resident and family members when a significant change in the residents condition necessitates additional support services or a physical transfer to a higher level of care for residents whose needs have significantly changed.
* Assist residents and family members by offering support in dealing emotionally with the aging process and illnesses.
* Provide day-to-day oversight of medication technicians and LVN staff to ensure standards are being met for delivery of these services to the community residents. Manage clinic services to include but not limited to pre-admission health screening for residents
* Support for the management of chronic conditions, coordination of clinical visits by other professionals, and assistance with medications and ADLs.
* Responsible for the interviewing, selection, training, supervision and performance management of all residential health services staff.
* Responsible for ensuring all staff training is conducted, completed and documentation of training is maintained.
* Other duties as assigned by supervisor.
QUALIFICATIONS:
* Current California Licensed Vocational NurseLicense in good standing with the LVN Board of Nursing.
* RCFE preferred
* CPR, First Aid, and AED required
* Five years experience working with seniors in long-term care, home health or other health care setting is preferable.
* Five years organizational or management experience; supervising others is preferable.
* Must have a passion for working with the elderly, possess strong clinical skills, and have strong customer service, leadership, and interpersonal skills.

ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature.
ACT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week.
Overview
The LicensedPracticalNurse (LPN) functions as a primary practitioner who assesses and monitors ACT participants' clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assist individuals in their achievement of mental health, medical, rehabilitation and recovery goals.
